    Causes, incidence, and risk factors: Platelets are blood cells that are essential for blood clotting. Platelet disorders can include inappropriate number of platelets (too many or too few), or normal number but inappropriate functioning of the platelets. Any platelet disorder affects blood clotting.
    Disorders of platelet function can be caused by congenital diseases or acquired conditions. Acquired platelet function disorders are disorders of inappropriate platelet function, and that develop as a result of another disease or condition (acquired). In many cases, the platelet count may be normal or even elevated, but evidence of a bleeding disorder will be present. Myeloproliferative disorders such as primary thrombocytopenia polycythemia vera chronic myelogenous leukemia , and myelofibrosis can produce abnormalities in platelet function. Other causes include

    History and examination of... Investigations Management Platelets are produced predominantly by bone marrow megakaryocytes as a result of budding of the cytoplasmic membrane. Megakaryocytes are derived from the haemopoietic stem cell, which is stimulated to differentiate to mature megakaryocytes under the influence of various cytokines, including thrombopoietin. The life span of a platelet is 7-10 days, and the normal count for all ages is 150-450x10 /l Once released from the bone marrow, young platelets are trapped in the spleen for 36 hours before entering the circulation, where they have a primary haemostatic role.     Introduction
    Platelets play a major role in haemostasis, and defects in platelet function may lead to bleeding. These defects may be inherited or acquired - the latter are far more common. Defective platelet function may present clinically with a variety of symptoms indicative of primary haemostatic failure, such as bruises, ecchymoses, epistaxis, gastrointestinal haemorrhage or menorrhagia. Platelet defects usually give rise to a fairly mild bleeding disorder, and patients may present with exzessive bleeding only after surgery.

    Inherited disorders of the platelet membrane
    Glanzmann Thrombasthenia
    Thrombasthenia was first described by Glanzmann in 1918. It is an autosomal recessive defect of GPIIb/IIIa, a member of the Integrine family of cell surface receptors. Although most patients with this defect have no detectable platelet fibrinogen and absent clot retraction, platelets of some patients contain fibrinogen and can support clot retraction. These criteria were used to classify thrombasthenic patients as type I and II, respectively. Platelets of patients with type I have undetectable intact platelet GPIIb/IIIa complex, whereas in type II disease measureable amounts of functional GPIIb/IIIa can be detected.
    Clinical symptoms occur only in patients homozygous for thrombasthenia. The heterozygous condition appears to be asymptomatic, even though these subjects have only a half-normal concentration of GPIIb/IIIa

    Introduction
    Thrombopoietin (Tpo) is a haematopoietic growth factor that stimulates the development of megakaryocyte precursors, megakaryocytes and platelets. The existence of such a factor was already suggested in 1958 (Kelemen et al , 1958) and demonstrated in the early sixties by studies in rats (Odell et al , 1961). However, attempts to obtain thrombopoietin were unsuccessful in the years thereafter.
