The presence of protein in urine is more than just a diagnostic test for kidney disease. New and increasing evidence shows: 1) that the presence of even relatively small increments of protein or albumin in urine is an early sign of kidney injury; 2) persistent proteinuria is associated with loss of kidney function; 3) the magnitude of persistent proteinuria directly correlates to the rate of loss of kidney function; 4) interventional measures that reduce the amount of persistent proteinuria slow the progression of chronic kidney disease; 5) proteinuria is a strong and independent predictor of increased risk for cardiovascular morbidity and mortality, especially in certain high risk groups such as diabetics, hypertensives, the elderly, and those with chronic kidney disease and 6) there is a direct correlation and agreement across the range of proteinuria of death from cardiovascular disease.

PROTEINURIA
I. Quantification
  • 150-3500mg/d = "proteinuria" >3500mg/d = "massive proteinuria"; usually mostly albumin If hepatic albumin synthesis can't keep up, "nephrotic syndrome" results: hypoalbuminemia, edema due to reduced intravascular oncotic pressure, and hyperlipidemia
  • II. Testing for proteinuria
  • Dipsticks Respond best to albumin; may give false negatives with other proteins Use fasting morning samples to avoid false negative from dilute urine "Trace" >50mg/l protein If dipstick positive, do 24h collection to quantify If 24h urine positive, do protein electrophoresis If mainly albumin, it's a glomerular lesion If lots of Bence-Jones, may be multiple myeloma
  • III. Mechanisms of proteinuria
  • Tubular Tubular disease, e.g. tubulointerstitial nephropathy, causes excretion of these proteins with no albuminuria! Usually excrete 1-3g/d protein Albumin isn't lost, so no edema/hyperlipidemia

    OBJECTIVE To evaluate the rate of progression of renal disease in proteinuric type 2 diabetic patients with and without retinopathy. RESEARCH DESIGN AND METHODS Thirty-eight proteinuric type 2 diabetic patients with diabetic retinopathy and 27 without were enrolled in an observational study for the evaluation of rate of glomerular filtration rate (GFR) decline and followed up for a median period of 6 years. GFR was determined at least once per year, and blood pressure, glycated hemoglobin, and proteinuria were determined every 4 months. CONCLUSIONS The rate of progression of renal disease in proteinuric type 2 diabetic patients with retinopathy is faster than that observed in those without retinopathy. The screening for retinopathy identifies patients at high risk for rapid deterioration of kidney function.

Greene, Clinical Medicine I found the following things about Proteinuria: Adults usually excrete less than 200 mg of protein in the urine per day, whereas the glomeruli receive almost 10 kg every 24 hours! That is the result of the kidneys ability to filter and excrete waste molecules while retaining several proteins. Conservation of proteins:
    - the tubuli are able to reabsorb and recycle filtered proteins
    - the glomerular basement membrane serves as a anatomic and electrostatic barrier to macromolecules The filtration barrier:
    - fenestrated endothelial cells; most molecules can pass through easily
    - basement membrane: trilaminar glycoprotein structure which serves as the most discriminating component of the filtration barrier
    - large molecules (>40 000 Dalton molecular weight) encounter greater impedance than smaller ones
    - protein bound molecules and protein molecules bound to each other have difficulties negotiating the membrane
    - as the basement membrane carries a negative charge, all molecules that are negatively charged at plasma pH are repelled

    The renal proximal tubular epithelial cell is capable of mediating tubulointerstitial inflammation, a final common pathway leading to end-stage kidney failure in many forms of proteinuric kidney diseases. We are one of the active research groups studying the molecular mechanisms through which abnormally filtered urinary proteins switch on the proinflammatory phenotype of renal tubular cells. Basic research topics :
    • Primary culture of human renal proximal tubular epithelial cells Renal tubular cell expression of complement, proinflammatory and fibrogenic chemokines and cytokines, growth factors, and adhesion molecules in proteinuria Elucidation of intracellular signal transduction pathways and tubuloglomerular feedback mechanisms
