Introduction

Within primary myositis (or inflammatory myopathies), three main groups can be distinguished, defined according to clinical and immunohistochemical aspects: dermatomyositis (DM), polymyositis (PM) and inclusion body myositis (IBM). Although these three entities exhibit marked clinical and biological polymorphisms, they have in common an immune dysfunction with inflammatory involvement of striated muscles. Their etiologies, still poorly elucidated, associate environmental and genetic factors. Considerable progress in our understanding and management of these diseases has nonetheless been made in the past few years. Epidemiology
PM is a rare connective tissue disease, whose annual incidence is estimated to be 5–10 cases/1,000,000 inhabitants with a prevalence of 6–7 cases/100,000 individuals. In the United States, a seasonal character has been reported, notably for certain subgroups of PM. PM preferentially affects females, with a F/M sex ratio of 2/1. It can occur at any age, but primarily affects adults. Clinical manifestations
Motor deficit

The motor deficit bilaterally, symmetrically and non-selectively affects striated muscle. It is a myogenic deficit, predominantly of proximal muscles, notably the shoulder and pelvic girdles, and cervical muscles. The intensity of the muscle weakness varies from one subject to another, ranging from a simple functional annoyance to a bedridden state. Myalgias, observed in 25–70% of PM patients, can be the major sign. A late mild motor deficit of distal muscles is seen in 25–30% of the patients. Twenty-five to 30% of the patients experience involvement of the striated muscles of the pharynx and the upper part of the esophagus, causing dysphonia, dysphagia and/or deglutition disorders responsible for inhalation pneumopathies that can influence the vital prognosis.

What are the symptoms of polymyositis and dermatomyositis? The main symptom of polymyositis is muscle weakness. The first muscles affected are usually in the hips and thighs, although any muscle in the body can be affected. It may be difficult to climb stairs, get dressed or get out of bed. There also is commonly pain in the joints between the bones. However, the joints are not usually warm or swollen, as with some other forms of arthritis. A deep red (almost purple) rash, usually on the face, scalp, neck and chest, appears with dermatomyositis. Finally, there may be coughing and shortness of breath, and some patients may have trouble swallowing. A claim for polymyositis or dermatomyositis must include a diagnosis of the disease made by a board-certified rhematologist based upon personal examination of the patient. Supporting medical documentation must affirmatively reveal that the following criteria are present:

Polymyositis is the inflammation of several muscle groups. The muscles in the shoulder area and pelvis are the most often affected. In this disease, patients usually have the following symptoms:
* weakness in the hips and shoulders,
* swelling and redness in the joints of the hands and ankles,
* chronic shortness of breath, even at rest, and
* problems swallowing. If you have the above symptoms, but also have a rash, you may have dermatomyositis. Dermatomyositis is basically the same disease, but also with a red rash that can be anywhere on your body. Patients who get this disease are usually young women, between the ages of 23 and 60. Occasionally, children, ages 5 to 15 years of age are diagnosed with polymyositis. Healthcare providers may suspect this disease if you report these symptoms and are in the right age group. Your provider may do the following tests to see if you have this disease:

A personal update
Feel poorly from my prednisone taper. Like wolverine on the x-men with nausea. Had a long post written here but lost it, daen it. Maybe I'll feel better later and post from my pc - this was tapped out on my pda. Link Comments (3) TrackBack

Almost in Remission!!
I got some great news the other day: my CK level is down to 380. As my neurologist told me, if I went out and chopped up a bunch of wood, if I came in and did a blood draw my CK would be 400. Normal is 40-150. I updated my graph again (on the right sidebar). At this rate, I'll be in clinical remission in about 2 weeks. Hurray!! Also, I am scheduled to stop taking one of my medications, Cellcept, in two weeks. I am down to one pill (500mg) per day from 2000mg/d when I went on the stuff back in October. If things continue to go well, I'll also be off of prednisone on May 24th. Right now I'm down to 15mg/d. It's been really hard to taper the prednisone because I am tired and cranky, like the longest stretch of PMS I have ever been through. But I don't want to slow the taper, because I just want to get off of the stuff! As long as the depression doesn't take over my life, I'd rather just weather it and get on with life. It's been about a year now since I got sick, though I didn't know I was sick at the time last April. When I look back, I know I already had the polymyositis. At one time I thought I'd never be better (October, November) and so I am elated that I have done exactly as they said I would - I have about 80% of my functionality back, and almost live a normal life when it comes to daily activity.
