What is Raynaud's phenomenon?
Raynaud's phenomenon, sometimes called Raynaud's syndrome or disease, is a disorder of blood circulation in the fingers. This condition aggravate with cold exposure. Exposure to cold abnormally reduces blood circulation causing the fingers to become pale, waxy-white or purple. The disorder is sometimes called "white finger," "wax finger" or "dead finger." Raynauds Phenomenon
Raynaud's Phenomenon is a Disorder of the small blood vessels that feed the skin. During an attack of Raynaud's, these arteries contract briefly, limiting blood flow. This is called a vasospasm. Deprived of the blood's oxygen, the skin first turns white then blue. The skin turns red as the arteries relax and blood flows again. Extremitieshands and feetare most commonly affected, but Raynaud's can attack other areas such as the nose and ears. Doctors classify Raynaud's phenomenon as either the primary or the secondary form. In medical literature, "primary Raynaud's phenomenon" may also be called Raynaud's disease, idiopathic Raynaud's phenomenon, or primary Raynaud's syndrome. The terms idiopathic and primary both mean that the cause is unknown. Most people who have Raynaud's phenomenon have the primary form (the milder version). A person who has primary Raynaud's phenomenon has no underlying disease or associated medical problems. More women than men are affected, and approximately 75 percent of all cases are diagnosed in women who are between 15 and 40 years old, but anyone can have the problem. It is not known for sure how many people suffer from these symptoms, but Raynaud's is a common problem. People who have only vasospastic attacks for several years, without involvement of other body systems or organs, rarely have or will develop a secondary disease (that is, a connective tissue disorder such as scleroderma) later. Several researchers who studied people who appeared to have primary Raynaud's phenomenon over long periods of time found that less than 9 percent of these people developed a secondary disease.

Painful Breastfeeding

Ingrid was born at Ostra Hospital in Goteborg, Sweden in December 1996. We had breastfeeding problems from the start: first we were seperated at birth and then later she developed a fever and was taken to another ward where she was given formula (despite my protests).
Poor Ingrid! I tried to breastfeed her, but she was so confused she just cried. It would take up to two hours for her just to get latched on. So I pumped and fed her with a spoon (after having my milk weighed by staff). I felt like a total failure. After five days I finally found someone who had the necessary skills to get Ingrid latched on again properly. After the longest week of my life we went home.
A week later an ache began deep in the center of my breasts. They felt bruised and my nipples started to hurt, shifting in colour from red to white. When I asked, I was told this was perfectly normal. I had a thrush test done, that proved negative. As the days passed nursing became more and more painful. At three weeks I began using a nipple shield just to cope. At the weekly check-ups Ingrid was gaining weight slowly. I was asked to supplement with formula, but refused. Then I went home and cried, feeling like a total failure again.
When Ingrid increased her feedings at around five weeks I used Syntocinon spray to help the let-down (and lessen the pressure on the nipple). I began to wonder if something wasn't seriously wrong. I tried to get a friend to nurse Ingrid, hoping she would be able to say if Ingrid was doing something wrong with her mouth, but Ingrid refused to cooperate.

When very severe, the circulation to the fingers and toes can be permanently affected and cause damage to the tissues. In most sufferers the condition is relatively mild and can be helped by limiting exposure to the cold and investing in good quality, fur-lined mittens (better than gloves) and boots. Occasionally, drugs that improve the circulation may be prescribed, however they can cause side effects. Some people with Raynaud's phenomenon have underlying disease, but this is not very common. The color may change to blue and finally red, as the blood returns to the tissues. This can take from several minutes to many hours, and can be extremely painful. It may also affect the ears and nose. Why Raynaud's phenomenon occurs is not well understood. Usually, the body conserves heat by reducing blood circulation to the extremities, particularly the hands and feet. This response uses a complex system of nerves and muscles to control blood flow through the smallest blood vessels in the skin. In people with Raynaud's phenomenon , this control system becomes too sensitive to cold and greatly reduces blood flow in the fingers. Damage to either the muscles or nerves that control blood flow may be the cause of Raynaud's phenomenon.
Treatment may vary according to the severity of the symptoms. In mild cases it may be sufficient to maintain a constant body temperature, and, if possible, avoid extreme cold and sudden changes in temperature. Attacks can also be caused by stress and smoking. It is important to try and prevent attacks occurring in the first place.

Definition Raynaud's disease or syndrome is a disorder of blood circulation, mainly in the fingers and toes. The following are some lifestyle tips for managing Raynaud's phenomenon:
  • Keeping warm is the primary goal for preventing the onset of Raynaud's phenomenon. Air-conditioning and exposure to refrigeration can trigger this syndrome. If patients go out in cold weather, they should dress warmly with many layers. Wearing a hat is essential.
  • Living in a warm climate may help relieve symptoms, although a recent study found that weather changes themselves had little effect on the disorder.
  • Exercise is helpful in maintaining a sense of well being, in keeping warm, and helping to sustain skin pliability. (Patients with Raynaud's phenomenon may want to avoid exercising outdoors in cold weather, however.)
  • Quitting smoking is, of course, essential for anyone, but it is critical for people with scleroderma.
  • Patients would be wise to seek relaxation and anti-stress techniques, which might help reduce some triggers of Raynaud's phenomenon. Many effective methods are available [ see Well-Connected Report #31
