What is polycystic ovarian syndrome? Women develop polycystic ovarian syndrome (PCOS) when they have too many male hormones and not enough female hormones, making ovulation rare or irregular. Normally a woman ovulates when an egg-containing follicle grows and releases a mature egg. Polycystic ovarian syndrome keeps those follicles from growing, causing them to build up in the ovaries and form small benign estrogen-releasing cysts. The combination of the estrogen and the male hormones throws the production of two other hormones, follicle-stimulating hormone (FSH) and luteinizing hormone (LH), which also helps inhibit ovulation, off balance. Without ovulation, male hormone levels remain high, and the cycle repeats itself. Polycystic ovarian syndrome, also called Stein-Leventhal syndrome, affects between one and 4 percent of women in the United States.

Polycystic Ovarian Syndrome By M. Rosenthal WebMD Medical Reference from "The Fertility Sourcebook" Polycystic ovarian syndrome (PCO) is a classic female infertility problem. About 4% of the general female population suffers from PCO, which accounts for half of all hormonal disorders affecting female fertility. With PCO, your body secretes far too much androgen, which counteracts your ovaries' ability to make enough progesterone necessary for a normal cycle. Your estrogen levels are fine, though, and your levels of lutenizing hormone (LH) are higher than usual, working overtime to try to kick-start the cycle. But the androgen levels interfere with your follicle stimulating hormone (FSH), which you need to trigger progesterone. As a result, your follicles never develop, and instead turn into small, pea-size cysts on your ovaries. Your ovaries can then enlarge. The elevated androgen levels can also cause you to develop facial hair, hair on other parts of your body (this happens in 70% of the cases and is called hirsutism ), or even a balding problem. Acne is another typical symptom, as well as obesity, although women who are thin or of normal weight can also have PCO. Your periods will be irregular, and as a result you might be at greater risk for developing

Background
Polycystic ovarian syndrome is a common cause of anovulation and infertility in women. Women with this syndrome do not ovulate (release eggs) regularly and therefore have irregular menstrual cycles. Their ovaries contain multiple small cystic structures, usually about 4-9 mm in diameter. This gives the ovaries a characteristic "polycystic" (many cysts) appearance on ultrasound. There are several possible ways to attempt ovulation induction in women with polycystic ovaries. The easiest and least complicated method is the use of clomiphene citrate tablets (Clomid, Serophene) Many will be able to get pregnant using clomiphene to induce ovulation. For women that do not ovulate with clomiphene, the "traditional" next step has been to use injectable gonadotropins. About 90% of women that do not ovulate with clomiphene will ovulate with this medication and the majority will get pregnant as well. However, these medications are expensive and there are risks of ovarian hyperstimulation and multiple pregnancy involved. The daily injections and trips to the office for monitoring are also somewhat inconvenient for most women.

Are some doctors not aware of this syndrome? Is it wise for me to get a second opinion or ask to see a specialist? Polycystic Ovarian Syndrome
Polycystic Ovarian Syndrome (also referred to as Stein-Leventhal syndrome, polycystic ovarian disease or hyperandrogenic chronic anovulation) is an endocrine disorder found in 5%-10% women. Estrogen and progesterone are the female hormones produced by the ovaries that cause monthly menstrual cycles to occur. However, a third hormone, testosterone, also is produced by the ovaries, usually in small amounts. Testosterone is in a broad class of hormones called androgens, and it is the dominant sex hormone in men. No one is certain why insulin resistance occurs. It is thought that polycystic ovary syndrome, like most cases of insulin resistance, is caused by an inherited gene defect.
