Duodenal ulcers, which are found lower down in the abdomen, are more common in men. They heal more easily than the gastric type and usually develop just at the beginning of the duodenum. Symptoms The symptoms of peptic ulcers tend to overlap but there is a fairly general pattern.

    What is Helicobacter pylori?
    Helicobacter pylori (H. pylori) is a bacterium (germ) that lives in the inner lining of the stomach. It produces a number of chemicals that damage the lining of the stomach. Once the infection is present, it persists for many years, if not for life.
    How common is Helicobacter pylori infection in Australia?
    About 40% of persons over 40 years of age are infected with this germ compared to less than 10% of children. Most people are infected in childhood and in Australia the risk of becoming infected is now much less than in the past. There is no difference in infection rate between men and women.
    H. pylori is more common in older people and in certain ethnic populations (eg. Middle Eastern, Asian and Eastern European). In general the germ is more common in people from a poorer background, people living in institutions (eg. mentally handicapped) and those who have migrated from countries where infection is more common, especially the developing world. However, any individual may be infected.

Gastric juice produced by the lining of the stomach contains acid and an enzyme (called pepsin) that digests protein in our food. This acidic juice can cause an erosion of the lining of the stomach or duodenum if it is excessive. This lining is normally protected by a layer of thick mucus, like a coating of slime. Once it is broken, the raw area of an ulcer can form. A common modern cause is the use of drugs to treat pain and arthritis, known as non-steroidal anti-inflammatory drugs (NSAIDS). Most likely it's a bacterial infection. And the infection can be treated and permanently eliminated in most people. All it usually takes is 2 weeks of treatment with a combination of antibiotics and a drug that blocks acid production in the stomach. Just a few years ago, most of us believed ulcers were caused by too much spicy food, too much coffee, too much alcohol, and too much stress. But now we know that about 90% of ulcers are caused by a tough little germ called Helicobacter pylori (H pylori) that lives on the lining of the stomach. H pylori is common everywhere in the world, although it does not always cause ulcers. Scientists in Australia were the first to recognize that many people with ulcers had strange, corkscrewlike bacteria in their stomachs. Convincing the world that infection can cause ulcers is no easy task, however. To prove his point, one of the Australian doctors infected himself with H pylori and quickly became sick with typical ulcer symptoms. His experiment led to the first major breakthrough proving ulcers could be cured.

    A generalized irritation of the stomach is called gastritis ; a localized injury that is irritated and inflamed is an ulcer . An ulcer contains dead or damaged tissue. The irritation can be due to infection or to chemical agents. Peptic ulcers include gastric ulcers in your stomach and duodenal ulcers in the first part of your small intestine immediately below the stomach. You may find that they return year after year, sometimes in the same place and in the same season. How bad is it? Peptic gastritis and ulcers can cause you a lot of distress, mostly pain after eating and in the middle of the night. Beyond that, they can cause life-threatening complications such as bleeding, perforation (an abnormal opening) with spreading infection throughout your abdomen and obstruction so that food cannot pass through. What causes it? It was believed until recently that these ulcers were due entirely to stomach acid and other digestive juices that had penetrated your stomach's defenses. Certainly these chemicals have a lot to do with causing ulcers. But not long ago, a physician discovered a bacterium called Helicobacter pylori infecting these ulcers. After the initial skepticism that any germ could actually live in hydrochloric acid wore off, a revolution in treatment occurred that is still evolving. Now, in addition to controlling acid, you may be given antibiotics to eliminate this infection.

Contrary to popular belief, they are not caused by stress, nor are they caused by eating spicy foods. They can be caused by long-term use of anti-inflammatory medicines such as aspirin, ibuprofen, naproxen, ketoprofen and some prescription drugs. However, in 90% of cases, ulcers are caused by infection with a bacteria called Helicobacter pylori
