As nicknames go, Pedro Martinez could do better than Typhoid Mary. Forget SARS or West Nile Virus, this could be the epidemic that brings a region to its knees. And anyone who saw the movie "Outbreak" knows two things: Dustin Hoffman is capable of seriously overacting, and infectious disease is no laughing or even mildly entertaining matter. We asked the folks at the Centers for Disease Control headquarters in Atlanta for the full rundown on pharyngitis. They never got back to us, but we've seen enough episodes of "Scrubs" to offer up a quick tutorial as a service to concerned Page 2 readers. By Bob Hohler, Globe Staff, 8/22/2003 Just as their mounting on-field woes posed one of their gravest challenges of the season, the Red Sox yesterday confronted another daunting obstacle when Pedro Martinez was sidelined with severe throat inflammation. ADVERTISEMENT The illness, which came on suddenly the previous night, forced the Sox to replace their ace with Casey Fossum in the finale of a pivotal three-game series against the A's and raised questions about whether Martinez will remain on schedule to face the Yankees in each of Boston's final two series against their archrivals. Martinez was on medication for an abnormally high fever and throat discomfort and was resting at home as the Sox prepared to the face the A's, according to team spokesman Kevin Shea. Team physician Bill Morgan diagnosed the ace's condition as pharyngitis, a severe inflammation of the throat, though the results of a battery of tests were pending, Shea said. The Sox said Morgan was unavailable to discuss the team's prized righthander. Manager Grady Little said Martinez, who leads the American League in ERA (2.32), will pitch again as soon as he is able, but the team could not estimate a recovery period until Morgan evaluates the test results. Pharyngitis can involve either a viral or bacterial infection and can be related to strep throat, among other conditions.

If you have a severe case, you may find it hard to breathe. Most cases of acute pharyngitis last a few days with treatment. If you smoke, face regular exposure to environmental irritants, or have a continuing infection in your sinuses, lungs, or mouth, you may develop chronic pharyngitis, in which your symptoms will come back from time to time. The viral form of pharyngitis usually accompanies a cold, flu, or mononucleosis. Strep throat is the best-known example of a bacterial form of pharyngitis.     Throat anatomy Definition: Pharyngitis is an inflammation of the pharynx that frequently results in a sore throat . It may be caused by a variety of microorganisms. Causes, incidence, and risk factors: Pharyngitis is caused by a variety of microorganisms. Most cases are caused by a virus, including the virus causing the common cold, flu (influenza virus), adenovirus, mononucleosis, HIV, and various others. Bacterial causes include Group A streptococcus , which causes strep throat , in addition to corynebacterium arcanobacterium Neisseria gonorrhoeae Chlamydia pneumoniae, and others. In up to 30% of cases, no organism is identified. Most cases of pharyngitis occur during the colder months during respiratory disease season. It often spreads among family members. Strep throat is a serious cause of pharyngitis. The complications of strep throat can include acute rheumatic fever, kidney dysfunction, and severe diseases such as bacteremia and streptococcal toxic shock syndrome. Symptoms:
    • Sore throat Strep throat may be accompanied by fever , headache, and swollen lymph nodes in the neck Viral pharyngitis may be associated with runny nose and postnasal drip Severe cases may be accompanied by difficulty swallowing and, rarely, difficulty breathing

    BACKGROUND
    A 61-year-old woman with HIV infection and a CD4+ T-lymphocyte count of 280 cells per cubic millimeter presents with a fever, pharyngitis, and dysphagia that have lasted 3 days. Her physical examination is remarkable for an ulcerative lesion with exudates in the left tonsillar fossa. She has no drooling or stridor. The remaining physical findings are notable for generalized lymphadenopathy; somnolence with arousability; and a generalized rash with macules, papules, pustules, and vesicles, which primarily appear on the face, trunk, and abdomen but not palms or soles. A CT scan of the neck is obtained to evaluate for a retropharyngeal abscess. The CT scan reveals an abnormal, asymmetric, soft tissue thickening at the floor of the mouth and at the base of the tongue, with epiglottic swelling and inflammation extending into the soft tissues of the neck. The parotid gland is not involved. The white blood cell count and blood chemistry findings are normal. Hint
