Lifestyle Behaviors
The following are some lifestyle tips for managing Raynaud's phenomenon:
  • Keeping warm is the primary goal for preventing the onset of Raynaud's phenomenon. Air-conditioning and exposure to refrigeration can trigger this syndrome. If patients go out in cold weather, they should dress warmly with many layers. Wearing a hat is essential.
  • Living in a warm climate may help relieve symptoms, although a recent study found that weather changes themselves had little effect on the disorder.
  • Exercise is helpful in maintaining a sense of well being, in keeping warm, and helping to sustain skin pliability. (Patients with Raynaud's phenomenon may want to avoid exercising outdoors in cold weather, however.)
  • Quitting smoking is, of course, essential for anyone, but it is critical for people with scleroderma.
  • Patients would be wise to seek relaxation and anti-stress techniques, which might help reduce some triggers of Raynaud's phenomenon. Many effective methods are available [ see Well-Connected Report #31

peripheral vascular disease, namely caused by an inappropriate response of the
peripheral arteries in reaction to environmental stimuli usually to the cold.
The term primary Raynaud's or Raynaud's disease is used when no underlying
disease can be found. The term secondary Raynaud's, or Raynaud's phenomenon or
syndrome, is used when an underlying disease is primarily causative and a range
of symptoms including the peripheral vascular spasm and the Raynaud's condition
is secondary as a result of this underlying disease. However, today the term
Raynaud's is often used medically along with the investigation of symptoms and
signs of underlying disorders (mostly connective tissue/autoimmune disorders) to

Raynaud's Phenomenon
What is it?
Raynaud's phenomenon is the name given to the temporary interruption of blood supply to the fingers or toes (though occasionally the ears and nose can also be affected), which occurs as a result of spasm of the arteries bringing the blood to the fingers etc. It is characterised by a series of changes in the colour of the skin. Typically the finger initially becomes white as blood supply is lost, then blue or dusky, and finally pink as blood flow improves and eventually returns to normal. The sequence is of variable duration and can last from minutes to hours.
Why is it called that?
It is named after the French physician Maurice Raynaud who in 1862 was the first to describe the process and give an explanation for it.
Around 5% of the population and up to 20% of women at some time in their life are said to have had an episode of Raynaud's. Some disorders however specifically predispose to the problem. While these conditions are rare, they are important to identify because they are likely to need specific treatment. Included among these disorders are systemic lupus erythematosous (SLE) and scleroderma, where over 90% of patients experience Raynaud's attacks. The "white finger" of chain saw operators is an example of an occupation-related disorder. The arterial spasm which as has been mentioned, is the initial phase of an episode of Raynaud's is most commonly triggered by cold, though emotional upset, the use of some vibrating tools, and certain drugs (including caffeine and cigarettes) can also do this.

Raynaud's Phenomenon Handy Hints
Here is a collection of hints that have been collected from many different sources, many of them may be helpful to you in trying to prevent attacks of Raynaud's from occurring. The following tips can help you prevent getting chilled: Always wear a hat : It is true that you lose a lot of heat through the top of your head. Covering the head helps to keep the whole body warm by preventing heat from escaping. Dress warmly : Layers of loose clothes trap warm air around your body. Cotton socks worn over wool socks help keep the feet warm and dry. Mittens are warmer than gloves because the fingers keep each other warm. Tight gloves can cut off blood circulation in the fingers. Don't get wet : As water on the skin evaporates, it cools the skin - just the opposite of what you want. Cover up exposed skin : If it is very cold and windy, putting a scarf across your nose and cheeks will protect them and help you breathe better. If you are so cold that it hurts, get inside. The pain in your body is a warning signal.
Remember
Try to avoid changes in temperature - remember a change in temperature is even more likely to bring on an attack than just being in the cold. Do not sit in draughts.

Since this proceedure was done-fusion at L4-L5 and S1-my right foot has been cold incomparison to the other foot. The surgery required a vascular surgeon. The proceedure was done both through the front and the back. My doctor thinks that a nerve was damaged during the surgery and that since the condition has not improved in the year's time it most likely will never. My question, is this Raynaud's phenomenon or is this nerve damage and should I be concerned with the difference in temperature. During the winter months the discomfort is great. My right foot tends to "freeze" when we go to the mountains. I am concerned and would like to know if there is any testing to be done to make sure that all is a ok and there is no danger to my toes or foot.

Although much of the book is fairly complicated science, the book is a wonderful resource for those individuals who have been diagnosed with Raynauds Phenomenon. Finally I found a source that explained many of the "side effects" of Raynauds at a level that I could understand. If you, or someone you know, has been diagnosed I (a fellow Raynauds "sufferer") absolutely recommend it!

    When to refer
  • to identify a secondary cause e.g. Systemic lupus erythematosis
  • rheumatoid arthritis
  • scleroderma/CREST syndrome

  • When first line management is not effective.
  • When there is pulp and nail fold changes in fingers or feet.
  • Persistence of symptoms through summer and winter.
  • To evaluate vibration white finger (a notifiable occupational disease).
  • Investigations
    • FBC ESR/PV Profile Autoantibodies including Rheumatoid factor Rheumatology referral
    First Line Management
  • Ensure core body temperature is well maintained this often necessitates wearing thermal underwear. Wear prewarmed gloves and put them on BEFORE entering a change of temperature. Vasodilators: try Thymoxamine (opilon) upto 80mg tds or nifedipine 20mg upto tds. Advice to start 2 days before weather forecast advises a cold snap and continue for about 2 weeks.
