What is reactive arthritis?
Reactive arthritis, also known as Reiter's syndrome, is a type of arthritis that occurs as a reaction to an infection somewhere in the body. Most infections that cause the disease originate in the genitourinary tract (the bladder, urethra, penis, or vagina) and are spread through sexual intercourse, a form of the disease called genitourinary Reiter's syndrome, or urogenital Reiter's syndrome. Other infections that can cause reactive arthritis include gastrointestinal infections due to eating contaminated food or handling contaminated substances, a form of the disease called gastrointestinal Reiter's syndrome, or enteric Reiter's syndrome.

    Reactive Arthritis
    Reactive Arthritis is one of more than one hundred different types of arthritis and mostly affects young men 16-35 years old. It is one of the most common forms of arthritis affecting young men, but it may also affect young women and children. It is called Reactive Arthritis because an infection in one part of the body causes a reaction in another part of the body without the infection being found there. Reactive Arthritis causes inflammation (redness, swelling, pain) in different parts of the body at the same time or in different parts of the body at different times. Arthritis is the most important symptom in a chain of events which is the attack of reactive arthritis. In a person at risk the attack is usually triggered by an infection. (A blood test may identify a protein on your white blood cells that indicates that you are a person at risk. This protein is called HLA B27. Up to 25% of patients with Reactive Arthritis do not have HLA B27). Often it is a bowel infection causing loose, frequent bowel motions (diarrhoea) or it may be an infection transmitted during sexual intercourse. Occasionally there does not seem to be a triggering infection. In sexually acquired reactive arthritis the first symptom noticed is usually a pain when passing urine or discharge of pus from the inside of the penis (urethritis). In women a discharge from the neck of the womb (cervix) may not be noticed. More...

    The Fourth International Workshop on Reactive Arthritis, held in the beautiful city of Berlin, Germany, July 3 to 6, 1999, was organized by Joachim Sieper, Gabrielle Kingsley, and Jurgen Braun. Forty-two invited participants from Europe and North America attended the meeting.

    T-CELL PROTECTION AGAINST MICROBES

    Stefan Kaufmann from Berlin pointed out that protection against microbes by T cells is regulated by Ag-presenting molecules, which comprise the classic MHC class I and class II molecules, the nonclassic MHC class Ib molecules, and the CD1 molecules that have structural similarity to MHC class I molecules. Human Group 1 CD1-restricted T cells play a regulatory role in the immune response against intracellular bacteria. Recent findings suggest that they recognize ligands such as glycolipids of mycobacteria, using alpha beta T cell receptor. The NK T cells, which also perform immune regulatory functions in infections, are restricted by Group II CD1 (CD1d) molecules.
    BACTERIAL ANTIGENS AND BACTERIAL DNA
    Bacterial antigens (lipopolysaccharide and heat-shock protein) persist in peripheral blood mononuclear cells and polymorphonuclear phagocytes for a long time in patients who develop ReA after Yersinia enterocolitica O:3 infection. These antigens can also be present in the synovial fluid cells of some of these patients. Kaisa Granfors has observed bacterial degradation products, but not bacterial DNA, in the inflamed joints of patients with Salmonella-triggered ReA.
