What is reflex sympathetic dystrophy?
Reflex sympathetic dystrophy syndrome (RSDS) is a condition that features a group of typical symptoms, including pain (often "burning" type), tenderness, and swelling of an extremity associated with varying degrees of sweating, warmth and/or coolness, flushing, discoloration, and shiny skin. What causes reflex sympathetic dystrophy?
RSDS is also referred to as "the shoulder-hand syndrome," "causalgia," and "Sudeck's atrophy." The exact mechanism of how RSDS develops is poorly understood. The theories include irritation and abnormal excitation of nervous tissue, leading to abnormal impulses along nerves that affect blood vessels and skin. A variety of events can trigger the condition, including trauma, surgery, heart disease, degenerative arthritis of the neck, stroke or other brain diseases, nerve irritation by entrapment (such as carpal tunnel syndrome) or shingles, shoulder problems, breast cancer, and drugs for tuberculosis and barbiturates. There is no associated event in one third of patients.

What Is It? Reflex sympathetic (REE-flecks SIM-pah-theh-tick) dystrophy syndrome, or RSDS for short, involves a disturbance in the sympathetic nervous system which is the network of nerves located alongside the spinal cord and controls certain bodily functions, such as opening and closing blood vessels or sweat glands. It primarily affects the hands and feet. What Are the Symptoms?
  • First Stage
    • Affected area is painful and swollen. Changes in temperature and color of skin Rapid nail and hair growth Stiffness
    The second stage occurs after weeks or months, and includes the following symptoms:
    • Burning pain Cool skin Brittle nails Swelling Muscles spasms

    Reflex Sympathetic Dystrophy
    What is it? What causes it? Signs and symptoms. The pain associated with relex sympathetic dystrophy is often described as burning in nature. Swelling can cause painful joints and stiffness. RSD has three stages: Stage I (acute) may last up to three months. During this stage the symptoms include pain and swelling, increased warmth in the affected part/limb, and excessive sweating. There may be faster-than-normal nail and hair growth and joint pain during movement of the affected area (Figure 2). Stage II (dystrophic) can last three to 12 months. Swelling is more constant, skin wrinkles disappear, skin temperature becomes cooler, and fingernails become brittle. The pain is more widespread, stiffness increases, and the affected area becomes sensitive to touch. Stage III (atrophic) occurs from one year on. The skin of the affected area is now pale, dry, tightly stretched, and shiny. The area is stiff, pain may decrease, and there is less hope of getting motion back. Diagnosis.

    What is Reflex Sympathetic Dystrophy?
    Reflex Sympathetic Dystrophy Syndrome or RSD (also known as Complex Regional Pain Syndrome) is a neurological syndrome characterized by disabling pain, usually following some sort of neurological or bone injury (particularly fractures). Other symptoms may include pathological changes in bone and skin, excessive sweating, tissue swelling, and extreme sensitivity to touch. While the clinical syndrome of RSD is distinct, there is no specific laboratory test to diagnose RSD/CRPS. As a result, the diagnosis depends upon the physician’s assessment of the patient’s complaints (medical history) as well as any objective findings upon physical examination. While RSD is not a rare disorder, it is rarely discussed. As a result, RSD is generally poorly understood by patients, their families, and healthcare professionals. Cases can range significantly in severity. RSD may affect millions of Americans. It occurs after 1 to 2 % of various fractures and after 2 to 5% of peripheral nerve injuries. It may also occur following surgery. The injury need not be a major one; RSD can even develop as the result of sprains. In 10-20% of cases, however, no causal event can be identified. Regrettably, the diagnosis is often missed or delayed. While some mild cases may resolve without treatment, the usual pattern of spread is up the affected extremity and then may continue to spread on the same side of the body or to the opposite extremity. RSD may even spread to a distant site.

    Reflex sympathetic dystrophy (RSD)—recently renamed complex regional pain syndrome types I and II—is a documented condition described in the literature as a pain syndrome occurring as a result of trauma to a body part. Repetitive strain injuries have been described as injuries that at least partially involve microtrauma to soft tissue. In a number of cases patients with RSI have slowly developed RSD, which has been neglected in the literature. I have recently been introduced to the possibility that many RSI patients have "mild" or "early" RSD, and I want to alert other clinicians as well as patients. Traditionally, physicians have diagnosed RSD when certain documented medical criteria have been met. In order for the diagnosis of "probable RSD" to be made, the patient would need to exhibit five or more of the following criteria as indicated in the literature by Colton and Fallat (1966): 1. Allodynia (perception of pain from a stimulus that is normally not noxious) or hyperpathia (an exaggerated response to painful stimuli)

This syndrome can be precipitated by any neurally related visceral, musculoskeletal, neurologic or vascular condition. Frequently, however, a cause is not identifiable. Trauma, cerebrovascular disorders, degenerative disease of the cervical spine, discal herniation, postinfectious states, calcific tendinitis, vasculitis and neoplasm have all been associated with reflex sympathetic dystrophy. Various theories have been advanced to explain the pathogenesis; the most widely held explanation involves the existence of an "internuncial pool." In this theory an injury or lesion is assumed to produce painful impulses that travel via afferent pathways to the spinal cord, where a series of reflexes are initiated that spread via the interconnecting pool of neurons. The lateral and anterior tracts are stimulated, which provokes efferent pathways to the peripheral nerves, producing the local findings of reflex sympathetic dystrophy. Patients may exhibit variable symptoms and signs, often in the shoulder and hand. Stiffness, pain, tenderness and weakness may be associated with swelling, vasomotor changes, hyperaesthesia and disability. Glenohumeral joint periarthritis may be accompanied by a shoulder hand syndrome.

Other characters Reflex sympathetic dystrophy, sympathetic nervous system changes seen in the limbs following an insult which is often relatively minor in nature. Clinical signs are variable and include stiffness, pain, tenderness, loss of power, hyperaesthesia and vasomotor changes. On radiographs there is generalized regional osteopenia with periarticular osteoporosis, but with preservation of the joint spaces. The osteoporosis may be "spotty". Bone scintigraphy during the acute phase shows increased flow on all three phases but without focal pathology. Children who very rapidly sustain sympathetic diminution in flow to a limb with disuse may not show these features but may have a cold scan instead of increased uptake.
