A Prospective Study of 34 Consecutive Cases Study objectives: Obesity has many detrimental effects on the respiratory function and may lead to chronic hypoventilation in some patients, an association known as the obesity-hypoventilation syndrome (OHS). In many cases, patients with OHS also have sleep apneas. Hereafter, we describe several features of a cohort (n = 34) of patients with OHS and show the comparisons with a large cohort (n = 220) of patients with obstructive sleep apnea syndrome (OSAS). We compare also OHS patients with a group of patients with the association of OSAS and COPD, also known as "overlap" patients. Design: Descriptive analysis of prospectively collected clinical data.

A number of questions come to mind regarding OHS, its pathogenesis, and relationship to sleep and OSAS. For example, why do a minority of obese subjects develop OHS? What are the relative contributions of abnormalities of chest wall mechanics, respiratory muscle fatigue, and of ventilatory drive in the development of OHS? What role if any does OSAS play in the pathogenesis of OHS? Can we prevent the development of OHS by recognizing those obese patients who are at risk? Next

Obesity hypoventilation syndrome (OHS)
Overview Symptoms Treatment Prevention Definition: Obesity hypoventilation syndrome (OHS) is a condition related to obstructive sleep apnea in which a very obese person does not breathe a sufficient amount of oxygen during sleep or while awake.
Alternative Names: Pickwickian syndrome
Causes, incidence, and risk factors: The cause of OHS in unknown, but it is likely to involve a combination of a disorder of the brain's control over breathing and the effects of massive obesity on the chest wall. With the excess weight of massive obesity, the muscles of the chest wall can have difficulty expanding the thorax enough to exchange air efficiently. This results in a decreased ability to oxygenate the blood and retention of carbon dioxide (see respiratory acidosis ). Affected individuals suffer from chronic fatigue. This is from sleep loss, poor sleep quality, as well as chronic hypoxia (decreased blood oxygen). Morbid (massive or excessive) obesity is the main risk factor.

    Definition
    A group of symptoms that generally accompany massive obesity Description Pickwickian syndrome is a complex of symptoms that primarily affect patients with extreme obesity. The syndrome is named after a character in a Charles Dickens novel, The Pickwick Papers , who seemed to show some of the traits of this disease. The major health problem that occurs in patients with this disease is sleep apnea . This is caused in part by the excess amounts of fatty tissue surrounding the chest muscles. This excess fat places a strain on the heart, lungs, and diaphragm of the patient, making it difficult to breathe. Causes and symptoms The major cause of Pickwickian syndrome is extreme obesity. This obesity places an excessive load on the pulmonary system. The role of genetics is also being studied. Symptoms of Pickwickian syndrome include excessive daytime sleepiness, shortness of breath due to elevated blood carbon dioxide pressure, disturbed sleep at night, and flushed face. The skin can also have a bluish tint, and the patient may have high blood pressure, an enlarged liver, and an abnormally high red blood cell count. Diagnosis Some tests that can be used to diagnose this condition include echocardiography to determine heart enlargement or pulmonary hypertension . Giving the patient multiple sleep latency tests can help give an objective measurement of daytime sleepiness. Magnetic resonance imaging (MRI), computed tomography (CT) scans, or fiberoptic evaluation of the upper airway may also be used.
