As the prostate enlarges, which happens to almost all men as they get older, it can press on the urethra (the tube that empties urine from the bladder to the outside) causing problems with urination. Prostate enlargement is often called benign prostatic hypertrophy or hyperplasia (BPH). It is not cancer and it does not put you at increased risk for prostate cancer.

Nonoperative therapy, such as drug therapy or lifestyle modification, is aimed primarily at reducing bothersome lower urinary tract symptoms and preventing serious morbidity such as urinary retention and renal function impairment. As more men reach advanced age, it becomes crucial for the primary care physician to be aware of the epidemiology, pathophysiology, natural history, clinical presentation, and therapeutic modalities available for the treatment of clinically significant cases. B enign prostatic hypertrophy (BPH) is the nonmalignant enlargement of the prostate gland from an increase in cellular growth. BPH is the most common benign neoplasm among men, and its incidence increases steadily with age. Clinical BPH is well defined and quite prevalent: Up to 20% of men between 40 and 64and 40% of men older than 65are afflicted. Histologic BPH is found in 50% of men older than 60 and up to 88% of men older than 80.

    AHCPR funded a multidisciplinary Patient Outcomes Research Team (PORT) to assess surgical and non-surgical interventions for benign prostatic hyperplasia (BPH) and localized cancer of the prostate. When a man has to stand for long time before he can start urinating, he is usually diagnosed as having Benign Prostatic Hypertrophy, which means that his prostate gland is so large that it squeezes his urinary tube and blocks the flow of urine from his bladder to the outside. Doctors don't have the foggiest idea what causes BPH, but they continue to cut out or burn prostates, even though the surgery often causes men to become incontinent, impotent, ejaculate backwards into the bladder, and have prostate infections.     Definition
    Benign prostatic hyperplasia is non-malignant enlargement of the prostate. The prostate is a walnut-sized gland located at the neck of the bladder surrounding the urethra. It is part of the male reproductive system.
    Causes
    The exact cause of BPH is unknown. It may be related to changes in hormone levels as men age. These changes probably cause the prostate to grow. Eventually, the prostate becomes so enlarged that it puts pressure on the urethra. This causes the urethra to narrow or, in some cases, close completely.
    Risk Factors
    A risk factor is something that increases your chance of getting a disease or condition. The main risk factor for BPH is being over 50 years old. By age 60, 50% of all men will have BPH. By age 80, 90% of men will have BPH.

    GENERAL INFORMATION: What is it?
    • Benign (buh-nine) prostatic (prah-stah-tik) hypertrophy (hi-per-tro-fee) is when the prostate (prah-state) slows grows larger. It is also called "BPH." The prostate is a male sex gland that helps makes semen. It sits below the bladder and wraps around the urethra like a donut. The urethra (u-ree-thruh) is the tube that carries urine from the bladder into the penis. The urine is then passed out of the body.
      As the prostate grows, it can squeeze the urethra (u-ree-thruh) making it harder to urinate (pass urine). This can block urine flow and cause urinary problems. BPH is very common in men over 45 years of age but rarely causes problems before age 60. Tests will be done to learn about your prostate. You may need drug treatment or prostate surgery if your urinary problems get worse.

    Causes: It is not known what causes BPH. It may be just part of getting older. Some caregivers believe it is caused by a change in hormones as men age.

    Contents of This CME Activity The past decade has seen an unprecedented evolution in our understanding of health and disease, accompanied by rapid development of new therapies and therapeutic approaches. As this expansion of medical knowledge was occurring, however, it was perhaps inevitable that some of the original treatment options for some of the more general disease states should lose their status in the medical continuum. But should the latest state-of-the-art advances always relegate the established standard-of-care to the history books? One of the staples of medical treatment for two important conditions, hypertension and benign prostatic hypertrophy (BPH), has been blockade of the alpha-adrenergic system, with agents such as prazosin (MiniPress), Tamsulosin (Flomax), Terazosin (Hytrin), and doxazosin (Cardura). However, the position of alpha blockade has slipped a bit in recent treatment guidelines (eg, the JNC-VI hypertension guidelines) and clinical trials (ALLHAT, final results to be announced at the end of 2002). And yet, particularly in older males, alpha blockers remain an attractive and reliable treatment option.
