General Discussion Intracranial Hypertension (Pseudotumor cerebri) is characterized by increased pressure inside the skull. Intracranial means inside the skull and hypertension means high fluid pressure. Basically, the pressure of the fluid that surrounds the brain (cerebrospinal fluid or CSF) is too high. Elevated CSF pressure produces severe headache and often visual difficulties, which, if left untreated can result in loss of vision or blindness. Pseudotumor Cerebri and Benign Intracranial Hypertension are both former names for Intracranial Hypertension (IH) which are now considered inaccurate. These names do not adequately describe the disorder and downplay the seriousness of IH. There are two categories of IH: Primary Intracranial Hypertension and Secondary Intracranial Hypertension.

Pseudotumor cerebri , also known as benign intracranial hypertension, idiopathic intracranial hypertension, or a false brain tumor , is a neurologic disease that is caused by increased intracranial pressure in the subarachnoid space surrounding the brain without any indication of intracranial pathology. This pressure increase can be attributed to either buildup or poor absorption of cerebrospinal fluid (CSF). It most commonly affects women between ages 20 and 50. Symptoms are headache nausea transient visual obscurations , and vomiting . The name of the disease comes from the fact that these symptoms are also frequently observed in patients with brain tumor. Depending on the severity of the condition, pressure may be decreased by repeated spinal taps (to remove excessive cerebrospinal fluid ), steroids, drugs that reduce cerebrospinal fluid production and / or diuretics. Papilledema is a very common sign of Pseudotumor cerebri. The diagnosis of Pseudotumor cerebri is one of exclusion. Due to the possibility of causing a brain herniation if an actual lesion is present, a spinal tap should only be performed after a negative

What is Pseudotumor Cerebri?
Pseudotumor cerebri, also called benign intracranial hypertension, literally means "false brain tumor." It is caused by increased pressure within the brain and is most common in women between the ages of 20 and 50. Symptoms of pseudotumor cerebri, which include headache, nausea, vomiting, and pulsating intracranial noises, closely mimic symptoms of brain tumors, possibly because of the abnormal buildup of pressure within the brain. a.k.a. Idiopathic Intracranial Hypertension, Benign Intracranial Hypertension
The main means of treating pseudotumor is to relieve symptoms. Medications that decrease the production of spinal fluid or increase its absorption are used quite frequently. In doing this the headache and pressure on the optic nerve, which causes the vision loss, is relieved to some point. Spinal taps drain some of the fluid, but within a day that pressure starts to build again. Shunting is another means of relieving this pressure. A small-diameter tube is placed either in the head or spinal column, with a one way pressure sensitive valve in it,that drains the excess fluid to another place in the body to be absorbed. Another procedure that is done is primarily to relieve the vision problems associated with pseudotumor. It is called an Optic Nerve Sheath Fenestration, meaning they put slits into the sheath that covers the optic nerve to relieve the pressure.
There have been many theories about pseudotumor over the years. It was first believed to be only women. But there are males that have this diagnosis now to dispell that myth. It was also believed to only be in obese patients. There are now normal weight as well as underweight patients who have documented pseudotumor. Some have gone so far to attribute it to eating polar bear meat. There aren't too many that eat polar bear meat anymore so that has fallen by the way side as well. They are making correlations between the use of some medications and pseudotumor, but time will tell which of these is true medical finding or just coincidence. It was also believed that pseudotumor is "self-limiting" and would "only" last up to two years. Another myth. There are some that have had this syndrome for many years and continually have symptoms.

Last Revision Date: 2003 Treatment of raised intracranial pressure is both medical and surgical. It is aimed mainly at lowering of intracranial pressure and secondarily at treating symptoms directly (e.g. headache). Unfortunately, all reports to date are anecdotal as there have not been any controlled clinical treatment trials for idiopathic intracranial hypertension. Weight Loss
Weight loss has been used to treat IIH for many years. Newborg in 1974 reported remission of papilledema in all nine patients placed on a strict diet. She used a low calorie adaptation of Kempner's rice diet. The patient's intake was 400-1000 calories per day by fruits, rice, vegetables and occasionally 1-2 oz of meat. Fluids were limited to 750-1250 ml/day and sodium to less than 100 mg/day. All patients had reversal of their papilledema. Unfortunately, there was no mention of the patients' visual testing. The beneficial effects of weight loss have also been reported more recently. Kupersmith and colleagues retrospectively reviewed the charts of 250 IIH medically treated IIH patients from two centers and tabulated results on 56 patients that had at least 6 months of follow-up and otherwise met entry criteria.

Pseudotumor Cerebri Symptoms
The common symptoms are headache, dizziness and impaired vision.
Headache is often worse on awakening, and can be intensified by coughing, laughing, bending over, crying, and increased physical activity.
Impaired vision shows up in several ways. It can be visual blurring, brief moments of dimming or loss of vision called transient visual obscurations (TVO's), small specks appearing or vision distortions (items moving up or down).
Blindness can occur.
Other symptoms affecting PTC patients include: shoulder/arm pain, neck pain, memory problems, awkward coordination, muscle weakness, fatigue, back pain, and depression.
Possible symptoms include dizziness, hearing loss, ringing in the ears or noises within the head called tinnitus Regarding headaches: a study showed 92% of patients interviewed had headaches; 93% of those with headaches said it was the most severe headache they had ever experienced. The head pain was described as a pulsing headache that kept increasing in intensity. Another description was a "pressure" headache, resembling a percolator. Seventy four percent of those with headaches had it on a daily basis. Can also experience neck stiffness or nausea.
