Bacterial prostatitis. Bacterial prostatitis is an infection of the prostate. It can be acute or chronic. What is bacterial prostatitis?

Bacterial Prostatitis
Bacterial prostatitis is an infection of the prostate. It can be acute or chronic. Symptoms include pain, mainly at the base of the penis and around the anus. Treatment includes a course of antibiotics for 4-6 weeks.
What is bacterial prostatitis? Bacterial prostatitis means you have an infection of your prostate gland. There are two types of bacterial prostatitis - acute (sudden onset) and chronic (persistent). (Another leaflet deals with non-infective prostatitis which is also called chronic pelvic pain syndrome.) What is the prostate gland? Only men have a prostate gland. It lies just beneath the bladder. It is normally about the size of a chestnut. The urethra (the tube that passes urine from the bladder) runs through the middle of the prostate. The prostate helps to make semen, but most semen is made by another gland nearby (the seminal vesicle). What is the difference between acute and chronic bacterial prostatitis? ACUTE BACTERIAL PROSTATITIS
This is when symptoms develop quickly. It usually occurs at the same time as a urine infection. So, you may also have a bladder infection (cystitis). Bacteria (bugs) are found in your urine if a urine sample is taken. Symptoms include:
  • Pain from your prostate, which may be severe. You feel this mainly at the base of your penis, around your anus, and lower back. Pain may spread to your penis and testes. Passing stools (faeces) can be painful.

enign prostatic hyperplasia or BPH. If you're like most men who've been given this diagnosis by their doctor, chances are you haven't been told the whole story. Here are the answers to some commonly asked questions about BPH. What is BPH? To begin with, "benign" means not malignant. Prostatic simply means prostate. And hyperplasia means "excessive formation of cells." So when your doctor tells you "you suffer from benign prostatic hyperplasia or BPH," what he's really telling you is you have a nonmalignant enlarged prostate. Who Is Affected By BPH?

7.08 Prostatitis
Presentation
A man complains of fever, chills, perineal or low back pain, and may have urinary urgency and frequency, as well as signs of obstruction to urinary flow ranging from a weak stream to urinary retention. On gentle examination, the prostate is swollen and tender. The infection may spread from or into, the contiguous urogenital tract (epididymis, bladder, urethra), or the bloodstream.
What to do:
  • Perform a rectal examination and only once, gently palpate the prostate to see if it is tender, swollen, or edematous.
  • Culture the urine to help identify the organism responsible (although there is no guarantee that the bacteria in the prostate will be in the urine).
  • For patients 35 years and younger, treat for gonorrhea and other urethritis with ceftriaxone (Rocephin) 125mg im to1000mg iv and azithromycin (Zithromax) 1000mg po.
  • For men over 35 years old, begin empirical treatment with ciprofloxacin 400mg iv, then 500mg po bid.
  • Arrange for urological followup.

To put it simply, prostatitis is inflammation of the prostate gland. Here are some of the main classifications of prostatitis:
    • Acute bacterial prostatitis
      Chronic bacterial prostatitis
      Nonbacterial prostatitis
Back to top 2. What are the symptoms of prostatitis?
Each type of prostatitis has a slight variation in signs and symptoms:
    • Acute bacterial prostatitis
Fever and chills Pain in lower back or rectum Urinary symptoms
      Chronic bacterial prostatitis
Relapsing urinary tract infections Painful urination or other voiding problems Genital pain
      Nonbacterial prostatitis
Urinary symptoms such as difficult or painful urination
Back to top 3. How is prostatitis diagnosed?
Since the prostate lies in front of the rectum, your doctor will most likely perform a digital rectal examination (DRE) by inserting a gloved, lubricated finger into the rectum. By doing this, he or she will be able to feel the prostate and determine if it is enlarged or if it has lumps or other abnormalities. The physician may also check for possible infection or other prostate or urinary tract disorders. Back to top 4. Who gets prostatitis?

Prostatitis: The Basics What is it? Prostatitis is an acute or chronic inflammation of the prostate, a walnut-size gland located just below the bladder in men, which produces about 30 percent of the fluid portion of semen. Inflammation may be caused by an infection. Acute prostatitis is characterized by the sudden onset of symptoms; chronic prostatitis causes persistent, long-term symptoms that are less severe.
Chronic prostatitis may affect as many as 35 percent of men over age 50. An inflamed prostate may press upon the urethra and completely block the flow of urine. Additionally, if left untreated, acute prostatitis may lead to a prostate abscess, an infected, pus-filled cavity. Treatment for the acute and chronic forms of prostatitis may differ considerably. What causes it?

    Q. When is Prostatitis NOT Prostatitis?
    A. When the diagnosis is made by a Physiotherapist.
By John D. Perry, PhD How tragic it is that the various professions seldom speak to each other, nor read each other's literature. While urologists discuss "prostatitis", physical therapists talk about "Levator Ani Syndrome". Are these just two different perspectives on the same disease? Or are we dealing with two distinctly different diseases? And, if different, is there any overlap between "prostatitis" and "levator ani"? Is this another classic "chicken vs. egg" question? And, why should men's diseases be any different from women's? These questions are provoked by a recent article by four urologists [Zermann et al, 1999] which asks "whether chronic pelvic pain syndrome may be a consequence of functional disease within the pelvis". They studied clinic patients with "chronic abacterial prostatitis"; It turns out that for 88 percent of 103 patients, "myofascial tenderness" in the rectal area was "always associated with the inability to relax the pelvic floor efficiently"; "rarely did these patients learn to relax and control pelvic muscles." Measured with urodynamic instruments, they found 92.2% of patients had "dysfunction of the pelvic floor muscles."

    Abstract
BACKGROUND: Chronic prostatitis is a common diagnosis but the etiology of this disease is unknown. We wished to catalog symptoms and etiologic factors for chronic prostatitis to gather epidemiological data about the disease. We also wished to determine the feasibility of doing this using the internet. METHODS: A 54 question survey was designed and posted to several internet newsgroups dealing with chronic prostatitis. The questions dealt with symptoms, predisposing factors, demographic data and sexual practices. Responses were collected by electronic mail and tabulated at a central site. RESULTS: We received 163 completed surveys over a 2 month period. Nine percent of responses came from individuals outside of the U.S. or Canada. Only 9.8% of completed surveys contained one or more unreadable answers. The responding population consisted of relatively young men (mean 43 years) with a chronic, relapsing set of symptoms mostly characterized by pain at various locations in the pelvis, irritative voiding symptoms and pain associated with ejaculation. Most respondents were potent and had a single sexual partner.
