R estless legs syndrome (RLS) is a common, underdiagnosed and treatable condition. A neurologic movement disorder, RLS is often associated with a sleep complaint. Patients with RLS may suffer an almost irresistible urge to move the legs, usually due to disagreeable leg sensations that are worse during inactivity and often interfere with sleep. RLS may be described as an agitated inability to rest that can have a negative impact on quality of life by causing waking discomfort, chronic sleep deprivation and stress. This article provides science-based information about RLS and its assessment and management in the primary care setting. Consequences of RLS Direct adverse effects of RLS include discomfort, sleep disturbances and fatigue. These consequences have a secondary impact on functioning by affecting occupational activities, social activities and family life. Disrupted sleep and an inability to tolerate sedentary activities can lead to job loss, a compromised ability to enjoy life and problems with relationships.

Why it happens
Up to 15 percent of pregnant women develop restless legs syndrome (RLS) during the second half of pregnancy, when sensations such as crawling, creeping, or tingling in the lower legs begin to disrupt sleep. Most of the time, the sensations are felt between the ankles and the knees, but some women experience this uncomfortable sensation in their thighs or feet. A few women may feel cramping, burning, or pain, but nearly all report an overpowering urge to move their legs. Unfortunately, stretching, bending, rubbing your legs, and walking around offer only temporary relief; as soon as you sit or lie down, the symptoms return. The condition usually shows up at rest, especially right before falling asleep. Symptoms can also occur at other times, especially when sitting still for long periods, such as at the movies or during long car rides. No one knows for sure what causes RLS, but many women develop it during pregnancy. One theory is that RLS is related to

Restless leg syndrome is a common cause of painful legs. The leg pain of restless leg syndrome typically eases with motion of the legs and becomes more noticeable at rest. Restless leg syndrome also features worsening of symptoms during the early evening or later at night. The characteristic nighttime worsening of symptoms in persons with restless legs syndrome frequency leads to insomnia. Restless leg syndrome usually begins slowly. Over time, the legs become more affected. Less frequently, restless leg syndrome can affect the arms. What causes restless leg syndrome?
The cause of restless leg syndrome is unknown in most patients. However, restless leg syndrome has been associated with pregnancy, obesity , smoking, iron deficiency and anemia , nerve disease, polyneuropathy (which can be associated with hypothyroidism , heavy metal toxicity, toxins, and many other conditions), other hormone disease, such as diabetes, and kidney failure (which can be associated with vitamin and mineral deficiency). Some drugs and medications have been associated with restless leg syndrome including caffeine, alcohol, H2-histamine blockers (such as Zantac and Tagamet) and certain antidepressants (such as Elavil).

Originally defined by a Swedish neurologist in the 1940s, restless leg syndrome is now thought to affect up to 15% of the population, particularly older people. Four basic elements must be present to make the diagnosis:
  • A desire to move the limbs, often associated with paresthesia or dysaesthesia Symptoms exacerbated by rest and relieved by activity Motor restlessness Nocturnal worsening of the symptoms

  • What helps? L-dopa and benzodiazepines have been tried, but there are now three high-quality randomised studies of pergolide (a dopamine receptor agonist) which show it to be highly effective. Details of the studies are shown in the Table. All of the studies were well-designed and highly detailed in the outcomes they examined, and all of them allowed a period of dose-escalation with both pergolide and comparator with drug formulations of identical appearance.

    Bandolier reader asked about restless leg syndrome, whether it is real, how common it is, and how to cure it. Bandolier found no simple answer to the third part of the question, but a good source of information about the first two parts in a large prevalence study that examined both restless leg syndrome (RLS), and the related periodic limb movement disorder (PLMD) [1]. The answer is that about 1 person in 20 has RLS, about 1 in 25 PLMD, and an unlucky 1 in 100 has both.
    Study
    The study was conducted in five European countries (UK, Germany, Italy, Portugal, and Spain) in the mid-1990s. The target population was adults aged 15 or over living at home. A random selection of telephone numbers was drawn after geographical stratification, and a household member selected by age and sex. Expert system software was used to administer questionnaires in a formulated and consistent way. The software had been specifically designed and tested to evaluate sleep disorders, and to use key diagnostic criteria administered in an exact way. The software also prompted questions about sociodemographic information, medical history, use of alcohol, tobacco and coffee, medicine use, physical activity, stress, and mental disorder. The definitions used for PLMD and RLS were those of the International Classification of Sleep Disorders.

    Periodic limb movement disorder (PLMD) and restless leg syndrome (RLS) are distinct disorders, but often occur simultaneously. Both PLMD and RLS are also called (nocturnal) myoclonus, which describes frequent or involuntary muscle spasms. Periodic limb movement was formally described first in the 1950s, and, by the 1970s, it was listed as a potential cause of insomnia. In addition to producing similar symptoms, PLMD and RLS are treated similarly. Periodic Limb Movement Disorder Periodic limb movement disorder affects people only during sleep. The condition is characterized by behavior ranging from shallow, continual movement of the ankle or toes, to wild and strenuous kicking and flailing of the legs and arms. Furthermore, abdominal, oral, and nasal movement sometimes accompanies PLMD. Movement of the legs is more typical than movement of the arms in cases of PLMD. Movements typically occur for 0.5 to 10 seconds, in intervals separated by five to 90 seconds. In 1979, the Association of Sleep Disorder Centers (ASDC) set the parameters for determining the presence of PLMD:
