Living with Pseudotumor Cerebri
Pseudotumor cerebri is a relatively rare disease. (We estimate there are about 12,000 people living in the United States with this disease.) The majority of PTC patients are women of child-bearing age with a tendency to be overweight. However, there are PTC patients of all ages- male and female, and all weight ranges- from obese to downright skinny.  Pseudotumor cerebri is also an individualized disease. Each person responds differently regarding symptoms, treatment, and side effects. There is no "standard" for how the condition will appear, how long it will last, whether
it will come back, or the degree it will impact your life.
There are people who have success with medication or treatment, and their PTC goes away. There are others who become disabled with this condition, and
treatment or surgery is ineffective.

A free, new (9/12/00) clinical research study at the Jewish Hospital Cholesterol Center, 3200 Burnet Avenue, Cincinnati, OH, 45229. Purpose: Our specific aim is to assess major thrombophilic and hypofibrinolytic pathoetiologies of pseudotumor cerebri ('benign' intracranial hypertension). Our second specific aim is to determine whether treatment of the intracranial hypertension with Diamox will produce symptomatic relief. We postulate that when exogenous thrombophilic factors (estrogen-containing oral contraceptives, estrogens, corticosteroids) are superimposed on heritable thrombophilic and/or hypofibrinolytic coagulation disorders, multiple ischemic cerebral strokes occur. We postulate that as these strokes resolve, they leave a residual effect of increased intracranial pressure, either by virtue of reduced cerebral clearance of cerebrospinal fluid (CSF), or increased production of CSF, or both. We expect that most patients seen by ophthalmologists with pseudotumor cerebri can be shown to have a coagulation disorder as the pathoetiology, particularly when exogenous estrogens are superimposed on a coagulation disorder. Significance in Relationship to Human Health: Recently, it has been realized that sequelae of ischemic stroke, particularly those ischemic strokes mediated by coagulation disorders, may be pseudotumor cerebri (intracranial hypertension).

Definition / Leitsymptome / Pathophysiologie
    O) ohne Nachweis einer intrakraniellen Raumforderung bzw. ohne Nachweis eines Hydrozephalus. Klinische Leitsymptome sind die Folgen einer diffusen intrakraniellen Drucksteigerung:
    • Kopfschmerz
    • Stauungspapille.
    • Affektion des N. opticus, Visusminderungen bis zur Erblindung.
    • Ein- oder doppelseitige Abducensparese.
      (a) Sinusvenenthrombose,
      (b) Adipositas (besondere bei jungen Frauen zumeist vorhanden) und
    Untersuchungen:
    • Notwendig:
      • Kopfschmerzanamnese
      • Gewichtsentwicklung (Adipositas bei jungen Frauen, rasche Gewichtszunahme)
      • Auschluss einer intrakraniellen Raumforderung (Tumor, Blutung etc.)
      • Ausschluss eines Hydrozephalus internus

Accutane has been associated with pseudotumor cerebri!
Under Federal Law, the manufacturer of a drug is required to revise its label to include a warning as soon as there is reasonable evidence of an association of a serious hazard with a drug.
The FDA has required Hoffmann-La Roche to change their label for Accutane many times. The most recent label states - Pseudotumor Cerebri: Accutane use has been associated with a number of cases of pseudotumor cerebri (benign intracranial hypertension), some of which involved concomitant use of tetracyclines. Concomitant treatment with tetracyclines should therefore be avoided. Early signs and symptoms of pseudotumor cerebri include papilledema, headache, nausea and vomiting, and visual disturbances. Patients with these symptoms should be screened for papilledema and, if present, they should be told to discontinue Accutane immediately and be referred to a neurologist for further diagnosis and care.
Pursuant to FDA Rules and Regulation, the reporting of Adverse Side Effects is on a voluntary basis. In other words, the doctor, hospital, or other medical person or facility are not required by law to report an adverse side effect from a drug. The FDA reported that perhaps only one in one hundred side effects ever get reported. One could only imagine about how many serious Adverse Side Effects there are from taking Accutane, that have never been reported to anyone!

Pseudotumor Cerebri What is pseudotumor cerebri? Pseudotumor cerebri is a condition in which high cerebrospinal fluid pressure inside your head can cause headaches and problems with your vision. Cerebrospinal fluid is the clear fluid that surrounds the brain and spinal cord. The term “pseudotumor” (which means “false tumor”) comes from the days before brain scans, when doctors who noted swelling of the optic disc (the visible portion of the optic nerve in the back of the eye) considered the possibility of a brain tumor. Patients with optic disc swelling but no evidence of a tumor were said to have a “pseudotumor”. In Pseudotumor cerebri, the flow of cerebrospinal fluid is blocked from flowing back out of the head as it should, leading to high pressure inside the head. This pressure results in swelling of the optic disc at the back of the eye, which can damage (sometimes permanently) the optic nerve, causing vision loss. High pressure may also cause damage to the nerves that move the eyes, resulting in double vision. What causes pseudotumor cerebri?

Wissenschaftlichen Mitteilungen zufolge kann neben den neurologischen und augenärztlichen Verlaufsuntersuchungen mithilfe der Sonographie ebenfalls verlä§lich die Aufweitung des Sehnerven gemessen und im Verlauf kontrolliert werden. How can I be experiencing so many visual problems without having swelling of the optic nerves? Can PTC be caused by the narrowed carotid artery from the dissection?

Dear MSB:
Sorry that your having trouble with your vision. Usually a carotid dissection should not cause PTC. Since CSF is absorbed by the venous not the arterial system, it doesn't make too much sense that it has a connection to the PTC. We most commonly see PTC in overweight females and the usual is to start diamox and encourage weight loss. There are medications such as vitamin A and medications such as tetracyclines that can induce PTC. Yes, one can have visual problems without frank papilledema but it is rare and usually it is just a matter of time before it develops. Since the diuretics are not working, it may be important to begin another medication such as a steriod burst. As you are probably aware, the big worry is loss of vision with PTC. Sometimes, it even comes to having to shunt a patient, draining CSF fluid from the brain. I do hope that you can get relief.
