et al. 1992). Furthermore, high plasma insulin levels have been associated with an increased risk of ischemic heart disease in several prospective population studies (Py¶r¤l¤ et al. 1985, Casassus et al. et al. 1996, Folsom et al. et al. et al. 1987, Mbanya et al. The underlying metabolic defect explaining the cluster has been assumed to be resistance to insulin-stimulated glucose uptake, i.e. insulin resistance (Reaven 1988). The primary cause of insulin resistance is unclear, though genetic predisposition is probably important, exposing an individual to such environmental effects as excessive energy intake leading to obesity, which in turn deteriorates insulin sensitivity. Possible pathogenetic mechanisms for the development of the MS have been constructed to include hypertension, glucose intolerance, hyperinsulinemia, low HDL cholesterol and high triglycerides. Insulin resistance is defined as a condition in which the body fails to respond normally to insulin (DeFronzo et al.

Fighting the resistance
Understanding the role insulin resistance plays in type 2 diabetes can help pharmacists appreciate more fully this complex disease and its treatment. In this article, the first of a two-part feature, Irene Gummerson explains insulin resistance and the complications of Type 2 diabetes
OBJECTIVES
  • To understand the actions of insulin
  • To understand what causes type 2 diabetes
  • To recognise the significance of diabetic complications
  • To be aware of the risk factors for type 2 diabetes
  • To appreciate the pharmacist's role in diabetes management
Some ethnic groups have an increased risk of diabetes. In the Asian population for example, type 2 diabetes is between two and four times as common as in the white population. This means that there is a lifetime risk of one in eight for white people, compared with one in three for the South East Asian population. There is an increased rate of renal disease and diabetes with hypertension in African Caribbeans. There are two main types of diabetes. Type 1 diabetes develops when there is a severe lack of insulin in the body because most or all of the pancreatic cells that produce it have been destroyed. This type of diabetes usually appears in people under the age of 40, often in childhood. It is treated with insulin injections and diet.
Type 2 diabetes develops when the body can still produce some insulin, though not enough for its needs, or when the insulin produced is ineffective. This type of diabetes usually appears in people over the age of 40. It is treated by diet alone, or by a combination of diet and oral hypoglycaemics, or by a combination of diet and insulin.

Abstract Although there is evidence that variations in insulin action are under genetic regulation, probably as much as 50% of the variability in insulin-mediated glucose disposal within a population of normal volunteers is secondary to difference in life-style. In this context, obesity and decreased physical activity play particularly crucial roles, each contributing approximately equally to the variability in insulin-mediated glucose disposal seen in the population at large. It is also worth noting that powerful ethnic differences exist, with essentially every other ethnic group studied being more insulin resistant than individuals of European ancestry. Insulin resistance

Pre-metabolic syndrome: the real initial stage of metabolic-syndrome, type 2 diabetes and arteroscleropathy. Sergio Stagnaro (17 February 2004) Private I agree completely with this exciting article authors who state that MS, PD, and T2DM are associated with endothelial dysfunction and carry an elevated risk for both micro and macrovascular disease that are often present at the time of diagnosis of overt T2DM. Postnatal vascularization in a response to injury mechanism to the endothelium and the arterial vessel wall as well as the capillary bed result in most of the complications associated with these disorders. I demonstrated, since 30 years, in early papers that all individuals with Campbell de Morgan spots (cutaneous microangiomas) are involved by a particular mitochondrial cytopathology, termed Congenital Acidosic Enzyme –Metabolic Hystangiopathy-a (CAEMH-a) (1, 2, 3, 4), conditio sine qua non of diabetic, dyslipidemic, arteriosclerotic, hypertensive, a.s.o., constitutions (See HONCode web site 233736, www.semeioticabiofisica.it).

    How high is your risk?

By Dr. Rich In recent years, both doctors and patients have begun hearing about an apparently new medical condition called "metabolic syndrome X." Actually, this disorder has been around as long as modern civilization, but the recognition that this particular constellation of metabolic disorders constitutes its own syndrome has been recognized only recently. Metabolic syndrome X is important for four reasons. First, it is very prevalent in Western countries. Second, it is associated with an extremely high incidence of cardiovascular disease and premature death. Third, it is often unrecognized by patients who have it and their doctors. Fourth, when it is recognized, aggressive treatment successfully reduces the risk of life-threatening complications. Note: The modifier "metabolic" is used to differentiate this condition from a different syndrome X - the so-called "chest pain syndrome X" - which is angina-like chest pain in the absence of visible coronary artery disease. DrRich would personally like to "thank" the individuals who chose to give these different conditions the same name.

    http://www.reproductiveassociates.org/syndromeo.html
What Is Syndrome O? Everyday, millions of women are struggling with heartbreaking problems — infertility, miscarriage, obesity, and cosmetic nightmares unimaginable to most men. Most of these women suffer in silence, since they don’t know where to turn for help. Persistent frustration also pervades the lives of women who do seek care, aggravated by frequent doctor visits and costly, sometimes risky, fertility treatments. If that’s not enough, there are the gynecologic “lady’s problems” for countless others — missed menstrual cycles, heavy and unpredictable vaginal bleeding, bad reactions to birth control pills, and even unnecessary hysterectomy operations. Amazingly, all of these diverse female health problems are closely related — a fact that is often not recognized by most doctors and nurses. Alternatively, many women are told by their health providers that they may have ‘polycystic ovaries’ or polycystic ovary syndrome (PCOS), a clinical label which doesn’t really explain the root of their problem.

    Crazy for Carbs
December 6: In his book, The Zone, Barry Sears advocates a low carbohydrate diet. He also suggests that people who eat a lot of carbs will become caught in a cycle that leads to overeating, obesity and Syndrome X. (Full article as appeared at www.ABCNEWS.com) Americans now eat more carbohydrates than ever: 50 more pounds per person, per year, than a decade ago. Carbohydrate-related diseases have also reached a peak. After decades of being warned away from fats, many people have turned to carbohydrates instead. At the same time, obesity levels in the country are greater than ever. Is there a connection? Do carbohydrates lead people to become obese? Experts in the field have different opinions. The Zone Biochemist Barry Sears, author of the best selling low-carbohydrate diet book The Zone, believes carbohydrates do cause people to gain weight. He has developed a controversial theory that suggests that people who eat a lot of carbohydrates get caught up in a cycle of overeating because carbohydrates can be addictive in a way other foods are not.
