Priapism is a painful and potentially harmful medical condition in which the erect penis erection ) does not return to its flaccid state (in the absence of both physical and psychological stimulation) because blood cannot exit the tissues or vessels of the penis . Potential complications include ischemia (swelling), clotting of the blood retained in the penis, and damage to the blood vessels of the penis which may result in an impaired erectile function or impotence. If I haven't already pointed him out, check out Hermes , one I might consider my "brother blog" as it is also a 2nd year resident with many similar fears and emotions as me. I wonder if he would call me his "sister blog?"
Also, check out the hilarious blog by medical transcriptionist Ilyka Damen We all need a good dose of humility and she does a great job at providing perspective to the work that we do. I don't often think about who is at the other end of my dictation, assuming they just transcribe but don't really listen. I wonder what my nickname is among our transcriptionists?

Another Flight
Tonight about 20 minutes before the end of my shift, I flew for a MVC (motor vehicle crash...they are not called accidents anymore) in which an SUV wrapped itself around a tree. One person was killed, the driver was taken by ambulance, a third patient was flown on the chopper and then I hopped on the helicopter and we returned to the same scene to pick up the 4th person who had been entrapped for about 45 minutes.

So in summary, I have to say that I highly respect the ground crews, they do an amazing job, try their hardest to help that patient get well and go out of their way to help me get teh information I want and need. I always try to credit the ground crew in my reports. Great job!

Named after Priapus, the Greek god of fertility, this condition is best treated quickly to prevent painful side-effects.
Priapism is the occurrence of any persistent erection for more than four hours duration in the absence of sexual stimulation. Priapism is named after Priapus, the Greek god of fertility, and the son of Aphrodite, the goddess of love. He was apparently an ugly, satyrlike man with enormous genitalia. He was the god of gardens, bees, goats, and sheep. According to the story, Priapus had a huge tongue, a fat belly, and his penis was so large that he was restricted to the position of scarecrow in the fields. Most erections due to sexual stimulation never approach four to six hours. Priapism is not associated with sexual excitement, at least not initially, and the erection does not subside after ejaculation. Priapism can occur in all age groups, including newborns. Most cases of priapism are clustered between two age groups: between the ages of five to ten and twenty to fifty years. Priapism constitutes a true urologic emergency. Men often joke that they wish they could have a permanent erection, but in reality, men are extremely miserable when this actually happens.

Definition: Priapism is a persistent, unwanted erection of the penis.
Priapism may occur in one of the following ways:
  • There may be repeated, reversible painful erections occurring over several hours (penis becomes erect then erection goes away, then becomes erect again, then erection goes away, etc.) This is also called "stuttering" priapism. There is not a problem with sexual functioning once the episode has gotten better. Repeated episodes of this type of priapism are common and occur in up to 40% of patients. This type of priapism can usually be managed at home. Patients are encouraged to take warm baths, increase fluid intake and empty their bladder (urinate) often. If the episode is not better after 3 hours, notify the doctor. Take Tylenol for pain.
  • There may be a prolonged, painful erection that does not go away for more than several hours. This can last for up to several days or weeks. This type of priapism needs attention by a doctor. Without medical treatment, severe priapism can lead to partial or complete impotence in over 80% of cases. Sometimes a blood transfusion is given during the attack.
  • Persistent penile enlargement or hardening that may last for weeks to years; this type of priapism is usually painless. It usually develops after a long episode of priapism (as described in #2 above). Sexual functioning is often impaired (for example-impotence).

CASE REPORT A 36-year-old man was referred by his general practitioner with a 2½-week history of priapism. Three weeks previously he had slipped on his pedals while cycling, injuring his perineum on the cross-bar of his bicycle. Three days after this he had intercourse and developed a persistent painful erection. The pain improved after a few days and the erection became a little less hard, so he did not seek medical advice immediately. The next day the patient underwent angiography. Selective internal iliac artery catheterization was performed under local anaesthetic by the right femoral route using a Levin catheter. Angiography on this side was normal. The catheter was manoeuvred around the aortic bifurcation and the left internal pudendal artery was selectively cannulated. On contrast injection the artery was noticeably larger than that on the right and immediate staining of the corpora was evident, showing the arterio-cavernosal fistula. Trans-catheter embolization was performed on the left side only using finely diced Gelfoam sponge injected down the internal pudendal artery. This caused little discomfort and almost immediately a degree of detumescence was observed. Over the next 2 days the oedema settled and the patient was discharged. At review 1 week later his penis had returned to a normal appearance and occasional tumescence had occurred.

Its management differs radically from the low flow (venoocclusive) priapism, hence making the differentiation important. High flow priapism can be elegantly treated by transarterial embolisation, resulting in a cure while avoiding the complications of surgery. Case Report A 26-year-old man with an acute straddle injury to the perineum while on a ship had difficulty voiding after the injury and was catheterized. He developed persistent erection after 48 hours. On removal of the urinary catheter 15 days later, urinary stream gradually diminished in caliber and force. On examination, there was sustained erection, turgid corpora with slight swelling and tenderness in the region of the bulbar urethra and anterior perineum. There was no external injury. A micturating cystourethrogram (Fig. 1) revealed a short segment stricture of the bulbar urethra. Color Doppler (Fig2. a,b) showed a 14mm sac with arterial flow in the right corpora cavernosa, being fed by the deep artery of penis. The venous sinusoids of the corpora were distended, and the spectral pattern in the deep artery of penis was highly pulsatile, reversing during the diastolic phase.
