It is now known that many infants with this disorder who are treated with ablative therapy in a timely fashion will avoid this serious outcome. It is essential that those caring for premature infants know who is at risk of retinopathy of prematurity, when screening must begin and how often these infants need to be examined. It is also important to know when to treat those infants who develop severe retinopathy of prematurity and what long term follow-up is needed to manage other complications of retinopathy of prematurity. Table 1 provides answers to these questions. The background data upon which these recommendations are based are found in the accompanying article, Paediatrics and Child Health The recommendations are based on currently available information. Further research is needed in the following areas: systematic reviews of new therapies (eg, surfactant) and their impact on ROP; prevention of ROP; randomized trials of the currently available surgical treatment for ROP; and a Canadian surveillance system for ROP.

Alex, Andrew and Robbie Alcorn were born fourteen weeks early; each weighed less than two pounds. Together, they weighed only a little more than a five-pound bag of sugar. Their parents, Michele and Bob Alcorn, spent the next weeks holding vigil over their sons in the intensive-care nursery of a Cleveland, Ohio hospital. When the triplets were about six weeks old, a doctor examined their eyes for signs of retinopathy of prematurity, or ROP, a potentially blinding eye disease that is common among very low birthweight babies. The Alcorns were devastated to learn that all three sons had developed ROP, and Robbie's disease was quickly progressing. In its mildest form, ROP is self-correcting and causes little, if any, damage to the eye. At its severest, the disease results in significant vision loss. Of the more than 400,000 premature infants born each year, about 600 will go blind from ROP. "The fact that Robbie could go blind and Alex and Andrew could also have serious eye problems was initially incomprehensible," Michele says. "I went completely numb. I kept asking the doctor over and over to explain it all." How ROP Develops During the last trimester of pregnancy, a baby's eyes are actively developing, particularly the blood vessels within the retina (the inner lining of the eye that receives visual images). When preterm birth occurs, the blood vessels briefly stop developing and are exposed to room air. The vessels may then resume growth in an errant pattern.

Prematurity Retinopathy of prematurity or ROP is a retinal disorder found exclusively in premature infants. The incidence of ROP increases for younger and smaller babies. It is normally not seen in infants weighing more than 1500 grams or about three pounds five ounces. With ROP, the normal blood vessels of the retina, which is the nerve tissue in the back of the eye, do not complete their growth until the end of a normal full term pregnancy. In very small babies, the blood vessels may stop growing when they are born. When they resume their growth, it can be in an abnormal fashion which causes damage to the retina. A screening examination schedule has been determined to aid in detecting ROP and prevent its progression when possible. Eye examinations are usually performed in the hospital nursery with the use of eye drops to dilate the pupils so that the retina can be examined. If any ROP is found, follow-up evaluations are recommended over the following weeks at intervals determined by the stage of the disease. In most cases, the ROP will resolve spontaneously as the blood vessels complete their growth pattern. In some cases, the ROP reaches a certain stage for which treatment becomes necessary. This treatment is in the form of laser therapy to the retina. It has been shown that treatment at a certain stage of ROP can decrease, though not eliminate the chances of retinal damage and subsequent poor vision.

First, a little on how the eye is put together. The front of the eye has the small black circular opening called the pupil, which is where the light we see gets into the eye. Right around the pupil is the iris, the part of the eye that can be blue, brown, or whatever one’s "eye color" is. The eyeball is indeed a ball, most of which is hollow and filled with a clear jelly-like substance. The most of the inside surface of the eyeball is the retina. The retina’s job is to take the light, (which has come in the pupil and passed through the clear "jelly"), and change the information in that light into electrical information, much like a television camera does. This electrical information goes back to the brain for processing via the optic nerve. The optic nerve goes out the back of the eyeball and functions as a wire carrying the electrical information.

Q: What is retinopathy of prematurity (ROP)? A: Babies who are born prematurely still have a growing retina . The retina usually finishes growing a few weeks to a month after birth in full term babies, but in premature babies the retina is still growing. During the course of this growth, the blood vessels that bring blood to the retina can begin to develop abnormally. This abnormal growth is called retinopathy of prematurity. Many factors interact to cause retinopathy of prematurity. We do not understand all of the causes at present. This condition mainly occurs in the eyes of premature infants. The retina is the part of the eye that receives light. The retina also turns light into pictures that are sent to the brain. The retina is like the film of a camera. The blood vessels that supply the retina are one of the last parts of the eye to develop. Some premature infants have immature retina.   • The abnormal blood vessels may heal completely during the first year of life.
  • The abnormal blood vessels may partially heal. These infants may be nearsighted (myopia) and need glasses. These infants may develop lazy eye (amblyopia) or wandering eye (strabismus). Infants with these conditions should have regular eye exams to check vision.
  • Scar tissue may form and cause some vision problems that are not totally correctable. The scars are called retrolental fibroplasia (RLF). Severe RLF may cause the retina to pull away from the back of the eye (retinal detachment). This may cause serious loss of vision and sometimes blindness. However, this is rare.

Revised: What is Retinopathy of Prematurity? Retinopathy of Prematurity (ROP) is a developmental disease of the eye that affects premature infants. When a baby is born, the retinal blood vessels have not completed their development. In patients with ROP, the blood vessels stop growing and new, abnormal blood vessels grow instead of normal retinal blood vessels. The developmental arrest and blood vessel maldevelopment may be temporary or permanent, minimal or severe. The most severe complication of this disease is bilateral blindness in early childhood.
