The disease has a gradual onset and generally begins in the second decade of life. Polymyositis rarely affects persons under the age of 18. The most common symptom is muscle weakness, usually affecting those muscles that are closest to the trunk of the body (proximal). Eventually, patients have difficulty rising from a sitting position, climbing stairs, lifting objects, or reaching overhead. In some cases, distal muscles (those not close to the trunk of the body) may also be affected later in the course of the disease. Trouble with swallowing (dysphagia) may occur. Very rarely, the muscles ache and are tender to touch. The disease may be associated with other collagen vascular, autoimmune or infectious disorders

Polymyositis
Polymyositis is an inflammatory myopathy. It is believed to result from an autoimmune reaction, whereby the body's own immune system attacks the muscle cells. This results in the breakdown of muscle. The result of this attack on the muscles is inflammation, with muscle tenderness as an occasional complaint. The onset of symptoms may be acute, but the condition usually progresses slowly. If left untreated, the patient's ability to ambulate may be compromised. Once the diagnosis is established, treatment is straight forward and often successful in reversing the symptoms.

Causes, incidence, and risk factors Return to top The cause of this disorder is unknown. It is thought that an autoimmune reaction or a viral infection of the skeletal muscle may cause the disease. It can affect people at any age, but most commonly occurs in those between 50 and 70 years old, or in children between 5 and 15 years old. It affects women twice as often as men. Muscle weakness may appear suddenly or occur slowly over weeks or months. There may be difficulty raising the arms over the head, rising from a sitting position, or climbing stairs. The voice may be affected through weakness of the throat muscles. Joint pain inflammation of the heart , and lung disease may occur.

Medical Author: William C. Shiel Jr., MD. FACP, FACR What is polymyositis?
Polymyositis is a disease of muscle featuring inflammation of the muscle fibers. The cause of the disease is not known. It begins when white blood cells, the immune cells of inflammation, spontaneously invade muscles. The muscles affected are mostly those closest to the trunk or torso. This results in weakness which can be severe. It is a chronic illness with periods of increased symptoms, called flares or relapses, and decreased symptoms, known as remissions. Polymyositis is slightly more common in females. It affects all age groups, although its onset is most common in middle childhood and in the twenties. Polymyositis occurs throughout the world. Polymyositis can be associated with skin rash and is then referred to as "dermatomyositis." It also can affect other areas of the body and is, therefore, a systemic illness. Occasionally, it is associated with

Polymyositis (cont.)
An electromyograph (EMG) and nerve conduction velocity are electrical tests of muscle and nerves that can show abnormal findings typical of polymyositis as well as exclude other nerve-muscle diseases. Imaging of the muscles using radiology tests, such as magnetic resonance imaging (MRI scanning) can show areas of inflammation of muscle. This can sometimes can be used to determine muscle biopsy sites. A muscle biopsy is used to confirm the presence of muscle inflammation typical only of polymyositis. This is a surgical procedure whereby muscle tissue is removed for analysis by a pathologist, a specialist in examining tissue under a microscope. Muscles often used for biopsy include the quadriceps muscle of the front of the thigh, the biceps muscle of the arm and the deltoid muscle of the shoulder. How is polymyositis treated?

Polymyositis
Polymyositis is an inflammatory disease involving the muscles. This disease most commonly affects middle-aged women. Children under 18 may also be affected. Symptoms - Symptoms include weakness of the shoulder, hip, neck, and throat muscles (the weakness may involve other muscles as well). This may cause frequent falls, difficulty raising the arms or rising from a chair or difficulty speaking or swallowing. Muscle pain commonly accompanies the other symptoms. Some may also exhibit signs of a skin rash, joint pain, or heart and lung disorders. Cause - Polymyositis is an autoimmune disorder (a disorder in which the immune system attacks normal healthy cells) whose cause is not well understood. The disease may be linked to a viral infections in the body. Diagnosis - Observation of the aforementioned symptoms will be noted and should be evaluated by a physician. The evaluation may include a physical examination, blood tests, muscle biopsy, and/or electromyography (EMG-measures muscle contraction). Treatment - This disease is a chronic condition that may get progressively worse leading to confinement to a wheelchair. Spontaneous remissions may occur in some cases. A physician may prescribe corticosteroid drugs until acute, inflammatory symptoms are alleviated. Some cases may respond to medications designed to suppress the immune system (immunosuppressive medications).

Letter from Maureen McGovern Questions and Answers The Immune System ... MDA's Search for a Cause, Treatment and Cure INTRODUCTION
Polymyositis and dermatomyositis are related diseases, both involving inflammation swelling or irritation of the voluntary muscles, those that normally govern movement, such as the muscles of the arms and legs. Myositis means inflammation of muscle. Polymyositis means inflammation of many muscles, and dermatomyositis means inflammation of muscle and skin. The results of either condition are weakened muscles and often muscle pain. These conditions are seldom fatal, but in the most severe cases a person may need a wheelchair or require assistance with activities of daily living. As early as 1863, a disease involving muscle inflammation and a reddish rash on the face and upper body was described and given the name polymyositis. In 1887, an apparent case of polymyositis without any rash was described by the German physician P. Hepp. Another German, H. Unverricht, then proposed using the name dermatomyositis for cases of muscle inflammation with skin changes and polymyositis for those without them. This terminology remains in use today. Both conditions are among the 40 neuromuscular diseases in the Muscular Dystrophy Association's program.
