In 1984 in Ontario, Canada, an outbreak of Salmonella typhimurium food poisoning occurred among police officers who were serving as security guards during a papal visit.1 Of the 1,608 police officers involved, 432 had acute gastroenteritis. Within three months following the outbreak, 27 of these officers had developed acute arthritis; over the next four months, it resolved in nine of them. The remaining 18 officers had recurrent symptoms or had developed a chronic arthritis on re-evaluation five years later. These officers had developed reactive arthritis, or Reiter's syndrome. Although this condition was once considered benign and self-limited, that may not always be the case. We review the current understanding of this illness and implications for treatment. Clinical Features Reactive arthritis is an aseptic inflammatory polyarthritis that usually follows nongonococcal urethritis or infectious dysentery.2 The classic triad of arthritis, urethritis and conjunctivitis does not occur in all patients (Table 1). Onset typically occurs one to three weeks following the infection and may present acutely or insidiously. Urethritis is often symptomatic in male patients, usually with a mucopurulent discharge, but sometimes presents as gross hematuria secondary to hemorrhagic cystitis. In female patients, nonspecific cervicitis may occur. However, in either sex, urethritis may be asymptomatic.

Well g'day!! This is my little story about one of the most pleasing things I have accomplished. The fight against Reiter's Syndrome (or Reactive Arthritis). There is a cure for this and similar illnesses such as rheumatoid arthritis and lupus. I believe that the cure is a medically proven fact. My story would back that up but please make your own judgements. Believe it or not I am very grateful that I suffered from this illness. I have learnt a lot about life and the value of it. It all started I guess a couple of years ago.... One day I had a sore elbow. So I went to my doctor. She gave me anti-inflammatories for it - which 'fixed it' short term obviously. As soon as the anti-inflammatories had worn off - my sore elbow came back. Pretty soon it got worse and eventually I could not straighten it out - and it hurt like hell to try. My Doctor couldn't figure out what was wrong. I had some blood tests but I can't remember what for. The anti-inflammatories would help only short term. Anyway things don't really improve and it's in my two biggest toes on my right foot - THAT hurts like hell. I am hobbling to work and on 'bad days' going down stairs one at a time - child style. Back to the specialist every now and then. One day he says - how about Methotrexate - a more "effective" treatment. But first we'll have you off to a sperm bank in case you want to have children later. Methotrexate can f*** up your fertility (pardon the French). I don't really like that idea.

Reactive arthritis
(Reiter’s syndrome) Reactive arthritis (ReA) refers to acute nonpurulent arthritis complicating an infection elsewhere in the body. The most common bacterial infections known to cause reactive arthritis are Salmonella, Y. enterocolitica, C. jejuni, and C. trachomatis.
Clinical manifestations
The majority of patients will present with a history of previous infection (1-4 weeks earlier). Urinary tract symptoms (urethritis) as well as occular symptoms (conjunctivitis) may occur later in the course of the disease.

Reactive arthritis is a form of arthritis, or joint inflammation, that occurs as a "reaction" to an infection elsewhere in the body. Inflammation is a characteristic reaction of tissues to injury or disease and is marked by swelling, redness, heat, and pain. Besides this joint inflammation, reactive arthritis is associated with two other symptoms: redness and inflammation of the eyes (conjunctivitis) and inflammation of the urinary tract (urethritis). These symptoms may occur alone, together, or not at all. Reactive arthritis is also known as Reiter's syndrome, and your doctor may refer to it by yet another term, as a seronegative spondyloarthropathy. The seronegative spondyloarthropathies are a group of disorders that can cause inflammation throughout the body, especially in the spine. (Examples of other disorders in this group include psoriatic arthritis, ankylosing spondylitis, and the kind of arthritis that sometimes accompanies inflammatory bowel disease.)

I am a 39 year old Male and was diagnosed with Spondyloarthropathy about 1 year and a half ago. Since then, I have developed sclerosis of the right Sacroiliac Joint as well as disk degeneration between the L-5 and S-1 vertebrae. The 6-month comparative x-rays appear to show no additional sclerosing of the SI Joints. There has, however, been progressive involvement in the Arthritis including Achilles Tendonitis and enthesitis of the elbow as well as bursitis and knee involvement. Corticosteroid shots have been successful with the enthesitis, however, not with the bursitis.
There has been gastrointestinal involvement associated with the arthritis which has lead my Rheumatologist to believe this is Reactive Arthritis due to an ongoing bacterial infection in the GI tract. Colonoscopy and endoscopy revealed chronic and acute inflammation of the upper GI, but no findings of Crohn's or Ulcerative Colitis. Fecal Samples revealed high levels of Gram Positive Bacilli and Gram Positive Cocci (Greater than 10/1000x). There has been chronic episodes of epididymitis and prostatitis, however, no casuative bacteria has been able to be located.
Blood results have not indicated a high Sed Rate and have been normal except for occasional elevated liver enzymes, AST (48 U/L) and ALT (43 U/L). Also, I was found to be HLA-B27 negative.

Reactive Arthritis

Reactive arthritis, formerly known as Reiter's Syndrome, is one of the spondyloarthropathies that is being diagnosed more and more. Arthur-itis, being the shrewd beast he is, has put together a puzzling combination for this one.
Reactive arthritis develops as a reaction to some other kind of infection. The body reacts after the infection by showing swollen joints, inflammation of the lower spine, and the characteristic conjunctivitis around the eyes and urethral and or bowel inflammation.
This type of arthritis often involves very sore and inflamed Achilles tendons of the heel, as well. The HLA-B27 gene can be pointed to as a marker of the tendency to develop reactive arthritis.
The diseases that this kind of arthritis can develop in response to ( auto-immune reaction) range from food poisoning to STD's to HIV infection. Stress contributes to the recurrence of symptoms in sufferers.
