Pemphigus Foliaceus
General Information
Pemphigus is a group of rare autoimmune blistering diseases of the skin and/or mucous membranes. Our immune system produces antibodies that normally attack hostile viruses and bacteria in an effort to keep us healthy. In a person with pemphigus, however, the immune system mistakenly perceives the cells in skin and/or mucous membrane as foreign, and attacks them. Antibodies that attack one's own cells are called autoantibodies. The part of the cells that are attacked in pemphigus are proteins called desmogleins. Desmogleins form the glue that attaches adjacent skin cells, keeping the skin intact. When auto-antibodies attack desmogleins, the cells become separated from each other. The skin virtually becomes unglued. This causes burn-like lesions or blisters that do not heal. In some cases, these blisters can cover a significant area of the skin.

PEMPHIGUS (Gr. ir~u~tE, a bubble),a skin disease, in which large blebs appear, on a red base, containing a clear or yellowish fluid; the blebs occasion much irritation, and when they burst leave raw ulcerated surfaces. The disease is principally known in unhealthy or neglected children. A variety of the malady, pemp/zigus foliaceous, affects the whole body, and gradually proves fatal. Pemphigus of an acute septicaemic type occurs in butchers or those who handle hides, and a diplococcus has been isolated by William Bullock. The treatment is mainly constitutional, by means of good nourishment, warm baths, local sedatives and tonics. In chronic pemphigus, streptococci have been found in the blebs, and the opsonic index was low to streptococci. Improvement has been known to take place on the injection of a vaccine of streptococci. Reference Interval: Not detected.
Application: Diagnosis of bullous skin diseases; differentiates pemphigus from pemphigoid.
Interpretation: Intercellular cement substance antibodies (also known as desmoglein antibodies) are specific for pemphigus. Titres correlate with disease activity and effect of treatment. Pemphigus-like antibodies occur in burns, drug reactions and other disorders with extensive skin damage.

Pemphigus Vulgaris

Pemphigus is a term used to describe blistering of the skin caused by binding of antibodies to the surface of the cells of the outer layer of the skin, the epidermis. In pemphigus vulgaris, the most common form of pemphigus, there are IgG antibodies that bind to the cell surfaces of epidermis of the skin as well as the epithelium lining mucosal surfaces such as the mouth. As a result, patients develop severe oral ulcerations, and may also have inflammation or erosions of the lining of the eye and eyelids (conjunctiva), the nasal mucosa, or the genital mucosa. Half of the patients also develop blisters or erosions of the skin, often in the head and neck area.
Diagnosis is made by biopsying the skin or oral mucosa. The biopsy specimens show deposition of antibodies on the cell surfaces of the epidermis or epithelium by using specialized immunofluorescence techniques performed in our Dermatoimmunology Laboratory. Our laboratory also uses immunofluorescence techniques to detect antibodies in the blood. The titer of these antibodies may be helpful in evaluating the severity of the disease as well as disease course and response to therapy. Without treatment, the mortality of pemphigus vulgaris approaches 100%. The current mortality rate still ranges from 5 to 25 %. However, this still high mortality rate is due to the use of unjustified massive doses of prednisone ( more than 80 mg/kg/day)

Pemphigus of the skin and mucous membranes is a chronic blistering disease most commonly found in middle-aged persons. There are three major types of pemphigus:
  • Vulgaris (literally meaning common)
  • Foliaceus (featuring superficial blisters)
  • Paraneoplastic (associated with some cancers)
Pemphigus is an autoimmune disorder in which the skin cells are attacked by the patient's own antibodies (immune proteins). This causes a separation between the layers of skin in which fluid subsequently accumulates, producing blisters. The introduction of corticosteroids in recent decades has turned what often had been a fatal disease into one that can be managed quite well. Causes The etiology of pemphigus is largely unknown, although genetic predisposition is thought to be the cause. An endemic form of pemphigus foliaceus has been identified in certain rural areas of Brazil and may be transmitted by the bite of a black fly. In rare cases, a medication can induce an attack. Such cases almost always resolve after the offending drug is discontinued. In paraneoplastic pemphigus, the skin problem is caused by cancer. The underlying cancer usually is lymphoid (lymphoma or leukemia) in origin. Paraneoplastic pemphigus typically resolves after the cancer has been treated successfully. It also can be associated with benign lymph tumors.
