It is likely due to high pressure caused by the buildup or poor absorption of cerebrospinal fluid in the subarachnoid space surrounding the brain. The disorder is most common in women between the ages of 20 and 50. Symptoms of pseudotumor cerebri, which include headache, nausea, vomiting, and pulsating intracranial noises, closely mimic symptoms of brain tumors. Is there any treatment?

Pseudotumor Cerebri Signs and Symptoms
Pseudotumor cerebri (PTC) is encountered most frequently in young, overweight women between the ages of 20 and 45. Headache is the most common presenting complaint, occurring in more than 90 percent of cases. Dizziness, nausea, and vomiting may also be encountered, but typically there are no alterations of consciousness or higher cognitive function. Tinnitus, or a "rushing" sound in the ears, is another frequent complaint. Visual symptoms are present in up to 70 percent of all patients with PTC, and include transient visual obscurations, general blurriness, and intermittent horizontal diplopia. These symptoms tend to worsen in association with Valsalva maneuvers and changes in posture. Reports of ocular pain, particularly with extreme eye movements, have also been noted. Pathophysiology
Pseudotumor cerebri is a syndrome disorder defined clinically by four criteria: (1) elevated intracranial pressure as demonstrated by lumbar puncture; (2) normal cerebral anatomy, as demonstrated by neuroradiographic evaluation; (3) normal cerebrospinal fluid composition; and (4) signs and symptoms of increased intracranial pressure, including papilledema. While the mechanism of PTC is not fully understood, most experts agree that the disorder results from poor absorption of cerebrospinal fluid by the meninges surrounding the brain and spinal cord. The subsequent increase in extracerebral fluid volume leads to elevated intracranial pressure. However, because the process is slow and insidious, there is ample time for the ventricular system to compensate and this explains why there is no dilation of the cerebral ventricles in PTC. Increased intracranial pressure induces stress on the peripheral aspects of the brain, including the cranial nerves. Stagnation of axoplasmic flow in the optic nerve (CN II) results in papilledema and transient visual obscurations; when the abducens nerve (CN VI) is involved, the result is intermittent nerve palsy and diplopia.

Pseudotumor Cerebri
Pseudotumor cerebri, also called benign intracranial hypertension, literally means "false brain tumor." It is caused by increased pressure within the brain and is most common in women between the ages of 20 and 50. Symptoms of pseudotumor cerebri, which include headache, nausea, vomiting, and pulsating intracranial noises, closely mimic symptoms of brain tumors, possibly because of the abnormal buildup of cerebrospinal fluid placing pressure on the brain. Treatment for pseudotumor cerebri is generally symptomatic. Pressure may be controlled by removing excess fluid with repeated spinal taps or by shunting. Steroids may be prescribed to reduce swelling of brain tissue. Hyperosmotic drugs may be used to reduce fluid buildup. Once the diagnosis is made and the disorder is treated, pseudotumor cerebri generally has no serious consequences. If visual loss occurs, however, it may be premanent regardless of treatment. In some cases, pseudotumor cerebri recurs.

The Dandy Criteria (originally penned by Walter E. Dandy in 1937) delineates the diagnostic paradigm for PTC. Individuals such as J. Living with Pseudotumor Cerebri
Pseudotumor cerebri is a relatively rare disease. (We estimate there are about 12,000 people living in the United States with this disease.) The majority of PTC patients are women of child-bearing age with a tendency to be overweight. However, there are PTC patients of all ages- male and female, and all weight ranges- from obese to downright skinny.  Pseudotumor cerebri is also an individualized disease. Each person responds differently regarding symptoms, treatment, and side effects. There is no "standard" for how the condition will appear, how long it will last, whether
it will come back, or the degree it will impact your life.
There are people who have success with medication or treatment, and their PTC goes away. There are others who become disabled with this condition, and
treatment or surgery is ineffective.

PSEUDOTUMOR CEREBRI I was diagnosed with PTC almost 12 years ago. I had been having extreme headaches and eye pain. My regular doctor was concerned because two years earlier I had been sick with encephelitis. He sent me to a Neurologist and had him check me out.
The Neurologist looked into my eyes and told me to go straight to the imaging center for an MRI and CT Scan. He then instructed me to head straight to the hospital where I would have a Lumpar Punctur (Spinal Tap) the next morning. He told me not to go home but straight to those locations. This of course frightened me because I thought perhaps I had a real tumor. So I did exactly what he told me to do. That night in the hospital, I had several doctors come look in my eyes to see the pressure. It was ok at first but since I already had this blinding headache, and the light they shined made it worse, I finally told them "ENOUGH!" The next day I had the LP and went home. The headache lessened for a few days then came back with a vengence. The doctor put me on Diamox 1000 mg to start. It seemes to help but it sure made my hands and feet tingle! I was also told to eat a lot of bananas and take pottassium supliments.

Pseudotumor Cerebri Overview Synonyms: Idiopathic Intracranial Hypertension, Benign Intracranial hypertension Epidemiology 3.5/100,000 in women 15-44 years 19.3/100,000 in women ages 20-44 years who are 20% or more above ideal body weight. Rare in patients over 45 years of age, men and slim adults, need to consider a secondary cause or another disease. Symptoms Headache: worsen by bending forward. cough, sneezing. Neck and back pain are often prominent. Brief episodes of monocular or binocular visual loss that may be partial or complete. Pulsatile tinnitus, may be unilateral or bilateral, described as a heartbeat or whooshing sound. May have VI nerve palsy. Differentiate papilledema from pseudopapilledema Difficult to differentiate with a direct ophthalmoscope. Tilted optic nerves and optic disc drusen, both benign conditions, can simulate optic nerve head swelling. Infiltrative optic neuropathies can produce elevation of the optic disc.
