Restless Leg Syndrome

I have a big problem with restless leg syndrome, actually not only legs but in the shoulder and elbows too. I understand that there is not a great deal that can be done for it. The pharmacist in our drug store has the problem and he is at a loss as to what to do for it. While taking Roxicet for some discomfort for cracked ribs, I discovered that it helps the spasms considerably. I only take it at night. Hot baths help too, but it is annoying when I have to take hot baths at 2 or 3 o'clock in the morning. Do you have any information on this condition?
A: Restless leg syndrome, (RLS), is one of those very annoying problems that is not life threatening but drives a person crazy. (It seems like many of the questions I get for the column fall under that category!) Although fairly common, affecting about 10-15% of people, RLS is poorly understood. Patients describe very unpleasant sensations as being creepy-crawly, jittery, tingly, or burning pain. These feelings occur mainly in the legs, especially the calves, but occasionally the arms may also be involved. Lying in bed or sitting in a car or theater can bring the symptoms on, and moving around, such as stretching, pacing the floor, or cycling seems to provide some relief. Massaging the legs or hot baths also may be soothing.

Recently I spent the month of August visiting my children and friends in Madison and Hartland (I live in Littleton, North Carolina), and I read the Milwaukee Journal-Sentinel everyday. In doing so, I found your column that discusses restless leg syndrome. That troubles me about one night a week. I read another article about this syndrome that told of a tonic called Dr. Johnís for Quiet Leg. I asked my son-in-law, a pharmacist, and my doctor, previously a surgeon in Philadelphia now practicing General Medicine at the Littleton Clinic, their opinions and both advised me to send for it as the ingredients are not harmful. Thought this information might be of interest to you and your column. I have ordered some and will try it. Sorry this letter is so long.
A: Hope you enjoyed your time in Wisconsin. Thank you for passing on a potential remedy for restless leg syndrome. It is a problem that has no effective standard medical treatment. I'm a little skeptical when a product advertises that it has been around for 20 years and is 100% guaranteed to work. Why hasn't its fame spread, if it's really that good, by now? Good luck with it!

Why it happens
Up to 15 percent of pregnant women develop restless legs syndrome (RLS) during the second half of pregnancy, when sensations such as crawling, creeping, or tingling in the lower legs begin to disrupt sleep. Most of the time, the sensations are felt between the ankles and the knees, but some women experience this uncomfortable sensation in their thighs or feet. A few women may feel cramping, burning, or pain, but nearly all report an overpowering urge to move their legs. Unfortunately, stretching, bending, rubbing your legs, and walking around offer only temporary relief; as soon as you sit or lie down, the symptoms return. The condition usually shows up at rest, especially right before falling asleep. Symptoms can also occur at other times, especially when sitting still for long periods, such as at the movies or during long car rides. No one knows for sure what causes RLS, but many women develop it during pregnancy. One theory is that RLS is related to

    Parkinson's Drug Effective In Treating Restless Leg Syndrome
    ST. PAUL, Minn. – Pergolide, a drug commonly used to treat Parkinson's disease symptoms, has recently been shown to be effective in treating restless legs syndrome.     Onset and Symptoms
    Symptoms of RLS are described as mild, moderate, and severe in relation to severity and duration. Mild RLS, a rare occurrence, produces mild disruption of sleep with little distress. In moderate cases, symptoms occur up to two times a week, resulting in significant delay of sleep onset with disruption of daytime functioning. In severe cases, symptoms occur three to five times a week and result in interruption of sleep and impairment of daytime functioning (Allen et al., 2003; Earley, 2003). Of RLS patients, 94% experience sleep disturbances (We Move, 2002).

    Restless Leg Syndrome
    Restless legs syndrome (RLS) is a sleep disorder in which a person experiences unpleasant sensations in the legs described as creeping, crawling, tingling, pulling or painful. These sensations usually occur in the calf area but may be felt anywhere from the thigh to the ankle.
    One or both legs may be affected; for some people, the sensations are also felt in the arms. These sensations occur when the person with RLS lies down or sits for prolonged periods of time, such as at a desk, riding in a car, or watching a movie. People with RLS describe an irresistible urge to move the legs when the sensations occur. Usually, moving the legs, walking, rubbing or massaging the legs, or doing knee bends can bring relief, at least briefly.
    RLS symptoms worsen during periods of relaxation and decreased activity. RLS symptoms also tend to follow a set daily cycle, with the evening and night hours being more troublesome for RLS sufferers than the morning hours. People with RLS may find it difficult to relax and fall asleep because of their strong urge to walk or do other activities to relieve the sensations in their legs. Persons with RLS often sleep best toward the end of the night or during the morning hours.

    Nutritional Influences on Illness by Melvyn R. Werbach, MD Restless Legs Syndrome This syndrome is marked by an unpleasant crawling or aching sensation in the lower legs, between the knee and the ankle, often accompanied by restlessness in other parts of the body, especially in the flexor muscles of the arms and legs. The discomfort appears only at rest and elicits an irresistible need to move the limbs. It generally appears in the evening and early night and may be associated with severe insomnia.1 While, as usual, most of the research is preliminary, the results of studies investigating the effects of nutrients on restless legs syndrome (RLS) suggest that it has several causes, and that patient-specific dietary changes, nutrient repletion and nutrient pharmacotherapy are often effective treatments. Dietary Factors Based on afternoon glucose tolerance testing, many patients with RLS, particularly if they also have spontaneous leg cramps, appear to have hyperinsulinism causing functional "hypoglycemia" during testing, in fact, occasional patients may have an attack of muscle cramps concomitantly with their lowest level of plasma glucose. In an open trial, a group of 350 patients with this type of glucose tolerance curve were placed on a sugar-free, high protein diet along with frequent nibbling and at least one night feeding. The vast majority experienced a prompt remission or, at least, a striking reduction in symptoms.2
