Special Medical Reports The following is part of " Tips From Other Journals " from the September 1, 1997 edition of American Family Physician Diagnosis and Management of Prolonged Penile Erection Priapism, a spontaneous, prolonged, usually painful penile erection, results from abnormal regulation of penile flaccidity. Harmon and Nehra review the diagnosis and management of different types of priapism. High-flow or arterial priapism results from increased arterial inflow into the cavernosal sinusoids, overwhelming venous outflow. This may cause a painless semirigid to rigid erection. Damage to cells is rare because of high oxygenation. Causes of high-flow priapism include idiopathic etiologies and groin or saddle trauma resulting in pudendal artery damage. Management of high-flow priapism is elective. In the flaccid state, vascular inflow is limited because of resistance caused by contracted cavernous arterial branches and sinuses. Neurotransmitters or vasoactive substances relax smooth muscles, with decreased compliance resulting in minimal resistance to incoming blood flow. Expansion of the sinusoidal walls inhibits venous outflow, and an erection occurs. Assessment of priapism includes a detailed history and a physical examination, observing the penile shaft for rigidity and pain. A complete blood count and a sickle cell preparation may be indicated. Medications most commonly associated with priapism include trazodone and chlorpromazine. Urologists treating erectile dysfunction with intracavernosal drug therapy using papaverine, phentolamine and prostaglandin E

What is priapism? Priapism is a prolonged and painful erection that can last from several hours up to a few days. The priapismic erection is NOT associated with sexual thoughts or sexual activity. What happens is that blood flows into the penis, but is unable to drain as it would in a normally flaccid penis. Considering that the penis provides little room for blood to circulate, the blood becomes stagnant after a while, acidifies, and loses oxygen. Without oxygen, the red blood cells become stiff, and even less able to squeeze their way out of the penis. Very few cases of priapism are without an obvious cause. In most cases, priapism results from one of two things: certain medications or medical conditions. Under the category of medications, there are two primary culprits. For one, penile injections used to treat some forms of impotence can cause priapism; however, this usually only happens when a man decides, on his own, to increase the dosage. Second, psychiatric medications, such as anti-depressants, seem to cause some cases of priapism; however, how these drugs affect priapism is unclear. The second major cause of priapism is certain medical conditions and diseases generally, any that cause the blood to thicken, or cause red blood cells to lose their flexibility and mobility. Sickle-cell anemia and leukemia are probably the most common conditions that cause priapism.

Priapism By Christopher Steidle, M.D. WebMD Medical Reference from "The Impotence Sourcebook" Priapism is the occurrence of any persistent erection for more than four hours duration in the absence of sexual stimulation. Priapism is named after Priapus, the Greek god of fertility, and the son of Aphrodite, the goddess of love. He was apparently an ugly, satyrlike man with enormous genitalia. He was the god of gardens, bees, goats, and sheep. According to the story, Priapus had a huge tongue, a fat belly, and his penis was so large that he was restricted to the position of scarecrow in the fields. Most erections due to sexual stimulation never approach four to six hours. Priapism is not associated with sexual excitement, at least not initially, and the erection does not subside after ejaculation. Priapism can occur in all age groups, including newborns. Most cases of priapism are clustered between two age groups: between the ages of five to ten and twenty to fifty years. Priapism constitutes a true urologic emergency. Men often joke that they wish they could have a permanent erection, but in reality, men are extremely miserable when this actually happens. Many physicians have characterized priapism in different ways. I find it simplest to classify it by what caused it. Children with priapism are typically those who have leukemia. In this situation, the white blood cells occlude, or block the outflow of blood from the penis causing priapism. Also, children with sickle-cell disease can be afflicted with priapism. In this situation, the penis receives low oxygen, and therefore, the blood sickles and prevents outflow because of sludging. Other rare causes of priapism in childhood include trauma, either to the penis or to the area underneath the penis known as the perineum. Additionally, spinal cord injuries can cause priapism. Extremely rare causes of priapism include drug side effects, but typically these drugs are not used in children.

Prozac has been a lifesaver for me, but it given me uncomfortable/
painful erections. These happen only in the middle of the night.
I discover them when I get up at night to urinate.
I stopped taking prozac for awhile, and the erections very slowly
subsided after about one month. Then I started prozac again (20
mgs.) and developed the problem again after 7 or so days.
(The only way to get the erections to go down is for me to stand up, and it takes maybe a minute or so for them to go away. Sometimes they come right back if I lie down, for some reason.) Questions: does this sound serious? Could this be a drug interaction?

  • Abnormal male horniness. Priapus was the ancient Greek god of procreation. An erection that won't go down, even after four hours. Up to that long is nothing to worry about. It can be caused by supersensitivity to impotence drugs such as Viagra or prostaglandins. 1 in 500 people have this. It can be caused by overdosing on such drugs. It can also happen naturally. It can be treated in stages. Three extra strength Sudafed should help, apply ice, jog 30 minutes then take a cold shower. If that does not work go to the emergency ward of your nearest hospital. They will first try xylocaine and draining the blood out of the penis with a needle. If that does not work they will try phenylephrine. If that does not work they will try neo-synephrine. If that does not work they will use surgery. If the problem is not treated within 12 hours, permanent damage to the penis may result from lack of oxygen.

    Priapism is a painful and continued erection of the penis in the absence of sexual arousal. Cause - Blood fills the penis and causes engorgement as a complication of an underlying cause such as nerve damage, blood disorder (i.e. Acute Leukemia , sickle cell anemia, pelvic blood clot), inflammation or infection around the male genitalia (i.e. Prostatitis Urethritis , or Cystitis ) or an adverse reaction to certain medications (i.e. trazodone, chlorpromazine, and certain antihypertensives, anticoagulants, and corticosteroids). Diagnosis - Observation of the aforementioned symptoms should prompt an immediate evaluation by a physician. Treatment - Emergency treatment is necessary to avoid permanent damage to the penis. This may include injection of anesthesia into the spinal cord region (if a nerve disorder is evident) or withdrawing blood from the penis directly. Further treatment may be necessary to address the underlying condition. If treatment is delayed the male may loss his ability to sustain a normal erection.     (08-Mar-04) I just started taking trazodone for ensomnia and I'm concerned about priapism. I read that it affects 1 in 10000 people. I take it before going to sleep. I've noticed if I wake up in the middle of the night I am semi erect. Each morning I wake up erect as well and it takes the lenght of my shower for the partial erection to go away. It is not painful and it does go away after I am up but still worries me after reading about priapism. Should I be concerned or is a normal side effect of the drug?

    (10-Mar-04) Priapism is such an unusal and rare side effect I don't think there is enough clinical information to answer your question. It certainly seems to be a relatively safe drug, and your description of erections does not sound alarming to me, but if you find it gets worse, then you might consider switching medicatins.

    P riapism is a urologic disorder and medical emergency with a variety of known etiologies including the use of psychotropic medications. The antidepressant trazodone is the agent most frequently implicated in the precipitation of priapism. Additionally, a number of drugs of abuse including marijuana, ethanol, and cocaine have been known to cause the disorder. It is unknown if drugs may act in an additive or a synergistic manner to cause priapism. We report a case of priapism which occurred following trazodone overdose in an individual actively using cocaine. This case suggests that combined trazodone and cocaine use may pose an additional risk of priapism. Since trazodone is commonly employed as a hypnotic and often chosen for polysubstance abusers due to its low abuse potential, clinicians should be aware of the possible additive risk of priapism in this patient population.
