What's the role of proteins in the body?
Proteins are essential in our bodies. Normally, proteins move around in our blood, carrying food, hormones and medicine to all parts of the body. They also help keep water inside tiny blood vessels. Return to top
What is proteinuria?
Proteinuria is the name of a medical condition. It means protein is in the urine. As blood moves through the kidneys, the kidneys filter out waste products, excess fluid and salts. The clean blood cycles back through the body. Most proteins are too big to go through the kidneys, so usually no protein is found in urine. If your doctor finds protein in your child's urine, that means your child's kidney may not be working as they shouldpossibly because of inflammation (swelling). Sometimes infection or toxic chemicals damage the kidneys, and this makes protein show up in the urine. Dipping a special strip of paper into a sample of urine tells your doctor if protein is in it. If only a little protein is in the urine, your child probably has a benign (harmless) condition such as orthostatic proteinuria (see below). Your doctor might do a few other tests to make sure.

    Definitions
    Proteinuria is the excretion of protein in the urine. We all have a small amount of protein in our urine. A normal person excretes less than 0.15g protein per day. Nephrotic syndrome occurs when there is excessive protein loss by the kidney. Many conditions result in mild proteinuria, but nephrotic syndrome occurs when this protein loss is very much exaggerated. In nephrotic syndrome, protein excretion is typically greater than 3g/day. Nephrotic syndrome typically causes other symptoms such as
    • Oedema (swollen legs) Weight gain (due to the fluid retention)
    In addition, doctors may recognise nephrotic syndrome from abnormal laboratory test results such as a reduced blood albumin level.

    Proteins are essential in our bodies. Normally, proteins move around in our blood, carrying food, hormones and medicine to all parts of the body. They also help keep water inside tiny blood vessels. A basic collection of urine Or 24 hour urine collection. This test helps show how well your child's kidneys are working. Your doctor may also do some blood tests. If a lot of protein is found in your child's urine, a more serious kidney disease might be the problem. Proteinuria doesn't cause pain. But when a lot of protein is in the urine, the level of protein in the blood may go down. This can cause swelling in your child's eyelids, ankles and legs. High blood pressure is another sign of this problem. What is Orthostatic Proteinuria ? Orthostatic proteinuria occurs in some older children and teenagers. The word orthostatic means "upright." The condition is called "orthostatic proteinuria" because protein goes into the urine only when the child is in the upright position (standing up). Children with this condition have no kidney damage but, for some unknown reason, they lose protein into the urine during the day when they are active. At night, while they sleep, their kidneys don't let any protein into the urine. Your doctor diagnoses this harmless condition by checking 2 urine samples. The first is collected in the morning, right after your child gets up. The second sample is collected throughout the day. The 2 samples are kept in separate containers. If your child has orthostatic proteinuria, the morning sample won't have protein in it, but the urine collected during the day will have protein in it.

    Proteinuria Home Health Center Health and Medical News Proteinuria Proteinuria is the presence of excessive protein in the urine, usually greater than 30mg/dl on a spot sample or more than 150 mg/24 hours in adults. It is generally an unwelcome surprise in underwriting, because many times it is a finding in an otherwise healthy and asymptomatic individual who was headed straight for a standard or preferred policy issue that is suddenly derailed. Proteinuria can be persistent (present in all samples) or intermittent (caused by a transient illness or an acute problem).
    When proteinuria is constant, it is usually a sign of underlying kidney disease, and secondarily a cause of increased mortality. Insurance studies have shown a mortality of almost 200% in applicants with significant proteinuria in those who had no symptoms of disease, and far greater than that in those where the protein in the urine was in large amounts. Diseases of the heart and circulatory system were the most important causes of eventual mortality in these people. Additionally kidney failure was a leading cause of illness as well.
    Proteinuria can be the result of advanced or end stage disease from a different systemic illness. Diabetes is the most important of these. In diabetes, the presence of protein in the urine generally means advanced or uncontrolled disease. The blood vessels in the kidneys are affected, but it is important to realize that if this is happening on the level of the excretory system, it is also happening in other blood vessels in the body as well, such as the heart.     Diagnosis/Definition
    • Greater than 150 mg of proteinuria per day.
    Initial Diagnosis and Management
    • Diagnosis is usually suspected on the basis of an abnormal dipstick urinalysis. The suspicion should be confirmed by either a 24-hour urine for protein or a spot urine protein to creatinine ratio of greater than 0.15 (equivalent to or greater than 150 mg of urinary protein daily). Proteinuria is a marker of intrinsic renal disease, and causes of this need to be assessed. Perform a history and physical examination to evaluate for evidence of a systemic disorder that may cause glomerulonephritis, i.e., signs/symptoms to suggest collagen vascular disease such as SLE, cancer, chronic infections, or diabetes. Evaluate for evidence of nephrotic syndrome: serum albumin (low), cholesterol (high), the presence of edema, and greater than 3 gm of proteinuria daily.
