Diagnosis of Restless Legs Syndrome
How is Restless Legs Syndrome Diagnosed? The diagnosis of RLS is based upon a thorough medical and neurologic evaluation as well as a detailed patient history. Some physicians may use a clinical assessment scale to aid in the diagnosis of RLS and help to determine disease severity. Diagnostic indicators include the following:
  • Desire to move the limbs usually in association with paresthesias dysesthesias Motor restlessness. Symptoms are worse or exclusively present at rest (i.e. lying, sitting) with at least partial and temporary relief by activity. Symptoms are worse in the evening/night.
Are there other factors that physicians consider when confirming a diagnosis of RLS? Additional features that may suggest RLS may include the following:
  • Difficulty initiating and maintaining sleep. Involuntary movements of the legs (or arms) that occur during sleep or while at rest. Absence of associated neurologic findings as revealed during a complete neurologic evaluation. Most cases of RLS are

What is restless legs syndrome (RLS)?
Restless legs syndrome (RLS) is a sleep disorder in which a person has unpleasant feelings or sensations in the legs. These feelings are described as creeping, crawling, tingling, pulling, or painful. While these sensations happen most often in the calf or lower leg area, they can be felt anywhere from the ankle to the upper thigh. RLS symptoms can occur in one or both legs and can also be felt in the arms. These symptoms occur most often when lying down, but can also occur when sitting for long periods of time, such as at a desk, riding in a car, or watching a movie. People with RLS talk about having an irresistible urge to move the legs. Moving the legs, walking, rubbing or massaging the legs, or doing knee bends can bring relief, at least for a short time. Unlike other conditions, RLS symptoms get worse when relaxing or lessening activity, particularly during the evening and nighttime sleeping hours. Many people with RLS have trouble falling asleep and staying asleep. If not treated, RLS can cause extreme tiredness and daytime

What is Restless Legs Syndrome?
If you have restless legs syndrome (RLS), you may recognize these symptoms:
  • An urge to move the legs, often accompanied by uncomfortable sensations in the legs, usually described as a creeping or crawling feeling, but sometimes as a tingling, cramping, burning or just plain pain. Some patients have no definite sensation, except for the need to move. (The arms may also be affected, but that's much less common.)
  • The need to move the legs to relieve the discomfort, by stretching or bending, rubbing the legs, tossing or turning in bed, or getting up and pacing the floor. Moving usually offers some temporary relief of symptoms.
  • A definite worsening of the discomfort when lying down, especially when you're trying to fall asleep at night, or during other forms of inactivity, including just sitting.

Many of these people do not know exactly what it is that causes the creepy, crawly feelings in their legs, arms or back that keeps them up at night or prevents them from sitting for long periods of time. Although there is much known about Restless Leg Syndrome (RLS) in the medical world, the general public does not know much about it.
The doctors do not know what causes it or how to cure it. But many people are out there looking for a way to make the feelings stop. I know, because I am one of those people. RLS has not only disrupted my life, but also my husband's. This syndrome has put a stress on our marriage, and has made it very difficult for me to work. My life has been turned upside down by what many people would call a harmless disease.

The condition is surprisingly common and often associated with periodic limb movement disorder. Symptom severity can range from annoying to virtually disabling. Recent advances in definition, evaluation, and treatment have made it possible to provide significant relief to many misdiagnosed, overlooked, or undertreated patients.
Dr. Phillips is Professor of Internal Medicine, Division of Pulmonary and Critical Care Medicine, University of Kentucky College of Medicine, Lexington. She is a recipient of a sleep academic award from the National Institutes of Health. Restless legs syndrome (RLS) is a neurologic disorder characterized by four clinical features: 1) dysesthesias, or uncomfortable sensations in the extremities that are 2) increased with inactivity and relieved with movement, 3) worsened at night, and 4) associated with motor hyperactivity. RLS is identified and diagnosed clinically, much as depression and pain are. Unfortunately, there is no specific biologic marker. Diagnostic criteria from the International Classification of Sleep Disorders are given in Table 1.

What is RLS?
Does This Sound Familiar? When you lie down, do you have tingly or unpleasant sensations in your legs causing you to have a strong desire to get up and move around? If so, and you experience other symptoms like a restlessness in your legs at night and a feeling of fatigue during the day, you may have restless legs syndrome.
What is PLMD?
Many people with RLS also experience a related sleep disorder called Periodic Limb Movement Disorder (PLMD). PLMD causes involuntary jerking movements during sleep. The movements range from small shudders of the ankles and toes to kicking and flailing of the arms and legs. Sometimes, oral, nasal and abdominal movement goes along with PLMD. The movements generally occur every 5 to 90 seconds and last between 0.5 and 10 seconds. The periodic jerking often wakes the individual (as well as their sleeping partner) and significantly disturbs their quality of sleep.

  • Overview . Restless legs syndrome (RLS) is a common condition afflicting between 5% and 15% of the population. It is characterized by dysesthesias and restlessness of the legs and may be associated with other medical conditions. RLS is treatable, although therapy must be individually tailored.
  • Signs and Symptoms
  • Unpleasant limb sensations . Sensations are described as crawling, creeping, aching, jittery, or fidgety in nature. Legs are much more commonly effected than the arms (hence the name). Sensations are precipitated by rest Compelling motor restlessness . Movement of the limb may briefly relieve the sensation. Worse at night . Sometimes occurs in the evening or while riding in a car. Periodic limb movements of sleep . Patients with RLS often have semirhythmic flexion/extension movements of the foot, which occur every 20 to 40 seconds during non-REM sleep.

    What is Restless Legs Syndrome?
    A "syndrome" is a medical condition that is categorized by a multitude of symptoms. Restless legs syndrome (RLS), also called Ekborn's Syndrome, is a sensori-motor (movement) disorder characterized by uncomfortable sensations in the legs. The uncomforatable sensations are worse during periods of inactivity or rest or while sitting or lying down. Restless Legs Syndrome varies in its severity from mild causes causing a certain amount of discomfort to severe forms that lead to significant distress and sleep disturbance. Restless Legs Syndrome can develop at any age but it is more common in the more elderly population. Restless legs syndrome is a life-long condition that cannot be cured. Symptoms may gradually worsen with age, and their most disabling feature is the sleep onset insomnia they cause, which can be severe. In rare cases, the arms may also be affected. Symptoms?
