Priapism is a condition in which a male develops a permanent erection. What is Priapism?
Priapism is the persistent, painful erection of the penis. It is relatively common in young males with sickle cell anemia. The onset may be in early childhood or adolescence. Recurrent or severe episodes can result in future impotence. Treatment is often not satisfactory.
Differential Diagnosis
- Priapism. The diagnosis of priapism is usually obvious from the physical examination. Patients usually present with the spontaneous onset of erection which frequently occurs in the middle of the night. Can be precipitated by prolonged intercourse or masturbation. Penis remains erect and is very painful. If there is a history of nocturnal erection, pain and persistence for more than thirty minutes must be present to differentiate priapism from normal nocturnal erections. There are three basic patterns seen with priapism. - Stuttering Priapism. Many patients, especially children, have a pattern of multiple short episodes over a period of days or several weeks. The prognosis is generally good and therapy is conservative.

Priapism occurs when blood gets trapped in the penile shaft, causing a persistent erection of the penis. This condition is painful, and is not caused by sexual desire, but has a variety of different causes, such as medications, injury, even black widow spider bites. A persistent, painful erection needs to be reported to your doctor at once in order to receive medical attention.

Priapism
Definition: Priapism is a persistent, unwanted erection of the penis.
Priapism may occur in one of the following ways:
  • There may be repeated, reversible painful erections occurring over several hours (penis becomes erect then erection goes away, then becomes erect again, then erection goes away, etc.) This is also called "stuttering" priapism. There is not a problem with sexual functioning once the episode has gotten better. Repeated episodes of this type of priapism are common and occur in up to 40% of patients. This type of priapism can usually be managed at home. Patients are encouraged to take warm baths, increase fluid intake and empty their bladder (urinate) often. If the episode is not better after 3 hours, notify the doctor. Take Tylenol for pain.
  • There may be a prolonged, painful erection that does not go away for more than several hours. This can last for up to several days or weeks. This type of priapism needs attention by a doctor. Without medical treatment, severe priapism can lead to partial or complete impotence in over 80% of cases. Sometimes a blood transfusion is given during the attack.

    Doctors warn of a dangerous new method of cocaine abuse: injecting the drug directly into the urinary tract. Physicians from New York Hospital-Cornell Medical Center reported the case of a 34-year-old man who suffered severe bleeding under the skin after pumping cocaine into his urethra. It led to complications that destroyed his penis, nine fingers, and parts of his legs. "They fill an eye dropper or a syringe with a cocaine solution and inject it into the penis," said Dr. Samuel Perry, a professor of clinical psychiatry. The man had injected cocaine before intercourse in an effort to enhance sexual performance. He was admitted to the hospital because his penis had remained erect for three days, resulting in a painful inability to urinate. The medical term for a prolonged erection is "priapism." On his third day in the hospital, the man's erection suddenly subsided. Over the next 12 hours, blood leaked into the tissues of his feet, hands, genitals, back and chest. Blood coagulation caused tissues to die over large areas of the patient's body, and he was transferred to the burn unit of New York Hospital-Cornell Medical Center.

Less commonly, priapism occurs when so much blood flows into the penis, often through a damaged blood vessel, that it cannot drain from the penis, even by veins that function properly.

    Priapism Priapism is defined as a prolonged erection developing in the absence of sexual stimulation and unrelieved by ejaculation. The duration of the erection considered to represent priapism is generally over 4-6 hours. The condition obtains its name from the Greek god of fertility and lust Priapus. Priapism can be classified into 2 main types; venocclusive (synonymous with ischemic, low-flow) and arterial (high-flow, non-ischemic). Proper categorization is essential, as the management of the two conditions is very different, particularly as venocclusive is a urologic emergency while arterial is not. Venocclusive priapism results from failure of blood to drain from the erectile bodies due to prolonged blockage of the draining veins. Venocclusive priapism may result from paralysis (prolonged relaxation) of the erectile smooth muscle (generally due to drugs or toxins) or from sludging of blood (due to blood disorders) with subsequent prevention of venous drainage. Arterial priapism results from unregulated inflow of arterial blood into the erectile bodies (corpora cavernosa) secondary to a joining (fistula) between the main erection (cavernous) artery and the erectile body itself. This generally results from trauma to the perineum or penis.

    Priapism
    is a prolonged and painful erection that can last from several hours up to a few days but the priapismic erection is not associated with sexual thoughts or sexual activity. What happens is that blood flows into the penis, but is unable to drain as it would in a normally flaccid penis. Considering that the penis provides little room for blood to circulate, the blood becomes stagnant after a while, acidifies, and loses oxygen. Without oxygen, the red blood cells become stiff, and even less able to squeeze their way out of the penis. Causes Of Priapism
    In most cases, priapism results from one of two things: certain medications or medical conditions. Under the category of medications, there are two primary culprits. For one, penile injections used to treat some forms of impotence can cause priapism; however, this usually only happens when a man decides, on his own, to increase the dosage. Second, psychiatric medications, such as anti-depressants, seem to cause some cases of priapism; however, how these drugs affect priapism is unclear.
    The second major cause of priapism is certain medical conditions and diseases - generally, any that cause the blood to thicken, or cause red blood cells to lose their flexibility and mobility. Sickle-cell anemia and leukemia are probably the most common conditions that cause priapism.
