  • What Is It? Priapism is an abnormally prolonged, often painful erection. Usually, this erection is not caused by sexual desire or stimulation, and is not relieved by orgasm. Priapism may start after prolonged sexual activity, although this is not believed to cause priapism by itself.     What is priapism?
    Priapism is a persistent erection, usually painful, that is not necessarily associated with stimulation or sexual desire. Much debate has taken place as to the meaning of 'persistent erection', but the current view is that the erection needs to be present for six hours. In practice, the usual recommendation is to seek medical help for a painful erection persisting for more than four hours.
    What causes priapism?
    A normal erection results from relaxation of the muscles within the penis and an increase in arterial blood flow to the penis. The resulting swelling of the muscles compresses the penile veins, thus trapping blood within the penis. After ejaculation, the associated adrenaline surge constricts the arteries and relaxes the pressure on the veins, which leads to loss of erection (detumescence).
    Priapism may be spontaneous or caused by medical treatment (iatrogenic). Some 35 per cent of cases are idiopathic (unknown cause) and 21 per cent are associated with drug therapy or alcohol abuse. Trauma accounts for 21 per cent and blood disorders 8 per cent. Medically, the important issue is whether the condition is associated with high blood flow when there is little risk of tissue damage, or low blood flow when the risk of damage is higher.

    Priapism is a rare condition that causes a persistent, and often painful, penile erection. Description Priapism is drug induced, injury related, or caused by disease, not sexual desire. As in a normal erection, the penis fills with blood and becomes erect. However, unlike a normal erection that dissipates after sexual activity ends, the persistent erection caused by priapism is maintained because the blood in the penile shaft does not drain. The shaft remains hard, while the tip of the penis is soft. If it is not relieved promptly, priapism can lead to permanent scarring of the penis and inability to have a normal erection. Causes and symptoms Priapism is caused by leukemia, sickle cell disease , or spinal cord injury . It has also been associated as a rare side effect to trazodone (Desyrel), a drug prescribed to treat depression. An overdose of self-injected chemicals to counteract impotence has also been responsible for priapism. The chemicals are directly injected into the penis, and at least a quarter of all men who have used this method of treatment for over three months develop priapism. Diagnosis A physical examination is needed to diagnose priapism. Further testing, including nuclear scanning or Doppler ultrasound, will diagnose the underlying cause of the condition.

    I wrote this 2 years ago now I still have more to say and learn about this subject. Priapism is the thing i dread most about having Sickle Cell Anemia.

    I want to learn the best way to deal with it and overcome my fears. not too many patients of sickle cell speak of this in public. the male friends of mines who suffer from this, thank me for speaking out. But, i'm only one voice and wish we all had an understanding of everything relating to sickle cell disease, even the ones we are ashamed of. I will post stuff i learn about Priapism here... when i find anything NEW...

    What is Priapism anyway?
    Priapism is a prolonged and painful erection that can last from several hours up to a few days . The priapismic erection is NOT associated with sexual thoughts or sexual activity. What happens is that blood flows into the penis, but is unable to drain as it would in a normally flaccid penis. Considering that the penis provides little room for blood to circulate, the blood becomes stagnant after a while, acidifies, and loses oxygen. Without oxygen, the red blood cells become stiff and even less able to squeeze their way out of the penis. Thats the normal definition for someone without Sickle Cell Disease. But with sickle cell you can imagine our cells tend to sickle at their own leisure. All the sickled cells need is a place confined enough to get stuck and clog up. Priapism can scar the penis if not treated early enough, and this can lead to impotence. Knowing this potential side effect of this condition may help a male defier seek the help he needs despite the shame and embarrassment. Most of us are raised into a form of Christian morality that views anything sexual as shameful and sinful. There are dangers with this way of thinking. Here is a health condition that involves a painful erected sex organ, and some people are just not prepared to for this, due to their personal attitudes towards human sexuality. Some parents may make decisions towards self-treating their son's Priapism which can be very harmful... ending up scarring a child for life.

    MANAGEMENT AND THERAPY OF SICKLE CELL DISEASE NIH Publication No. 95-2117, Revised December 1995 (Third Edition) National Institutes of Health, National Heart, Lung, and Blood Institute Chapter 19-Priapism Priapism is a persistent, painful penile erection. Three clinical categories are discussed in this chapter, including recurrent acute priapism, acute prolonged priapism, and chronic priapism. RECURRENT ACUTE PRIAPISM This is the most common form of priapism in which the patient gets short attacks that subside spontaneously. The patient usually copes with this problem and may not seek medical advice. The natural history is variable. Most patients will continue to get attacks throughout their lives and will maintain some degree of potency. Others will eventually become impotent. A small group will develop an acute attack that will not subside without active and aggressive treatment in a timely fashion. ACUTE PROLONGED PRIAPISM The painful erection that does not subside after several hours is an emergency and should be treated aggressively. If untreated, the attack will last for several weeks, resulting in total impotence. Even with aggressive treatment, preservation of potency may be compromised. The presenting features are persistent erection, severe pain, and tenderness of the penile shaft. Radionuclear penile scan may show a very low flow, and penile blood gas may have a pO2 of less than 5. CHRONIC PRIAPISM This is a rare form of presentation that may follow an acute episode or may arise de novo. The penis is semierect and there is no pain. Clinically, this results in a progressive increase in the size of the penis, and penile scan shows good blood flow. Penile ultrasound shows dilated deep dorsal arteries in both corporeal bodies, and the patient has difficulty in getting a complete erection. Occasionally these patients may develop an acute episode. Suddenly for no apparent reason the penis becomes rigid and painful. These episodes may affect only a part of the penile shaft.

    Priapism is a persistent, usually painful, erection that lasts for more than four hours and occurs without sexual stimulation. The condition develops when blood in the penis becomes trapped and unable to drain. If the condition is not treated immediately, it can lead to scarring and permanent erectile dysfunction. It can occur in all age groups, including newborns. However, it usually affects men between the ages of 5 to 10 years and 20 to 50 years. There are two categories of priapism: low-flow and high-flow.
    • Low flow This type of priapism is the result of blood being trapped in the erection chambers. It often occurs without a known cause in men who are otherwise healthy, but also affects men with sickle-cell disease, leukemia (cancer of the blood) or malaria. High-flow : High-flow priapism is more rare than low-flow and usually less painful. It is the result of a ruptured artery from an injury to the penis or the perineum (area between the scrotum and anus), which prevents blood in the penis from circulating normally.
