Pharyngitis can resolve without medical intervention.     Citations Evidence from randomized trials was not found for most questions related to the management of sore throats. Four clinical characteristics (no cough, fever higher than 38 degrees C, exudate, and tender cervical nodes) suggest that a case of pharyngitis would be improved by antibiotic treatment. Use of a clinical score for management of GABHS pharyngitis can be recommended on the basis of the rarity of rheumatic fever in modern society, the resources devoted to management of upper respiratory tract illnesses, the volume of antibiotics prescribed, and the emergence of antibiotic resistance as a growing health issue.

Pharyngitis, called sore throat, is the inflammation of the pharynx (throat). The throat extends from the nasal passages above and behind the mouth to the esophagus (tube that carries food to the stomach) in the neck. Pharyngitis occurs most commonly with a viral upper respiratory infection (URI). It is also symptomatic of a number of diseases, including:
  • diphtheria,
  • mononucleosis caused by Epstein-Barr virus,
  • gonorrhea , and
  • acute HIV infection.
Incidence and Prevalence Approximately 40 – 60% of cases of pharyngitis are caused by a virus and about 15% are associated with Streptococcus infection (strep throat). In the United States, children typically average five sore throats per year and Streptococcus infection every 4 years. Adults typically experience two sore throats per year and

    Despite some progress in reducing inappropriate antibiotic use, the rapid growth of antibiotic resistance is still a critical public health issue. If prescribing patterns do not change, Harvard researchers predict that 40% of Streptococcus pneumoniae will be resistant to penicillin and erythromycin by 2004. To help ensure that physicians can effectively combat infectious diseases now and in the future, the Council for Affordable Quality Healthcare (CAQH) is working with the Centers for Disease Control (CDC) and the Alliance for the Prudent Use of Antibiotics (APUA) on Save Antibiotic Strength sm (SAS), a unique partnership aimed at preserving one of our most precious medical resources. Although pharyngitis is one of the most common reasons for antibiotic prescribing, only 15% of pediatric and 10% of adult episodes of pharyngitis are bacterial and helped by taking antibiotics.

    Streptococcal Pharyngitis
    What is streptococcal pharyngitis?
    Streptococcal pharyngitis (sore throat), or "strep throat," is an infection of the throat and tonsils caused by the bacteria Streptococcus pyogenes , also known as Group A beta-hemolytic streptococci (GABS). What are the symptoms of streptococcal pharyngitis? The symptoms of streptococcal pharyngitis include sore throat, pain on swallowing, fever, swollen and tender lymph nodes in the neck, and fatigue. The tonsils are swollen and often covered with pus. The roof of the mouth may have fine red lesions called petechiae. While cough, hoarseness, and runny nose are common symptoms of viral upper respiratory infections, these are NOT symptoms of streptococcal pharyngitis. It is important to realize that most sore throats are not due to streptococcal infections. When strep throat is accompanied by a red rash and fever, it is called scarlet fever. If left untreated strep pharyngitis lasts from 2 to 5 days; with antibiotics it lasts about 1-3 days. The rash of scarlet fever fades after several days but is often followed by flaking or peeling of the skin, especially around the fingertips, 1 to 3 weeks later.

    Treatment of Exudative Pharyngitis
    To the Editor : The dispatch by Izurieta et al. (Emerg Infect Dis 1997;1:65-8) reporting exudative pharyngitis possibly due to Corynebacterium pseudodiphtheriticum was very interesting, especially with the resurgence of diphtheria in the former Soviet Union. However, I was somewhat surprised at the treatment received by the 4-year-old patient whose case is reported. Erythromycin is an effective antibiotic in diphtheria, but it is secondary in importance to diphtheria antitoxin. The presence of a thick grayish white adherent pseudomembrane, adenopathy and cervical swelling, and low grade fever should certainly provoke a high index of suspicion of diphtheria, especially in a child who has not received pediatric immunization. The diagnosis of diphtheria is primarily made presumptively on clinical grounds and confirmed by the recovery of toxigenic Corynebacterium diphtheriae by the laboratory. Antitoxin treatment cannot wait for laboratory confirmation. Prompt administration of antitoxin is important because diphtheria toxin binds rapidly and irreversibly to tissue sites. Delay in initiating antitoxin treatment is associated with increased incidence of myocarditis, paralysis, and death. Also, it would have been good practice to have placed this child in isolation until the diagnosis was established by the laboratory. The primary care physician in this case is indeed fortunate that the patient did not have diphtheria; the results could have been tragic.
