    BILL O'REILLY, HOST: In the Back of the Book segment tonight, the parents of Terri Schiavo are hoping for a miracle. Their daughter has been in a vegetative state for 13 years, as you know. Ms. Schiavo's husband wants her to die, and so do many in the State of Florida.
    But listen to this. In 1995, Kate Adamson was in a similar position. She was in a vegetative state after suffering a stroke.
    For almost 70 days, she was totally unresponsive. Doctors finally pulled her feeding tube. And, for eight days, she was dying.
    Then Ms. Adamson began responding on her own. Doctors quickly put the feeding tube back in, and she recovered.
    Joining us now from Los Angeles is Kate Adamson and her husband, Steven Klugman. Ms. Adamson has written a book called Kate's Journey: Triumph Over Adversity.
    So, in the Schiavo case, Ms. Adamson, you must have very strong feelings about that.

    Preamble Present requirements of health reporting fails to provide an accurate estimate of the incidence and prevalence of worldwide individuals in a persistent vegetative state (PVS). Ten years ago, a prevalence of 2 to 3 per 100,000 was estimated for Japan. It seems likely that the absolute number of such cases has risen appreciably as a consequence of current practices in critical medicine, cardiorespiratory support, parenteral feeding, and control of infections in severely brain damaged patients. How to deal with this emotionally painful, financially costly, and generally unwanted outcome of modern medical treatment is an increasing problem.

    Once qualified clinicians have determined that a person is awake but unaware, the permanence of the vegetative state depends on the nature of the brain injury, the duration of the period of unawareness, and the estimated prognosis. Some persons less than 35 years old with coma after head trauma, as well as an occasional patient with coma after intracranial hemorrhage, may recover very slowly; thus, what appears to be a PVS at one to three months after an event causing coma may in rare cases evolve into a lesser degree of impairment by six months. On the other hand, the chances of regaining independence after being vegetative for three months are vanishingly small. Rare exceptions are claimed, but some of these may have represented patients who entered an unrecognized locked-in state shortly after reawakening from a coma-causing injury. Ultimately, all have been severely disabled.

Clark (2 Apr 2000): The issue of prolonging life is a very complex one, and this issue is brought up in a number of ethical thinkers in the vegetarian movement, most notably Dr. Peter Singer at the Princeton Center for Bioethics (New Jersey). Many handicapped, senior rights, and prolife groups have picketed Singer at Princeton (and elsewhere) and even at Harvard Medical School (where I heard him several weeks ago). As you probably know, even the diagnosis of persistent vegetative state is a difficult one and the brain injury literature is full of information on its frequent misdiagnosis. Almost every person who has sustained a severe brain injury has had their family told by competent medical personnel that their loved one is going to die ... and in the unlikely case that "such an one" should live, s/he would be (likened unto) a "vegetable" with an unacceptable quality of life. Despite this pessimistic diagnosis, the brain injury rehabilitation programs across the United States are full of patients, young and old, who somehow "beat the odds" and recover.

    I am sure most of you have followed the news about Terri Schiavo, a Florida woman who suffered brain damage when she was 26 years old. Last Wednesday (Oct 15) a judge gave an order for the removal of Terri Schiavo’s feeding tube. Unless nutrition and hydration are restored, Terri will die within 10 days to 2 weeks. The media have described Terri's condition as a "Persistent Vegetative State." However there is considerable dispute over that description. According to family members, “Terri recognizes voices and responds. At times, she vocalizes sounds, trying in her best way to speak. Terri is not a brain dead vegetable as characterized by her husband and legal guardian, Michael Schiavo nor a houseplant as implied by his attorney.” On October 14, Terri's parents released a videotape that indicates that Terri is in nothing like a PVS. According to Pete Vere, a canon lawyer who specializes in the rights of mentally and cognitively disabled, “This is just a sad day for every Catholic concerned with the Culture of Life and the rights of those with some form of cognitive or mental disability. Not only is the state sanctioning involuntary euthanasia, but this sets an awful precedent in which society judges an individual by his or her perceived utility. God created us human beings, not human doings. All human life is valuable in His eyes. I am particularly concerned for those who suffer from more severe forms of cognitive and mental disability. Under this new utilitarian ethic and definition of human life, will their right to life be challenged as well? In terms of mental and cognitive capacity, how far are they removed from Terri?”

    The study included 80 patients who were admitted to a trauma and rehabilitation center as a result of closed head injury between 1988 and 1996. The patients were in a subacute vegetative state that continued for at least six to eight weeks following the injury. Full clinical assessment with MRI was performed six to eight weeks after the injury. Clinical assessment, including documentation of the patient's score on the Glasgow Coma Scale, was repeated at two, three, six, nine and 12 months following the injury. International diagnostic criteria were used to define vegetative state; persistent vegetative state was defined as a vegetative state that had endured for at least 12 months following injury. MRIs were interpreted by three independent neuroradiologists who were unaware of the clinical findings or medical histories of the patients. The most common cause of head injury was motor vehicle accident. Patients who remained in a persistent vegetative state did not differ from those who recovered in terms of age, sex or initial score on the Glasgow Coma Scale. The two groups were also comparable in terms of medical complications or surgical intervention for evacuation of subdural or epidural hematoma. Of the 38 patients who recovered, 24 patients showed signs of recovery within three months, and 36 patients showed signs of recovery within six months of the injury. One half of those who recovered from vegetative states had a moderate disability. In general, better outcomes were associated with earlier signs of recovery.
