Use: Used to preserve bone density in osteoporosis patients.
Pregnancy Risk Factor and Implications: C; only use this drug during pregnancy if the benefits of its use outweigh the risks. It is not know if this drug is excreted in human milk; recommended that don't nurse while on this drug.
Contraindications: Patients with an allergy to synthetic calcitonin-salmon should not use this drug. Warnings/Precautions: Administration of calcitonin-salmon has been reported in a few cases to cause serious allergic-type reactions (e.g., bronchospasm, swelling of the tongue or throat, and anaphylactic shock), and in one case, death attributed to anaphylaxis (with injection, no serious allergic-type adverse reactions have been reported with the nasal spray). Also, the incidence of osteogenic sarcoma is known to be increased in Paget's disease. Use with caution since the administration of calcitonin possibly could lead to hypocalcemic tetany under special circumstances.

or the climacteric Menstruation depends upon a functioning ovary and this upon a healthy pituitary gland . The regular rhythm may depend upon a center in the hypothalamus , which is in close connection with the pituitary. After menstruation the denuded uterine endometrium is regenerated under the influence of the follicular hormone, oestradiol. The epithelium of the endometrium proliferates, and about a fortnight after the beginning of menstruation great development of the endometrial glands takes place under the influence of progesterone, the hormone secreted by the corpus luteum. These changes are made for the reception of the fertilized ovum. Without fertilization the uterine endometrium breaks down in the subsequent menstrual discharge. Disorders of menstruation In most healthy women, menstruation proceeds regularly for thirty years or more, with the exceptions connected with childbirth. In many women, however, menstruation may be absent, excessive or painful. The term amenorrhoea is applied to the condition of absent menstruation. The terms menorrhagia and metrorrhagia describe excessive menstrual loss, the former if the excess occurs at the regular periods, the latter if it is irregular. Dysmenorrhoea is the name given to painful menstruation.

Fact Sheet: Premature Ovarian Failure (POF) A Brief Look at POF As has been said elswhere on the site, premature ovarian failure (POF) is a condition that used to be called "premature menopause." It's marked by post-menopausal hormone levels and no periods (amenorrhea) ocurring before the age of 40. While it's similar to menopause, it's not strictly menopause. There are several key differences. One of the most important differences is that many women with POF can and do still ovulate from time to time and can even get pregnant. (According to statistics, this occurs to roughly 8 to 10% of women with POF.) This is because POF doesn't necessarily mean lack of viable follicles. Yes, some women with POF do have early depletion of follicles (which is then, in effect, menopause). But others nearly half according to some experts actually still have follicles. The problem in this case is a "disconnect" between hormones and ovarian function or an autoimmune problem. Premature ovarian failure as with many conditions develops over time. Prior to full-blown POF (i.e. no periods for at least 4-6 months and post-menopausal hormone levels), you move through different stages through what's now called "primary ovarian insufficiency" (POI)

Premature Ovarian Failure Ovarian failure results when there are no eggs available for maturation. With ovarian failure the fertility prognosis is poor unless donor eggs are used. The FSH concentration (ideally with LH and estradiol concentrations) can confirm ovarian failure. In one study, women with gonadal (ovarian) failure and primary amenorrhea were consistently found to have no follicular activity on pathology reports (of the ovarian tissue) when the FSH concentration was greater than 40 IU/L. These data may not be entirely applicable to ovarian failure with secondary amenorrhea, but consistently elevated FSH concentrations (usually greater than 25 IU/L and certainly greater than 40 IU/L) with a coincident low estradiol concentration (to rule out a FSH producing pituitary tumor) and secondary amenorrhea strongly suggests ovarian failure.
Menopause is the absence of menstruation due to the depletion of eggs capable of maturation. The average age of menopause in the USA is about 51-52 years old. Less than 1% of women have "menopause" when younger than 40 years old, so that when this occurs it is called "premature ovarian failure" or POF. There appear to be two main causes of this accelerated loss of eggs such that there is total depletion of maturation capable eggs at a very young age (POF). First, there may be a chromosomal abnormality in the X chromosome affecting the genes that regulate the rate of egg loss. Second, autoimmune disease that produces antibodies that can attack and destroy the ovaries is able to result in early ovarian failure.

    Primary Ovarian Ectopic Pregnancy Progressed to 28 Weeks, Mimicking An Intrauterine Foetal Death
    Punam M Satpute, Sneha D Shirodkar, Vaishali Patil, Vijay R Badhwar Primary ovarian ectopic of 28 wks gestation mimicking an intrauterine foetal death is rare. The diagnosis is usually surgical, however, an unyielding cervical os and failure of medical induction of labour, should alert the obstetrician of an extrauterine gestation. Presented here is a 28 weeks primary ovarian ectopic with foetal death, that mimicked an intrauterine foetal death clinically, was missed by 2 transabdominal USG scans and was suspected only after failed medical induction of labour. Timely surgical intervention before disseminated intravascular coagulopathy sets in, is recommended.
    INTRODUCTION
    Primary ovarian ectopic pregnancy is an uncommon ectopic gestation and accounts to 0.3-3% of all ectopic gestations. It is rare to progress upto 28 weeks. It is usually known to mimic an ovarian tumour, but to mimic a 28 week intrauterine gestation with foetal death clinically, and mislead by ultrasound, makes this an interesting case.
    CASE REPORT
    A 30 year primigravida with 7 months of amenorrhoea, conceived spontaneously after 11 yrs of infertility, was referred as a c/o IUFD. O/E - A single IUFD of 28 wks in a longitudinal lie with cephalic presentation and a relaxed uterus were noted. Pervaginum - 2.5 cm long tubular uneffaced cervix with a pinpoint external os and a closed internal os were seen. USG confirmed an IUFD of 28 wks (spalding sign+). Since it was a precious conception, patient repeated and confirmed the IUFD with her own sonologist. Routine investigations were WNL. Coagulation tests (BT-50 secs, CT - 4 1/2 minutes) were also WNL.

Premature ovarian failure is defined as cessation of menstruation together with elevated gonadotrophin levels before the age of 40 years. It is generally estimated that 1% of women will have premature ovarian failure. Clearly if the ovaries are removed at surgery, or if they are subjected to radiotherapy or chemotherapy, ovarian failure may result. About a quarter of women presenting with secondary amenorrhoea in their late thirties will prove to have premature ovarian failure. The most frequent abnormality to be found when investigating premature ovarian failure is autoantibodies ( Q 6. 16 ) Chromosome abnormalities can be found in about 3% of women with premature ovarian failure. If a hormone test suggests a diagnosis of premature ovarian failure, it is advisable to repeat the test. The diagnosis of premature ovarian failure can be devastating to a woman particularly if her family has not been completed so careful and sympathetic counselling is essential. The possibility of the resistant ovary syndrome should also be explained ( ). The long-term health risks of oestrogen deficiency (Chapter 26) are increased for those with premature menopause and the virtues of hormone replacement should be emphasised.
