Primary Immune Deficiency Diseases Item Primary Immune Deficiency Diseases The Committee is pleased to learn that NCI will participate in a symposium, in conjunction with the Office of Rare Diseases, NICHHD, NIAID, and NHGRI, to investigate the relationship between primary immune deficiency diseases and cancer with the goal of identifying areas of scientific research that can be enhanced through appropriate funding mechanisms. The symposium will bring together leading national and international experts in cancer, pediatrics, immunology, and genetics. The Committee looks forward to reviewing the report of the symposium prior to the fiscal year 2000 appropriations hearing. The Committee also supports NCI's interest in the creation of a trans-Institute intramural clinic for the diagnosis of immune deficient patients. (p.69) Action taken or to be taken The NCI, in collaboration with the NIAID and NHGRI, has scheduled a symposium to convene in March 2000 on the relationship between cancer and primary immune deficiency diseases. The purpose of this symposium is to bring together experts in the fields of cancer, genetics and immunology to share their insights into the etiology of cancer in immunodeficient patients. The ultimate goal is to focus on areas of scientific research that can be enhanced through appropriate funding mechanisms. The timing of this symposium was delayed from late 1999 to early 2000 to allow for a more comprehensive gathering of interested parties.

Primary Immune Diseases The human body has a complex immune defense system to protect itself from foreign invaders. This function protects us from infectious diseases and even cancer. An abnormal immune system is unable to kill invading microorganisms effectively. There are 3 types of white blood cells in the immune system. Phagocytes process microinvaders for the immune system and are responsible for the formation of "pus." B-lymphocytes produce antibodies known as immunoglobulins (IgM. IgG, IgA and IgE) which attack circulating foreigners. IgM is the first antibodies formed in response to infection. IgG crosses the placenta to provide immunity to the newborn. IgA is present in large quantities in secretions like tears, saliva and mucus and helps protect the body. IgE protects against parasite infections and is responsible for allergy. The T-lymphocytes secrete chemicals called cytokines that mainly destroy invading microorganisms and help B-lymphocytes function. When immunodeficiency is suspected, an infant or child should not receive immunization with a live virus (oral polio, measles) as this can cause serious illness or even death. A deficiency of B-lymphocyte function such as in agammaglobinemia is treated with gamma globulin usually throughout life. Treatment of combined deficiencies of B- and T-lymphocytes can frequently be treated by bone marrow transplantation. This is a transfer of healthy bone marrow to the immunodeficient child. In bone marrow transplant a close match is very important, to prevent rejection or graft-versus-host disease which could destroy the tissues of the recipient. These transplanted bone marrow cells can grow into functioning T- and B- lymphocytes. Most of the patients can be helped by early detection and treatment of infections .Antibiotics, good hygiene and a good diet are invaluable supportive measures. Gene treatment to correct severe combined immunodeficiency looks promising for the future.

Primary immune deficiency diseases
One part of the immune system fights a variety of invading organisms using special cells called lymphocytes. B lymphocytes fight infection by producing proteins called antibodies, which travel through the body to attack specific microbes, particularly bacteria. T lymphocytes trigger a variety of defenses to combat viruses, fungi, and some cancer cells. Primary immune deficiencies can impair either B lymphocytes, T lymphocytes, or sometimes both. Deficiencies in B-lymphocyte production interfere with the production of certain types of antibodies known as immunoglobulins. The complete absence of the immunoglobulin gamma globulin is called agammaglobulinemia. One form of this serious disease typically develops in male infants under the age of two. These infants are susceptible to recurrent lung and sinus infections. The most common and mildest immune deficiency disease is selective IgA deficiency, which results from a lack of the disease-fighting antibody immunoglobulin A (IgA). Occurring in as many as 1 in 400 people, IgA deficiency may present no symptoms at all in some people while others may have an increased number of respiratory or gastrointestinal infections. Immune deficiencies of T lymphocytes prevent the body from fighting virus and fungal infections. People deficient in T lymphocytes typically develop persistent fungal infections affecting the skin, mouth, and vagina. Another T-lymphocyte deficiency is DiGeorge Syndrome, a disease characterized by abnormal development of the human embryo and fetus that leads to improper development of the fetus’s thymus, the gland situated behind the breastbone in the chest. This condition results in a deficiency in the number of T lymphocytes, specialized cells that develop under the influence of the thymus. Infants with DiGeorge Syndrome typically have characteristic facial deformities, heart disorders, and a high susceptibility to infection.

Overview
When people are born with a faulty immune system, they are said to have a primary immune deficiency or immunodeficiency. Unlike people with AIDS, caused by the human immunodeficiency virus or HIV, people with primary immunodeficiency (PI) diseases have inherited abnormal changes in cells of their immune system. The various types of immune cells each have their own specialized function and must work together to fight disease effectively. Because there are many different types of cells that make up the immune system, an error in any one of them can disrupt our immune defenses. Depending on which cell and the type of error that occurs, more than 80 different forms of PI diseases are possible. Some are severe, while others cause few or no symptoms. All of them make people more susceptible to infections and other medical conditions. More boys than girls have PI, and patients’ first symptoms often begin in infancy or later in childhood.

Primary immune deficiency diseases are disorders in which part of the body's immune system is missing or does not function properly. In contrast to secondary immune deficiency disease in which the immune system is compromised by factors outside the immune system, such as with certain medications like chemotherapy or viruses, the primary immune deficiency diseases are caused by genetic or intrinsic defects in the immune system.
There are a wide variety of primary immune deficiencies. There are nearly 100 primary immune deficiency diseases including X-linked Agammaglobulinemia (Bruton's Disease), Common Variable Immune Deficiency (also called Hypogammaglobulinemia), Selective IgA Deficiency, and Severe Combined Immune Deficiency (boy-in-the-bubble disease). Some disorders, such as Selective IgA Deficiency can be quite common, occurring as often as 1/400 individuals. While others, such as Severe Combined Immune Deficiency, can be as rare as one in a million. Untreated primary immune deficiencies may be characterized by frequent life-threatening or chronic infections and debilitating illnesses.

Fauci, M.D., director of the NIAID. “The consortium will help advance our understanding by focusing research priorities, uniting established researchers with new investigators, funding small-scale clinical and preclinical projects, and creating or expanding research resources, including a cell line repository and Web-based patient registry to which all researchers will have access.”

NIAID Initiative Addresses Primary Immune Deficiency Diseases The initiative is co-sponsored by the National Institute of Allergy and Infectious Diseases (NIAID) and the National Institute of Child Health and Human Development. It is an outcome of a 2001 NIH advisory panel's recommendations to accelerate basic and clinical research in primary immunodeficiency diseases and to attract new investigators to the field. "Until now, a relative scarcity of study subjects has limited a researcher's options for investigating these lesser known diseases," says Anthony S. Fauci, M.D., director of the NIAID. "The consortium will help advance our understanding by focusing research priorities, uniting established researchers with new investigators, funding small-scale clinical and preclinical projects, and creating or expanding research resources, including a cell line repository and Web-based patient registry to which all researchers will have access." The registry will build upon a 1998 NIAID-funded registry of the Immune Deficiency Foundation that includes clinical information on individuals with eight primary immunodeficiency diseases. The registry will be accessible to researchers at a secure Web site that protects the privacy of those who are registered. The repository will include DNA from individuals included in the registry for use by investigators on approved research studies.
