I knew Gloria Fallows for about two years before she was admitted to Intensive Care. I first saw her as an outpatient in 1992 when she was 63. Even then she was enormous 275 pounds, five feet two inches and had trouble breathing. Her chief complaint was "shortness of my breath." "Oh Dr. Martin!" she exclaimed back then. "I can't walk from here to there without struggling." She pointed to a wall of my office about ten feet away. "How long Mrs. Fallows? How long's it been this bad?" "Only the past few months. But it seems to get worse each day."

A group of symptoms that generally accompany massive obesity Description Pickwickian syndrome is a complex of symptoms that primarily affect patients with extreme obesity. The syndrome is named after a character in a Charles Dickens novel, The Pickwick Papers , who seemed to show some of the traits of this disease. The major health problem that occurs in patients with this disease is sleep apnea . This is caused in part by the excess amounts of fatty tissue surrounding the chest muscles. This excess fat places a strain on the heart, lungs, and diaphragm of the patient, making it difficult to breathe. Causes and symptoms The major cause of Pickwickian syndrome is extreme obesity. This obesity places an excessive load on the pulmonary system. The role of genetics is also being studied. Symptoms of Pickwickian syndrome include excessive daytime sleepiness, shortness of breath due to elevated blood carbon dioxide pressure, disturbed sleep at night, and flushed face. The skin can also have a bluish tint, and the patient may have high blood pressure, an enlarged liver, and an abnormally high red blood cell count. Diagnosis Some tests that can be used to diagnose this condition include echocardiography to determine heart enlargement or pulmonary hypertension . Giving the patient multiple sleep latency tests can help give an objective measurement of daytime sleepiness. Magnetic resonance imaging (MRI), computed tomography (CT) scans, or fiberoptic evaluation of the upper airway may also be used.

BREATHING DISORDERS DURING SLEEP National Institutes of Health
National Heart, Lung, and Blood Institute "You Can Snore Your Life Away." This sounds more like a joke than a warning. But, in fact, habitual loud snoring is the most common symptom of breathing disorders that occur during sleep. The person who snores not only sleeps restlessly, but also is at risk for serious disorders of the heart and lungs. Snoring can therefore be lifethreatening because it can lead to high blood pressure, irregular heart beats, heart attacks, and sudden death. Normal breathing must continue at all times whether awake or asleep. The act of breathing is an automatic, highly regulated mechanical function of the body. In healthy sleeping individuals, most muscular and neural activities will slow or even shut down but respiration goes on under a neuromuscular "auto pilot." However, if something goes wrong with the auto pilot during sleep, breathing may become erratic and inefficient. Understanding Sleep Sleep is a complex neurological state. Its primary function is rest and restoring the body's energy levels. Repeated interruption of sleep by breathing abnormalities such as cessation of breathing (apnea) or heavy snoring, leads to fragmented sleep and abnormal oxygen and carbon dioxide levels in the blood. Excessive daytime sleepiness and various disorders of the heart, lungs, and the nervous system result.

Definition
Pickwickian syndrome is a disorder that was named after Joe, the fat, red faced boy in Charles Dickens' The Pickwick Papers This syndrome is a combination of obstructive apnea in moderately to severely obese people, most of whom attempt to sleep face up. Description
About 2.5 million people in the U.S. suffer from sleep apnea (episodes of cessation of breathing during sleep) with men outnumbering women. The several conditions often associated with obstructive sleep apnea include obesity and a short thick neck, and reduction in muscle tone of the soft palate, the uvula (the small, conical, fleshy tissue hanging from the center of the soft palate), and the pharynx. The upper airway may be narrowed by enlarged tonsils or adenoids, a deviated nasal septum, nasal polyps, or congenital abnormalities. Even at high altitudes sleep disruption may occur because of low oxygen concentration. The Pickwickian syndrome consists of moderate to extreme obesity, alveolar (a hollow sac or cavity) hypoventilation, prolonged drowsiness, twitching, cyanosis (a dark bluish to purplish coloration of the skin), periodic breathing, secondary polycythemia (excessive red blood cells caused by low oxygen), and right sided heart failure. Details of the origin and development of this obesity hypoventilation syndrome are unknown, as is its true incidence. Studies show that the syndrome can exist in non-obese patients as well.

Alternate Names : Pickwickian Syndrome Definition Obesity hypoventilation syndrome (OHS) is a condition related to (but can occur separately from) obstructive sleep apnea . In OHS, a very obese person does not breathe a sufficient amount of oxygen during sleep or while awake.
Respiratory System The cause of OHS in unknown, but it likely involves a combination of a disorder in the brain's control over breathing and the effects of obesity on the chest wall. With the excess weight of massive obesity, the muscles of the chest wall can have difficulty expanding the chest enough to exchange air efficiently. This results in a decreased ability to oxygenate the blood, and the retention of carbon dioxide (see

Word: Word Starts with Ends with Definition Also known as obesity hypoventilation syndrome, the Pickwickian syndrome is a bit of an anomaly amongst eponymous syndromes as it is neither named for a doctor who first described the condition nor a famous patient who had it. In The Posthumous Papers of the Pickwick Club , originally published in 1837 by Charles Dickens Charles John Huffam Dickens (February 7, 1812 - June 9, 1870) was a British writer of the Victorian era. His books remain in print. Charles was born in Portsmouth, England, to John Dickens, a naval pay clerk, and his wife Elizabeth Barrow. When Charles was five, the family moved to Chatham, Kent. When he was ten, the family relocated to Camden Town in London. He received some education

A most violent and startling knocking was heard at the door; it was not an ordinary double knock, but a constant and uninterrupted succession of the loudest single raps, as if the knocker were endowed with the perpetual motion, or the person outside had forgotten to leave off. . . The object that presented itself to the eyes of the astonished clerk, was a boy - a wonderfully fat boy - habited as a serving lad, standing upright on the mat, with his eyes closed as if in sleep. He had never seen such a fat boy, in or out of a travelling caravan; and this, coupled with the calmness and repose of his appearance, so very different from what was reasonably to have been expected in the inflicter of such knock, smote him with wonder.
