Description
Some researchers suggest that BPH may develop as a result of "instructions" given to cells early in life. According to this theory, BPH occurs because cells in one section of the gland follow these instructions and "reawaken" later in life. These "reawakened" cells then deliver signals to other cells in the gland, instructing them to grow or making them more sensitive to hormones that influence growth.

This enlargement constricts the urethra so the flow of urine is reduced, making it increasingly difficult to empty the bladder.
The prostate gland
The prostate is a gland about the size of a walnut that is only present in men. It is located just below the bladder and surrounds the urethra, the tube through which urine flows from the bladder and out through the penis. Please see the diagram, overleaf. One of the main functions of the prostate is to produce an important liquefying component of semen, which allows the sperm to move freely. The gland is divided into three zones, peripheral, transitional and central. With BPH, it is the central part where overgrowth of cells takes place.

As the prostate enlarges, which happens to almost all men as they get older, it can press on the urethra (the tube that empties urine from the bladder). This causes problems with urination. Prostate enlargement is often called benign prostatic hypertrophy or hyperplasia (BPH). It is not cancer and it does not put you at increased risk for prostate cancer. Causes, incidence, and risk factors Return to top The actual cause of prostate enlargement is unknown. In addition to age, overgrowth of the prostate seems to require normal function of the testicles, which produce testosterone. This is known because men who have had their testicles removed at a young age (for example, for testicular cancer) do not develop BPH. Similarly, if the testicles are removed after a man develops BPH, the prostate begins to shrink in size.
The likelihood of developing an enlarged prostate increases with age. In fact, BPH is so common that it has been said, "All men will have an enlarged prostate if they live long enough!" A small amount of prostate enlargement is present in many men over age 40 and more than 90% of men over age 80. No risk factors have been identified other than having normally functioning testicles.

BENIGN PROSTATIC HYPERTROPHY Gabe Mirkin, M.D. If you are a man who finds it difficult to start your stream, check with your doctor to see if you have an infection or a benign or cancerous enlargement of your prostate. Your doctor usually examines your prostate for potentially cancerous nodules, orders a PSA blood test and checks your urine for infection. If your doctor finds a nodule, he usually orders an ultrasound test and then cuts out small pieces of the nodules which are then analyzed for cancer. If he finds no cancer, he must check you for infection or benign prostate enlargement. It is so unusual to find bacteria in a man's urine, that it requires further tests to check your entire urinary system. PSA or Prostate Specific Antigen is made by prostate cells and released into the bloodstream. The larger your prostate, the more cells you have to release PSA into your bloodstream. Therefore, PSA is a measure of the size of the prostate gland, rather than just prostate cancer. A sudden rise in PSA means that the gland is enlarging rapidly which can be a sign of cancer, but it also can mean infection or benign enlargement. If you have an elevated PSA, burning on urination, feeling that you have to urinate all the time and terrible discomfort when your bladder is full, the odds are overwhelming that you have an infection, that can be very difficult to cure.

  • Signs and symptoms include decreased force and caliber of urinary stream, hesitancy, retention, postmicturition dribbling, double voiding (patient voids and is able to void again in 5 to 10 minutes), and overflow urinary incontinence (on straining or coughing). Irritative symptoms such as dysuria, frequency, nocturia, urgency, hematuria, and incontinence occur frequently. Flank pain during micturition, suprapubic pain, and azotemic symptoms occur less commonly. Exam . The bladder may be distended, and the prostate is enlarged, smooth, and symmetric. The prostate gland may be soft or firm and possibly nodular. However, the nodules lack the stony-hard consistency associated with carcinoma. Laboratory findings . UA may reveal signs of infection. If the obstruction has been severe enough to impair renal function, BUN and creatinine may be elevated. PSA may be elevated.

    THE PROSTATE What is the prostate? How can prostate abnormalities be detected? What are some common prostate abnormalities? What is the prostate? The prostate is a male sex gland that is important for reproduction in that it produces the majority of ejaculate fluid. It is a small walnut shaped gland that is located above the rectum and at the bottom of the bladder. The prostate surrounds the urethra (the tube that carries urine from the bladder) like a doughnut, at the point where the urethra connects to the bladder. The prostate gland continues to grow throughout most of a man's life. How can prostate abnormalities be detected? A rectal examination should be considered as part of the annual physical checkup for men beginning at age 50 years, unless there are factors that increase the risk of cancer (Afro-American, family history of prostate cancer, or working around cadmium), then the exams should start at age 40. By inserting a gloved finger in the rectum, the physician can feel the surface of the prostate through the wall of the bowel. Suspicious lumps, abnormal textures or hardness can be investigated through further testing. Unfortunately, there is no foolproof method of early detection. Today, researchers are evaluating new technologies and laboratory tests that could possibly improve the early detection rate of prostate cancer and other prostatic diseases.     Benign adenomatous hyperplasia of the periurethral prostate gland, causing variable degrees of bladder outlet obstruction.
    Etiology and Pathophysiology
    The etiology is unknown but may involve hormonal changes associated with aging. Multiple fibroadenomatous nodules occur in the periurethral region of the prostate gland, probably originating within the periurethral glands rather than in the true fibromuscular prostate (surgical capsule), which is displaced peripherally by progressive growth of the nodules. The hyperplasia may involve the lateral walls of the prostate (lateral lobe hyperplasia) or tissue at the inferior margin of the vesical neck (middle lobe hyperplasia). Histologically, the tissue is glandular, with varying proportions of fibrous stroma interposed. As the lumen of the prostatic urethra becomes compromised, urine outflow is progressively obstructed, accompanied by hypertrophy of the bladder detrusor, trabeculation, cellule formation, and diverticula. Incomplete bladder emptying causes stasis and predisposes to infection with secondary inflammatory changes in the bladder (chronic prostatitis, see below) and the upper urinary tract.     An extract of the fruit from the American saw palmetto plant has been used by native American Indians for hundreds of years to treat a variety of men's problems, and prostate gland swelling since the 1800s. Does it work? Systematic reviews [1,2] say that it does.
    Search
    The search for randomised controlled studies of Saw palmetto (using all those peculiar botanical names that plants have) against placebo or other treatments was thorough. It found 24 studies, 18 of which met the inclusion criteria. The mean age of men involved was 65 years and the mean duration of the studies was nine weeks.
    Results
    Compared with placebo, Saw palmetto improved peak urinary flow rate by an average of 1.9 mL/second and mean urinary flow rate by 2.2 mL/second (Figure 1). Saw palmetto did not decrease prostate size.

    Patient rating of prostate-related symptoms showed significant benefits for Saw palmetto over placebo. In six studies (Figure 2) 242/329 (74%) men had symptom improvement with Saw palmetto compared with 168/330 (51%) with placebo. This means that for every 4.4 men treated with Saw palmetto (95% CI 3.4 to 6.5) one will have symptoms improved who would not have done with placebo.
