RETINOBLASTOMA What is Retinoblastoma?
Retinoblastoma is the commonest eye cancer in infancy and childhood, affecting one in seventeen thousand live births. The retina is the living photographic film in the back of the eye. Retinoblastoma means tumours in the retina.
It occurs in infants and children up to five years old and, if diagnosed early, 95% of children with retinoblastoma survive. Retinoblastoma can appear in one or both eyes. What causes Retinoblastoma?
The cause is unknown. What is known is that the child's genetic material (DNA) plays a major part. If both eyes are affected there is a 50% chance that the disease will be passed on to the child's offspring. In these cases genetic counselling is important. If only one eye is affected there is no significant risk of the disease being passed on to the next generation unless others in the family are affected. What should I look for?
Retinoblastoma can show up in several ways. As the young infant can't communicate, the parents need to watch for anything unusual about the child's eyes.
Flashlight photos may reveal a "cat's eye reflex". A normal retina reflects a red colour in a photo taken using a flash (cameras without red-eye reduction). If the child has the tumours they will reflect a white light instead. Look at flash photos carefully to see if the red reflex in each eye is the same. A squint (the child's eyes do not line up properly) or poor sight can be the first sign of retinoblastoma. This may happen if the tumours grow across the reading centre of the eye.

What is Retinoblastoma?

I state here, on my page, that I'm not a doctor. I want to make sure everyone knows that. I am, as stated elsewhere, in the medical field, and I've done some research on the subject of Retinoblastoma. Here's my explanation of what Retinoblastoma is. For the most, up-to-date and late breaking information, try one of my LINKS
Retinoblastoma (RB) is a term which refers to the genetic condition causing tumors to grow on the Retina of the eye. Unilateral RB simply means that tumors have grown in only one eye. Unilateral might not be caused by genetic, but rather a spontaneous growth. Bilateral RB is when a tumor or tumors have grown in both eyes and usually indicates a genetic influence (either spontaneous mutation or inherited).
If you break Retino-blast-oma into it's separate components you can begin to get an idea of what it's about.
  • Retino refers to the Retina which is a part of our eye (see diagram below).
  • Blast is a medical term used to indicate that the tumor is derived from the "primitive" cell. Any tumor name with "blast" in it indicates that it's from the "infantile" cell, and almost always occur in children, before their cells mature.

Alternate Names : Cancer - Retina, Tumor - Retina Definition Retinoblastoma is a malignant tumor (cancer) of the retina (part of the eye) that generally affects children under the age of 6.
Eye Retinoblastoma occurs when a cell of the growing retina develops a mutation in the RB gene (a tumor suppresser gene). This mutation causes the cell to grow out of control and become cancer. Sometimes this mutation develops in a child whose family has never had eye cancer, but other times the mutation is present in several family members. If the mutation runs in the family, there is a 50% chance that an affected person's children will also have the mutation and a high risk of developing retinoblastoma.
One or both eyes may be affected. A visible whiteness in the pupil may be present.

Retinoblastoma
Retinoblastoma is a rare cancer of the eye. It may be hereditary, and one-third of the cases involve both eyes. Retinoblastoma often can be seen by looking at the young person's eye but is usually diagnosed by an examination under general anesthesia using an ophthalmoscope, an instrument used in examining the interior of the eye. The disease tends to remain localized for long periods, but in advanced stages, it can metastasize, or spread to other parts of the body. x-rays, bone marrow examination, MRI, and a bone scan can be done to check for metastases. If diagnosed early, it is possible to destroy the tumor with radiation therapy and preserve normal vision. If the tumor is so large that there is no hope of maintaining useful vision using radiation, the eye is removed. In cases where both eyes are involved, an attempt is made to preserve vision in both eyes through treatment with radiation. When advanced disease is found in both eyes, an attempt is made to preserve vision in at least one eye. Whenever there is any possibility of useful vision, all efforts are made to preserve it. Childhood Retinoblastoma Retinoblastoma is a cancerous tumor of the eye. It is the most common eye cancer of childhood and occurs in both hereditary and non-hereditary forms. The disease primarily affects very young children and may be present in one or both eyes. There may be one or more than one tumor in either eye. The anatomy of the eye The cornea is the convex outer portion of the eyeball that transmits light to the retina. Behind the cornea is the iris, which is the colored portion of the organ. The iris controls the amount of light entering into the eye by making the opening at the center, called the pupil, either larger or smaller. As light rays pass through the curved surface of the cornea, they are bent and then passed through the pupil. Inside the eye, sitting behind the pupil, is a disc-shaped structure called the lens. The lens is clear in the healthy eye, and has two curved surfaces which refract the light two more times on its journey to the back of the eye. As light rays are passed from the front to the back of the organ, they pass through a colorless, jelly-like material, called the vitreous, that fills the eyeball behind the lens. The inside lining of the eyeball consists of a structure called the retina. It is the retina that converts light energy into electrical impulses that are transmitted to the brain through the use of its sensory receptors, the rods and cones. The retina is a collection of nerve tissue in the eye and its job is to form the images that we see. The retina cells in babies and very young children are called retinoblasts. Retinoblastoma originates in the retina of the eye.

The tumor is composed mainly of undifferentiated anaplastic cells blasts that arise from the nuclear layers of the retina. It is the most common eye tumor in children and it usually occurs before the age of five. It can occur in one eye (unilateral) or in both eyes (bilateral).
About Retinoblastoma
Highly recommended: Retinoblastoma on the cancer.gov site In about 40% of the cases, retinoblastoma is hereditary. In fact, retinoblastoma was one of the first cancers recognized as "hereditary". Before surgery, children who had retinoblastoma died before they reached childbearing age. When doctors in the first part of this century began removing the affected eye to treat the disease, the children survived the cancer and lived to have their own children, some of whom in turn developed retinoblastoma. In the 1970s, researchers studied the DNA of retinoblastoma carriers and found one of the first "oncogenes". They found that deletions or mutations of the "retinoblastoma" gene within the q14 band of chromosome 13 are highly associated with a predisposition to retinoblastoma. Several sites already exist on the Internet which describe retinoblastoma (links just below). On the National Retinoblastoma Research and Support Foundation site (NRRSF), there are pictures of what the disease looks like.
