Benign Prostatic Hypertrophy (BPH)
As men age, many begin to suffer annoying urinary problems, frequent trips to the bathroom, waking at night to urinate, inability to fully empty the bladder, and increased sensations of urgency. These annoyances are often caused by benign prostatic hyperplasia. The prostate is a walnut-sized gland that forms part of the male reproductive system. The gland is made of two lobes, or regions, enclosed by an outer layer of tissue. The prostate is located in front of the rectum and just below the bladder, where urine is stored. The prostate also surrounds the urethra, the canal through which urine passes out of the body. Scientists do not know all the prostate's functions. One of its main roles, though, is to squeeze fluid into the urethra as sperm move through during sexual climax. This fluid, which helps make up semen, energizes the sperm and makes the vaginal canal less acidic. It is common for the prostate gland to become enlarged as a man ages. Benign Prostatic Hypertrophy (BPH): Nonmalignant (noncancerous) enlargement of the prostate gland. It is also known as benign prostatic hyperplasia (also abbreviated as BPH). As a man matures, the prostate goes through two main periods of growth. The first occurs early in puberty, when the prostate doubles in size. At around age 25, the gland begins to grow again. This second growth phase often results, years later, in BPH.

Benign prostatic hypertrophy (BPH) is an enlargement of the prostate associated with aging; etiology is unknown. Severe obstructive BPH can lead to urinary retention, infections, hematuria, and renal insufficiency. Surgical treatment is performed for relief from and improvement of symptoms by removing as much tissue obstructing the urethral passage as possible. In laser prostatectomy, light energy is absorbed and converted into heat to cause coagulation or vaporization of tissues. The heat of the laser irradiation induces protein denaturation and coagulation necroses.

    Enlarged Prostate
Definition: The prostate gland produces the liquid in which sperm are expelled from the penis during ejaculation. As the prostate enlarges, which happens to almost all men as they get older, it can press on the urethra (the tube that empties urine from the bladder to the outside) causing problems with urination. Prostate enlargement is often called benign prostatic hypertrophy or hyperplasia (BPH). It is not cancer and it does not put you at increased risk for prostate cancer.
Alternative Names: BPH; Benign prostatic hypertrophy (hyperplasia); Prostate - enlarged
Causes, incidence, and risk factors: The actual cause of prostate enlargement is unknown. In addition to age, overgrowth of the prostate seems to require normal function of the testicles, which produce testosterone. This is known because men who have had their testicles removed at a young age (for example, for testicular cancer) do not develop BPH. Similarly, if the testicles are removed after a man develops BPH, the prostate begins to shrink in size.
The likelihood of developing an enlarged prostate increases with age. In fact, BPH is so common that it has been said, "All men will have an enlarged prostate if they live long enough!" A small amount of prostate enlargement is present in many men over age 40 and more than 90% of men over age 80. No risk factors have been identified other than having normally functioning testicles.

The most common prostate condition is BPH (benign prostatic hypertrophy) In Europe approximately 75% of men over 50 develop symptoms of BPH What is BPH (benign prostatic hypertrophy)?
    • BPH is a non-malignant (non-cancerous) enlargement of the prostate gland - a common occurrence in older men. It is caused by an overgrowth of prostate cells. This enlargement constricts the urethra so the flow of urine is reduced, making it increasingly difficult to empty the bladder. BPH generally begins in a man's 30's, evolves slowly and usually only causes symptoms after 50. Although BPH is an extremely common condition, only 10 per cent of men need medical or surgical intervention. It is not a precursor to prostate cancer.

Clinical Policy Bulletins Medical Clinical Policy Bulletins Number: 0079
Subject: Benign Prostatic Hypertrophy (BPH) Treatments

    Important Note
This Clinical Policy Bulletin expresses Aetna's determination of whether certain services or supplies are medically necessary. Aetna has reached these conclusions based upon a review of currently available clinical information (including clinical outcome studies in the peer-reviewed published medical literature, regulatory status of the technology, evidence-based guidelines of public health and health research agencies, evidence-based guidelines and positions of leading national health professional organizations, views of physicians practicing in relevant clinical areas, and other relevant factors). Aetna expressly reserves the right to revise these conclusions as clinical information changes, and welcomes further relevant information. Each benefit plan defines which services are covered, which are excluded, and which are subject to dollar caps or other limits. Members and their providers will need to consult the member's benefit plan to determine if there are any exclusions or other benefit limitations applicable to this service or supply. The conclusion that a particular service or supply is medically necessary does not constitute a representation or warranty that this service or supply is covered (i.e., will be paid for by Aetna) for a particular member. The member's benefit plan determines coverage. Some plans exclude coverage for services or supplies that Aetna considers medically necessary. If there is a discrepancy between this policy and a member's plan of benefits, the benefits plan will govern. In addition, coverage may be mandated by applicable legal requirements of a State, the Federal government or CMS for Medicare and Medicaid members. CMS's Coverage Issues Manual can be found on the following website:
