A condition caused by damage to the nerves in the peripheral nervous system. The peripheral nervous system includes nerves that run from the brain and spinal cord to the rest of the body. Many of these nerves are involved with sensation and feeling things such as pain, temperature and touch. Up to one-third of people with HIV may get some symptoms of peripheral neuropathy. Peripheral neuropathy is usually felt at first as tingling and numbness in the hands and feet. Symptoms can be described as burning, shooting pain, throbbing, aching, and "feels like frostbite" or "walking on a bed of coals." Peripheral neuropathy can sometimes be caused by HIV but is most commonly a side effect of drugs. Sometimes neuropathy can be caused by vitamin deficiencies or diabetes. Diagnosis of peripheral neuropathy is done by a physical exam. Tests of vitamin B12 levels, thyroid function and glucose levels are also used to check for vitamin deficiencies or diabetes. If peripheral neuropathy is caused by a drug, the symptoms usually get better once the drug is stopped, although it can take 6-8 weeks and the pain can actually get worse for a while. Some other conditions can have the same symptoms and be confused with peripheral neuropathy. A symptom of CMV myelitis is a general weakening of the legs and is treated with anti-CMV drugs. Vacular myelopathy can cause numbness in the limbs and weakening of the spinal cord. New treatments are being studied for myelopathy. Information for health professionals on peripheral neuropathy Peripheral neuropathy is the general term that refers to disorders of peripheral nerves. The peripheral nervous system is made up of the nerves that branch out of the spinal cord to different parts of the body. Peripheral neuropathy is a relatively common neurological disorder that results from damage to the peripheral nerves. It can be caused by a number of diseases of the nerves or be the result of systemic illnesses. Many of the peripheral neuropathy's have a well established cause such as diabetes, alcoholism, uremia, AIDs, or nutritional deficiencies. Diabetes is, by far, one of the most common causes of peripheral neuropathy. Other less common causes include exposure to cold or radiation, a few medicines, toxic substances, vascular or collagen disorders, systemic lupus erythematosus, scleroderma and rheumatoid arthritis.
Symptoms of peripheral neuropathy:
The peripheral nervous system consists of motor (to help move) nerves and sensory (to help feel) nerves. Either or both sets of nerves can be affected in neuropathy.

What is a peripheral neuropathy?
The term peripheral neuropathy describes a problem with the functioning of the nerves outside of the spinal cord. The symptoms of a neuropathy may include numbness, weakness, burning pain (especially at night), and loss of reflexes. The pain may be severe and disabling. What causes a peripheral neuropathy?
There are many possible causes of peripheral neuropathy, including:
  • Some of the most common causes include repetitive activities such as typing or working on an assembly line. In this case, the neuropathy may be isolated to the upper extremities, such as with carpal tunnel syndrome.
    Pressure on a nerve can cause a peripheral neuropathy. For example, pressure on a nerve that comes out from the groin to the skin in front of the upper thigh (the lateral femoral cutaneous nerve) can cause burning and tingling in this location. This particular problem is called meralgia paresthetica and can be caused by wearing a tight belt or other restrictive clothing. Additionally, it can result from being overweight or pregnant.

    From Numb Toes and Aching Soles: Chapter 3, "Pain Medications" Introduction Although there is presently no true cure for PN, there are particular neuropathies that can be successfully treated by dealing with their underlying causes . Treatment as used here means attempting to restore the patient to some degree of normalcy, to bring the affected PNer back to where (or as close as possible to where) he or she was before the onset of the disorder. These neuropathies include:
    • diabetic neuropathy, which as noted in Chapter 8 can be controlled most effectively by lowering blood sugar levels;
      neuropathies induced by vitamin deficiencies, toxins and certain drugs, corrected by supplementing the deficiency (covered in Chapter 6) or by removing the causative agent;
      autoimmune and inflammatory neuropathies treated by plasmapheresis, IVIg, or immunosuppressive medications, discussed in Chapter 4;

Description Peripheral neuropathy is a widespread disorder, and there are many underlying causes. The most common worldwide cause of peripheral neuropathy is leprosy . Leprosy is caused by the bacterium Mycobacterium leprae , which attacks the peripheral nerves of affected people. According to statistics gathered by the World Health Organization, an estimated 1.15 million peoplehave leprosy worldwide. Leprosy is extremely rare in the United States, where diabetes is the most commonly known cause of peripheral neuropathy. It has been estimated that more than 17 million people in the United States and Europe have diabetes-related polyneuropathy. Many neuropathies are idiopathic, meaning that no known cause can be found. The most common of the inherited peripheral neuropathies in the United States is Charcot-Marie-Tooth disease , which affects approximately 125,000 persons.

    Symptoms of an autoimmune peripheral neuropathy may include weakness, cramping, decreased tendon reflexes, numbness, tingling, and pain affecting the arms and/or legs. Clinically, peripheral neuropathies are classified according to various characteristics: symmetric or asymmetric, proximal or distal, acute or chronic, slowly progressive or rapid onset, affecting one (mononeuropathy) or many nerves (polyneuropathy). Upon electrophysiological examination, assessment of a neuropathy can be further classified, depending on which part of the peripheral nervous system is affected, such as the axon or myelin sheath. Peripheral neuropathy can be demyelinating, axonal, or both, as determined by electromyography (EMG) and nerve conduction studies (NCS).

    Neurologic AIDS Research Consortium Peripheral Neuropathy Treatment of neuropathic pain such as is encountered in neuropathy is notoriously difficult. Minimizing neurotoxic drugs, optimizing diet, assuring that there are no contributing vitamin deficiencies (especially B and thiamine) are important first steps. Alcohol is often a neurotoxin, and continued heavy alcohol use may worsen symptoms. Routine analgesics such as aspirin and ibuprofen generally provide little relief. Even narcotics may not fully relieve this kind of pain. The most commonly helpful therapy is use of low doses of tricycle antidepressant drugs such as amitriptyline or imipramine, not because they can relieve depression, but because they change the pain processing in the brain, thus providing symptomatic relief for the patient. Newer antidepressants (Prozac and similar drugs) are generally less helpful than the older tricyclic compounds for this indication. Treatment of painful neuropathy is a major current focus for the Neurologic AIDS Research Consortium and of the AIDS Clinical Trial Group. The completed trial of nerve growth factor (ACTG 291) has been reported in Neurology NGF did provide significant relief to pain in patients with neuropathy based on Gracely Pain Scale scores. However, during the 18 week trial, quantitative sensory testing did not document return of function in the peripheral nerves.

    Overview Peripheral neuropathy Neuropathies may affect just one nerve (mononeuropathy) or several nerves (polyneuropathy). Your nerves provide communication between your brain and your muscles, skin, internal organs and blood vessels. When damaged, your nerves can't communicate properly, and that miscommunication causes symptoms such as pain or numbness. Peripheral neuropathy often affects people with diabetes and autoimmune diseases such as rheumatoid arthritis and lupus. Certain vitamin deficiencies, some medications and alcoholism can also damage peripheral nerves.

Neuropathy can be caused by radiation or chemotherapy. Chemotherapy is a common cause of PN in lymphoma patients and survivors. The drugs which tend to cause the most damage are vinblastine (the "V" in ABVD chemotherapy used for Hodgkin's Disease ) and vincristine (used in some regimes for Non-Hodgkin's Lymphoma like the "O" in CHOP ). Both these drugs are of the vinca alkaloid family (derived from the periwinkle plant Catharantus roseus Radiation therapy can also cause neuropathy. These can take the form of brachial plexopathy (arm/shoulder neuropathy), cervical plexopathy (neck), and lumbar-sacral plexopathy (lower back and legs). If diabetes is involved, then there is a condition called diabetic neuropathy. Often a fair percentage of chemotherapy and radiotherapy induced neuropathy is temporary - the nerves recover or regenerate in a matter of weeks.
