Endocrine disorders in pregnancy provide a unique challenge to the practitioner. The introduction of the fetal-placental unit alters the maternal endocrine metabolism and hormonal feedback mechanisms. Disease manifestations may be altered by the pregnancy environment and may be difficult to distinguish from the normal hypermetabolic state of pregnancy. Therapeutic needs for the mother and the fetus may differ, requiring a fine balance. PITUITARY DISORDERS IN PREGNANCY Anterior Pituitary Gland The pituitary enlarges during pregnancy (1) and may become hyperintense on scan (2). This enlargement is primarily due to estrogen-stimulated hypertrophy and hyperplasia of the lactotrophs (3), while gonadotrophs decline in number, and corticotrophs and thyrotrophs remain constant (4). Somatotrophs are generally suppressed and may function as lactotrophs (5). Placental estrogens stimulate prolactin synthesis (6,7), while progesterone also stimulates prolactin secretion (8). Prolactin levels progressively increase approximately 10-fold throughout gestation,(9) then decline postpartum in non-lactating women. Despite this increase, the normal lactotroph continues to respond to TRH and anti-dopaminergic stimulation.

Treatment guidelines Management Comments Non-drug treatment Surgical treatment may be required for removal of neoplasms. Surgery is required for large tumours, pituitary apoplexy, and hormone secreting tumours not controlled on drugs. Drug treatment IV sodium chloride (with glucose if hypoglycaemic) Dextrose 5% in sodium chloride solution 0.9% with additional sodium chloride solution 0.9% until dehydration and hyponatraemia are corrected. Glucocorticoid deficiency: Prednisone oral, 7.5 mg daily These are average maintenance doses. Start these at least 1 day before starting thyroxine. Thyroid deficiency: Titrate dose until FT4 concentration is within the high limits of normal and patient is clinically euthyroid Gonadal hormone deficiency: Woman - as for post menopausal HRT
Intact uterus ( no hysterectomy): Oestrogen/progestogen combination e.g. Norgestrel + estradiol, oral, 1 mg/ 2 mg daily

The posterior (back) lobe of the pituitary gland releases ADH (antidiuretic hormone) and oxytocin (a hormone to contract the uterus during childbirth and stimulate milk production). Antidiuretic hormone (ADH), which is also called vasopressin, helps the kidneys (and body) conserve the right amount of water. For example, when a person becomes dehydrated, more ADH is produced to help the body conserve the water it contains. Lack of ADH leads to too much excretion of water (diabetes insipidus), and too much ADH leads to excessive retention of water by the body (syndrome of inappropriate antidiuretic hormone secretion). Hello I am suffering from a pituitary disorder. I have an ACTH secreting microadenoma. Over the 16 years I have been suffering from symptoms I have done a fair deal of reading on Chronic Fatigue. My symptoms were:
Ongoing lethargy, weight gain, facial and excess body hair growth, headaches, PMT, candida, fast pulse rate, anemia and more. I am wondering how many people out there are put in the awkward position I was - in constantly seeking answers under the Chronic Fatigue banner when what I really needed was a thorough endocrine workup. I should add that I saw 5 endocrinologists over a 12 year period who all pronounced me clear of any pituitary/adrenal disorder. Regards
