Pseudotumor Cerebri is an extremely painful and often disabling Neurological Disorder, which can result in loss of vision and even blindness. There is nothing "Pseudo" or "Benign" about a PTC patients very real experience (which can mimic a brain tumor), not to mention the serious threat to his or her vision and overall quality of life. PTC is a rare condition affecting approximately one to two people of 100,000, most often women. The "usual" onset is the mid-twenties, though people of all ages are susceptible (and there isn't much "usual" about this condition - it's very much individualized). Recent weight gain has been associated with PTC in some patients, though the physiological factors which would then cause PTC, still have not been proven or discovered. There are two forms of PTC: Primary (PIH) and Secondary (SIH). With both, PTC occurs spontaneously and no one has yet found out why. In PIH there is no known cause. With SIH there is an identifiable causative agent such as underlying disease, an intracranial blood clot (Dural Sinus Thrombosis), or certain drugs and medications; to name a few. But even with SIH, the mechanism which causes PTC remains unknown.

What is pseudotumor cerebri? Pseudotumor cerebri is a condition in which there is:
  • An increase in the pressure of the fluid surrounding the brain and spinal cord, the cerebrospinal fluid (CSF), leading most commonly to headache. No detectable problem in the CSF (pressure aside) or in the anatomy of the brain and spinal cord.
  • The cause of the increased CSF pressure is an unsolved mystery. What is pseudo about it? Pseudotumor cerebri mimics a brain tumor in elevating the CSF pressure. In this respect, it is like a brain tumor (tumor cerebri, in Latin). Pseudotumor cerebri is also called benign intracranial hypertension (high pressure within the head). Who are common characteristics of patients with pseudotumor cerebri? The majority of pseudotumor cerebri patients are young, female, and obese: girls in their teens who are overweight.

    Idiopathic Intracranial Hypertension is a condition of high pressure in the fluid around the brain. It is also known as pseudotumor cerebri because there are some of the signs and symptoms of a brain tumor without a brain tumor being present (pseudo meaning false). The space around the brain is filled with a water-like fluid. (Fig 1.) If there is too much of this fluid present, (for example, if not enough being absorbed) (Fig 2), the pressure around the brain rises. This is because the space containing the fluid cannot expand. It is this high pressure that produces the symptoms of idiopathic intracranial hypertension (idiopathic means unknown cause; intracranial means inside the head; hypertension means the fluid is under high pressure).     Pseudotumor Cerebri
    Pseudotumor cerebri is de verouderde benaming voor een ziektebeeld dat tegenwoordig Idiopathische Intracraniële Hypertensie wordt genoemd. Ook komt men wel de term "Benigne Intracraniële Hypertensie" tegen. De naam pseudotumor cerebri is te danken aan het feit dat men in het verleden eerst heeft gedacht dat de patiënt een hersentumor (gezwel in de hersenen) had, omdat deze zoals bij een hersentumor de verschijnselen vertoonde van verhoogde druk in het hoofd (of intracraniële hypertensie, zoals de vakterm luidt). Bij nader onderzoek bleek er toch geen hersentumor te zijn, vandaar de toevoeging "pseudo-" (wat betekent schijnbaar) bij tumor cerebri, en de toevoeging "benigne" (wat betekent goedaardig) bij intracraniële hypertensie. Omdat het beloop wat betreft de afwijkingen aan de oogzenuw toch niet altijd zo goedaardig is, is de term idiopathisch geïntroduceerd. Idiopathisch betekent niet anders dan dat de oorzaak onbekend is.
    Symptomen
    Zoals gezegd, uit de aandoening zich door verschijnselen van verhoogde druk in het hoofd. Dat zijn: hoofdpijn, misselijkheid en braken, soms dubbelzien, maar wat vooral van belang is: wazig zien door zogenaamde stuwingspapillen. Stuwingspapillen duiden op het gestuwd zijn van de intredeplaats van de oogzenuw in het oog. Normaal ziet deze intredeplaats er bij het oogspiegelen uit als een hol schoteltje met scherpe randen (zie Figuur 1).

    A condition of increased pressure within the brain cavity, pseudotumor cerebri usually manifests itself as a bulging fontanelle in an otherwise happy and healthy infant, or as a sudden onset of strabismus in a child. It is also more properly known now as benign intracranial hypertension. This condition may be associated with some medicines (the antibiotic tetracycline, used in infants years ago but no longer, was the most common drug-related cause), may arise as a complication of mastoiditis , or may just happen mysteriously. Because intracranial hypertension might not have a benign cause , if you should notice these symptoms immediate consultation with your doctor is of course in order.

Klinische Leitsymptome sind die Folgen einer diffusen intrakraniellen Drucksteigerung: - Kopfschmerz - Stauungspapille. - Affektion des N. opticus, Visusminderungen bis zur Erblindung. - Ein- oder doppelseitige Abducensparese. Die Entstehung eines Pseudotumor cerebri Syndroms ist pathogenetisch heterogen. Zwei übergeordnete pathogenetische Prinzipien dürften für die Mehrzahl der Fälle verantwortlich sein: (1) Zustände mit erhöhtem venösen Abflusswiderstand (a) Sinusvenenthrombose, (b) Adipositas (besondere bei jungen Frauen zumeist vorhanden) und (2) medikamentös induzierte Formen.
