Restless legs syndrome
Definition
Restless legs syndrome (RLS) is characterized by unpleasant sensations in the limbs, usually the legs, that occur at rest or before sleep and are relieved by activity such as walking. These sensations are felt deep within the legs and are described as creeping, crawling, aching, or fidgety.
Description
Restless legs syndrome, also known as Ekbom syndrome, Wittmaack-Ekbom syndrome, anxietas tibiarum , or anxietas tibialis , affects up to 10-15% of the population. Some studies show that RLS is more common among elderly people. Almost half of patients over age 60 who complain of insomnia are diagnosed with RLS. In some cases, the patient has another medical condition with which RLS is associated. In idiopathic RLS, no cause can be found. In familial cases, RLS may be inherited from a close relative, most likely a parent.
Causes and symptoms
Most people experience mild symptoms. They may lie down to rest at the end of the day and, just before sleep, will experience discomfort in their legs that prompts them to stand up, massage the leg, or walk briefly. Eighty-five percent of RLS patients either have difficulty falling asleep or wake several times during the night, and almost half experience daytime fatigue or sleepiness. It is common for the symptoms to be intermittent. They may disappear for several months and then return for no apparent reason. Two-thirds of patients report that their symptoms become worse with time. Some older patients claim to have had symptoms since they were in their early 20s, but were not diagnosed until their 50s. Suspected under-diagnosis of RLS may be attributed to the difficulty experienced by patients in describing their symptoms.

The appointment was for one George Mitner, age 65. He came with his wife Sophie, who is a few years younger. Mr. Mitner's referral to a sleep specialist was typically convoluted. On paper his internist, Dr. Randolph, made the referral. What actually happened was this: His wife had offered the above complaint on a visit to her physician, Dr. Epstein, who then said: 'Why don't you have your husband see Dr. Martin?' To see me under his insurance plan Mr. Mitner had to be referred by Dr. Randolph, whom Mrs. Mitner called. She spoke with Dr. Randolph's nurse, who wrote out the referral form (after consulting with Dr. Randolph). Thus at the time of his visit to me, no physician had actually seen Mr. Mitner for his wife's complaint. The ultimate trigger for the referral was the phrase "during sleep." Anything related to these two words has become, by default, the province of the sleep specialist apart from conditions obviously related to heart or lung disease, such as a heart or asthma attack. Almost any general complaint will do. You could substitute "he kicks me" with "he snores" or "he dances" or "he talks of cabbages and kings" and a sleep referral would likely (and appropriately) follow. That's what sleep specialists are for.

Restless legs syndrome (RLS) is a common disorder characterized by uncomfortable sensations in the legs which are often described as pulling, crawling, tingling, prickly, and sometimes painful. Symptoms are more pronounced in the evening or at night, and during periods of inactivity such as long car trips and long-distance flights. The sensations are associated with an urge to move the legs which may give temporary relief. Patients will keep moving their legs, pace the floor, and toss and turn in bed. The cause of RLS is unknown and there is no cure. RLS affects women slightly more than men and, although it may begin at any age, most patients are middle-aged or older. RLS causes difficulty falling asleep and staying asleep, and most patients report periodic leg jerks during which cause repeated awakening and disrupted sleep. If untreated, patients experience severe daytime sleepiness or fatigue which can impair daytime function and concentration. RLS has been associated with iron deficiency anemia, kidney failure, diabetes and peripheral nerve disease. Pregnant women may experience RLS, especially in their last trimester, with symptoms usually disappearing within 4 weeks of delivery.
