    • Symptoms: The most common symptom is severe, burning, aching pain in the feet and legs, which may prevent walking. ( BALA

    • Sensory Neuropathy. The most common type of neuropathy associated with AIDS is called distal symmetrical polyneuropathy (DSPN). This syndrome is a result of the degeneration of the nerves responsible for conducting impulses, and is characterized by numbness and a sensation of pins and needles. DSPN causes few serious abnormalities and mostly results in numbness or tingling of the feet and slowed reflexes at the ankles. It generally occurs with more severe immunosuppression and is steadily progressive. Treatment with tricyclic antidepressants, such as Elavil, relieves symptoms but does not affect the underlying nerve damage.

For example, therapy for peripheral neuropathy caused by diabetes involves control of the diabetes. In cases where a tumor or ruptured disc is the cause, therapy may involve surgery to remove the tumor or to repair the ruptured disc. In entrapment or compression neuropathy treatment may consist of splinting or surgical decompression of the ulnar or median nerves. Peroneal and radial compression neuropathies may require avoidance of pressure. Physical therapy and/or splints may be useful in preventing contractures (a condition in which shortened muscles around joints cause abnormal and sometimes painful positioning of the joints).

    The diagnosis of peripheral neuropathies can be frustrating, time consuming and costly. Careful clinical and electrodiagnostic assessment, with attention to the pattern of involvement and the types of nerve fibers most affected, narrows the differential diagnosis and helps to focus the laboratory evaluation. An algorithmic approach to the evaluation and differential diagnosis of a patient with peripheral neuropathy is presented, based on important elements of the clinical history and physical examination, the use of electromyography and nerve conduction studies, autonomic testing, cerebrospinal fluid analysis and nerve biopsy findings. The underlying cause of axonal neuropathies can frequently be treated; demyelinating neuropathies are generally managed with the assistance of a neurologist. T he incidence of peripheral neuropathy is not known, but it is a common feature of many systemic diseases. Diabetes and alcoholism are the most common etiologies of peripheral neuropathy in adults living in developed countries. The primary worldwide cause of treatable neuropathy is leprosy.

    You may have come up with something which explains your problems and it is
    VERY important that you share this information with bothof your physicians.
    Amyloidosis is a disease in which abnormal protein polymers or aggregates
    are laid dowm in multiple organs, it usually occurrs in response to chronic
    infection or inflammation and given your history of chronic hepatitis you
    would be a likely candidate. Amyloid is laid dowm as I mentioned in multiple organs,including skin, spleen, kidneys and peripheral nerves. In solid organs it can cause enlargement,in peripheral nerves it can cause neuropathy. The diagnosis can be made by a biopsy of a peripheral nerve or any other involved tissue, I would bring this to the attention of your neurologist as it is obviously very relevant ot your subsequent evaluation and treatment.

References Peripheral neuropathy (PN), or damage to the peripheral nerves (nerves outside the brain or spinal chord), is a potential side effect of many anti-HIV therapies or HIV itself and can greatly affect quality of life. It is often caused by a breakdown of the myelin sheath, the coating around nerve fibers that acts as an electrical insulator. Early signs of PN can include a sensation of burning, tingling, or numbness in the fingers or toes. Some people describe an electric shock sensation or a strange plastic or scab-like sensation when something touches their fingers or toes. In severe cases, touching the affected area can feel like touching an open wound. In some cases, there may be a deep soreness or shooting pains in the muscles of the legs and lower arms that may be transient but always affect the same general area. In more serious cases, severe pain and altered feedback in the nervous system may even interfere with walking. Currently, there are no effective treatments that can stop or reverse this nerve damage. The most effective management of PN includes identifying the cause and, if possible, eliminating it. In many cases, the best treatment is simply pain management, and the type of medication used is generally determined by the severity of the PN.

    What is peripheral neuropathy?
    Peripheral neuropathy describes damage to the peripheral nervous system, the vast communications network that transmits information from the brain and spinal cord (the central nervous system) to every other part of the body. Peripheral nerves also send sensory information back to the brain and spinal cord, such as a message that the feet are cold or a finger is burned. Damage to the peripheral nervous system interferes with these vital connections. Like static on a telephone line, peripheral neuropathy distorts and sometimes interrupts messages between the brain and the rest of the body. Because every peripheral nerve has a highly specialized function in a specific part of the body, a wide array of symptoms can occur when nerves are damaged. Some people may experience temporary numbness, tingling, and pricking sensations (paresthesia), sensitivity to touch, or muscle weakness. Others may suffer more extreme symptoms, including burning pain (especially at night), muscle wasting, paralysis, or organ or gland dysfunction. People may become unable to digest food easily, maintain safe levels of blood pressure, sweat normally, or experience normal sexual function. In the most extreme cases, breathing may become difficult or organ failure may occur.
