The following article was adapted from the Multiple etiologies exist for platelet-derived bleeding disorders. The laboratory evaluation of these disorders can range from simple to complex and should initially include a thorough evaluation of the patient's medical history, concentrating on personal and familial bleeding disorders and current medications. With this as a starting point, the algorithms presented here may be helpful in elucidating the underlying etiology for platelet-derived bleeding. Role of platelets in hemostasis Platelets are small (2 µm-diameter), non-nucleated blood cells produced in the bone marrow from megakaryocytes. They are rapidly activated by blood vessel injury and are a crucial component of the primary hemostatic response. In their unactivated state, platelets are roughly discoid in shape and contain cytoplasmic organelles, cytoskeletal elements, invaginating open-canalicular membrane systems, and platelet-specific granules called alpha and dense granules. Platelets have numerous intrinsic glycoproteins attached to the outer surface of their plasma membrane that are receptors for such ligands as fibrinogen, collagen, thrombin, and thrombospondin to von Willebrand factor and fibronectin.

Acquired disorders of platelet number include thrombocytopenia and thrombocytosis. Acquired thrombocytopenia, usually immune-mediated, is the most common hemostatic disorder you will encounter in private veterinary practice. A platelet count should be performed on every animal presenting with clinical signs of hemorrhage.

Acquired thrombopathias should be suspected in animals with specific diseases and treated with certain drugs. Platelets can have increased (resulting in thrombosis) or decreased function (resulting in hemorrhage) in these situations. The risk of bleeding is unpredictable and typically less severe and consistent than inherited disorders. Although abnormal platelet function occurs in these diseases, there may be other causes for the excessive hemorrhage seen in these conditions, such as thrombocytopenia. However, it should be recognised that platelets are not functioning normally in these situations and drugs that may exacerbate this should be avoided in these patients.
Conditions in which there is decreased platelet function include neoplasia, monoclonal gammopathies, infectious disease, hepatic disease, renal disease, pancreatitis, DIC and immune-mediated thrombocytopenia.
  • Neoplasia : In essential thrombocythemia and acute megakaryocytic leukemia, platelet aggregation and adhesion are defective. This can also occur in other neoplastic conditions, including chronic myeloid leukemia.
  • Monoclonal gammopathy : Very high gamma globulin concentrations associated with some lymphoid or plasma cell neoplasms and atypical responses to some infectious diseases (e.g. Ehrlichiosis) can interfere sufficiently with platelet function

    ITxM Diagnostics Chief Science Officer INTRODUCTION Many clinicians are challenged by a patient at risk for bleeding due to the presence of an acquired or functional platelet disorder. These complex disorders are difficult to diagnose in part due to the ambiguities in the laboratory assessment of platelet function. This review will discuss platelet functional activity and the traditional and newer laboratory methods for platelet function analysis. PLATELET FUNCTION The mechanisms by which platelets participate in hemostasis are complex. Extensive reviews are available to detail the mechanisms and pathology of platelet disorders. In brief, platelet activity can be divided into the following functions: 1) Adhesion-Platelets adhere to damaged blood vessels in a process mediated in part by binding of von Willebrand factor to the glycoprotein Ib-IX-V complex on the platelet plasma membrane. 2) Aggregation-This platelet-to-platelet interaction is initiated by many different agonists, which bind to specific receptors on the platelet membrane.

    Platelet dysfunction may be acquired, inherited, or induced by platelet inhibiting agents. It is clinically important to assess platelet function as a potential cause of a bleeding diathesis, especially in critically ill patients who may develop life-threatening hemorrhages. The most common causes of platelet dysfunction are related to uremia, liver disease, von Willebrand’s disease (vWD) and exposure to agents such as acetyl salicylic acid (ASA, aspirin). Current methods to assess platelet function include bleeding time (BT), aggregation studies and whole blood in vitro test systems such as the closure time (CT). CLOSURE TIME TESTING Closure times are performed on a PFA-100, an instrument and test cartridge system in which the process of platelet adhesion and aggregation following a vascular injury is simulated in vitro This system allows for rapid evaluation of platelet function on samples of anticoagulated whole blood.
