Benign prostatic hypertrophy is a condition common to men as they age. It is caused by a combination of androgens (male hormones) with a permissive effect of estrogens (female hormones). More specifically, testosterone is converted to a second hormone, dihydrotestosterone by the enzyme 5 alpha-reductase. Dihydrotestosterone is what is responsible for post-pubertal enlargement of the prostate gland. Symptoms
The symptoms of BPH are many. Nocturia (going to the bathroom at night), urinary frequency, decreased force of urinary stream, double voiding, post-void dribbling, hesitancy of urination and intermittency of the urinary stream. Subjective effectiveness was observed in the improvement of sense of residual urine (92%), retardation (86%), night frequency (85%), strain on urination (56%), protraction (53%) and forceless urinary stream (53%). The overall subjective effectiveness was 80% of patients, and the overall objective effectiveness was 54% of patients. Night frequency, residual urine volume and tidal urine volume were improved significantly. The overall effectiveness was 80%. No side effects were observed. Benign prostatic hypertrophy (BPH)
The symptoms of BPH are a progressive frequency and urgency, difficulty initiating urination, decreased urine flow and force, and nocturia. Upon rectal examination the prostate is enlarged and has a rubbery consistency, and an abdominal exam may reveal a distended bladder that is palpable or percussible. The congestion of the of the superficial veins of the prostate and the trigone muscle of the bladder can cause hematuria if the patient strains while trying to void. Burning sensations and fever indicate secondary infection. The treatment of BPH is essentially the same as it is for chronic nonbacterial prostatitis. Treatment strategies for secondary infection will resemble that of bacterial prostatitis. Saw Palmetto (Serenoa serrulata) is one botanical that has undergone extensive investigation in the treatment of BPH, and has been found to inhibit 5-alpha-reductase, reduce inflammation, lower androgenic activity, and prevent spasm. It is by no means the only herb for BPH and should not be relied upon exclusively. Pumpkin and squash seeds (Cucurbita spp.)

  • Gradually progressive obstruction of the urinary outflow due to prostatic hypertrophy that may eventually result in acute or chronic urinary retention.
Initial Diagnosis and Management
  • Patients with obstructive and irritative voiding symptoms should complete the International Prostate Symptom Scoring (IPSS) Sheet. Contact Urology to obtain a copy of this. Urinalysis to rule out infection or to discover microhematuria. PSA and digital rectal exam to evaluate for prostate cancer. IVP is not indicated unless patient has hematuria.

The prostate makes some of the milky fluid (semen) that carries sperm. The gland is the size of a walnut and is found just below the bladder, which stores urine. The prostate wraps around a tube (the urethra) that carries urine from the bladder out through the tip of the penis ( Figure 1 ). During a man's orgasm (sexual climax), muscles squeeze the prostate's fluid into the urethra. Sperm, which are made in the testicles, also go into the urethra during orgasm. The milky fluid carries the sperm through the penis during orgasm.

Benign Prostatic Hypertrophy
What is it? Benign prostatic hypertrophy is also called "BPH." It is when the prostate gland slowly grows until it is larger than normal, or "enlarged." BPH is a very common problem in aging men. It is more common in men over 45 to 50 years of age. The prostate is a male sex gland that makes fluid called semen. It sits below the bladder and wraps around the urethra like a donut. The urethra is a tube that carries urine from the bladder into the penis. The urine is then passed out of the body. As the prostate grows, it can squeeze the urethra making it harder to pass urine. Causes: It is not known what causes BPH. It may be just part of getting older. Some care givers believe it is caused by the continual and cumulative exposure to testosterone (male sex hormone) as men age. Signs and Symptoms: Most men with BPH feel like they need to pass urine often. Some men may pass little or no urine. You may dribble or leak urine. You may pass urine while you sleep. Or you may need to wake up at night to urinate . You may have to push to start your urine stream. You may feel that you need to pass more urine even if you just did. You may also think your urine stream is weak. Your urine may look pink or red. This may mean there is blood in it. Wellness Recommendations: If you have prostatic enlargement you should avoid stimulants or decongestants (often found in over-the-counter cold and allergy medications) as these can contract the muscle fibers in the prostate and make the symptoms much worse.
