Priapism is a rare condition that causes a persistent, and often painful, penile erection.
Description
Priapism is drug induced, injury related, or caused by disease, not sexual desire. As in a normal erection, the penis fills with blood and becomes erect. However, unlike a normal erection that dissipates after sexual activity ends, the persistent erection caused by priapism is maintained because the blood in the penile shaft does not drain. The shaft remains hard, while the tip of the penis is soft. If it is not relieved promptly, priapism can lead to permanent scarring of the penis and inability to have a normal erection.
Causes and symptoms
Priapism is caused by leukemia, sickle cell disease , or spinal cord injury . It has also been associated as a rare side effect to trazodone (Desyrel), a drug prescribed to treat depression. An overdose of self-injected chemicals to counteract impotence has also been responsible for priapism. The chemicals are directly injected into the penis, and at least a quarter of all men who have used this method of treatment for over three months develop priapism.
Diagnosis
A physical examination is needed to diagnose priapism. Further testing, including nuclear scanning or Doppler ultrasound, will diagnose the underlying cause of the condition.

by James Eckman, M.D. and Allan Platt, PA-C Priapism Priapism is the persistent, painful erection of the penis. It is relatively common in young males with sickle cell anemia. The onset may be in early childhood or adolescence. Recurrent or severe episodes can result in future impotence. Treatment is often not satisfactory. Clinical Findings Subjective Data Present Illness . Note time of onset of symptoms, frequency and duration of attacks, time of night, pain, dysuria, frequency, discharge. Association with dreams, sexual activity, or masturbation. Determine response to previous therapy. Objective Data Physical Examination - Vital Signs: Temperature, blood pressure. - Genitourinary: Document size and turgor of penis. Soft glans and ability to urinate indicate little involvement of the corpora spongiosa. Glans involvement indicates tricorporal priapism. Rule out discharge and inguinal adenopathy. - Rectal: Prostate size and tenderness. Laboratory Minimum Evaluation. CBC with differential, reticulocyte count, and urinalysis. Additional Evaluation. Download the entire PDF file for Adobe Click Here to return to Contents Chapter 20 P RIAPISM Priapism, defined as a sustained, painful, and unwanted erection, is a well recognized complication of sickle cell disease (SCD) (1-3). According to one study, the mean age at which priapism occurs is 12 years, and by the age of 20, as many as 89 percent of males with SCD will have experienced one or more episodes of priapism (1). Priapism in males with SCD is due to vaso-occlusion, which causes obstruction of the venous drainage of the penis. Priapism can be classified as prolonged if it lasts for more than three hours or stuttering if it lasts for more than a few minutes but less than three hours and resolves spontaneously; however, such stuttering episodes may recur or develop into more prolonged events. Prolonged priapism is an emergency that requires urologic consultation.

Martin Dr Rob Hicks responds A priapism is a persistent erection of the penis that lasts for four hours or often much longer. Although many men may think this might be a good and desirable thing to have there are a number of reasons why it isn't. Firstly, it may be a real nuisance since priapism can occur when a man doesn't have any sexual desire. Secondly, priapism can damage the erectile tissues of the penis permanently. And thirdly, it can be extremely painful. Priapism occurs because the blood in the penis that forms the erection can't escape. This might be because of prolonged sexual intercourse, medical conditions such as polycythaemia, or drug side effects. Very occasionally drugs used to treat impotence can cause priapism too. The problem of priapism can be relieved immediately by releasing the blood from the penis. A doctor will do this at the local casualty (accident and emergency) department with a needle and syringe.
