Wat is het?
Pemphigus is een zeldzame blaarvormende ziekte. In Nederland wordt jaarlijks bij ongeveer 120 mensen de aandoening vastgesteld. Patienten met pemphigus zijn meestal van middelbare leeftijd. De blaren kunnen voorkomen op de huid en op de slijmvliezen, zoals het mondslijmvlies. Pemphigus gaat vaak met jeuk gepaard. Er worden 3 soorten pemphigus onderscheiden:
  • pemphigus vulgaris (komt het meeste voor) pemphigus foliaceus paraneoplastische pemphigus (een vorm die wordt gezien bij patienten met kanker)

Hoe ontstaat het?
Pemphigus ontstaat door een autoimmuun reactie, dat is een reactie van het lichaam tegen het eigen weefsel. Bij pemphigus is deze reactie gericht tegen bepaalde bestanddelen van de opperhuid, waardoor de opperhuid plaatselijk loslaat en blaren gaat vormen.
Waarom de autoimmuun reactie ontstaat is nog niet bekend. In sommige gevallen wordt het veroorzaakt door geneesmiddelen (penicillamine, captopril).
Bij pemphigus vulgaris zijn de blaren vrij slap en met vocht gevuld. De blaren gaan door stoten of krabben makkelijk stuk.
De blaren bij pemphigus foliaceus zijn heel slap en gaan al heel snel stuk zodat er al snel oppervlakkige wondjes (erosies) ontstaan.

related terms: pemphigus foliaceus, pemphigus erythematosus What is pemphigus? Pemphigus covers a group of uncommon disorders that occur in dogs. With these conditions, there is an abnormal immune response to normal components of the skin, resulting in separation of cells. This leads to blisters, pustules, and crusting erosions in the skin. There are some similarities to pemphigus in humans, but many significant differences as well. Breed predispositions are recognized for 2 forms - pemphigus foliaceus and the milder pemphigus erythematosus. (There are no breed predispositions recognized for the other 2 forms - p. vulgaris and p. vegetans). How is pemphigus inherited? unknown What breeds are affected by pemphigus pemphigus foliaceus: bearded collie, akita, Doberman pinscher, Newfoundland, Schipperke pemphigus erythematosus: collie, German shepherd and German shepherd crosses For many breeds and many disorders, the studies to determine the mode of inheritance or the frequency in the breed have not been carried out, or are inconclusive. We have listed breeds for which there is a consensus among those investigating in this field and among veterinary practitioners, that the condition is significant in this breed. What does pemphigus Pemphigus foliaceus and erythematosus develop around 4 years of age. P. erythematosus is thought to be a milder form of p. foliaceus. Both conditions begin with pustular, crusty lesions on the face and ears. However with p. foliaceus, the lesions spread to the feet, the groin and other areas; there may be itching and pain; and severely affected dogs may lose their appetite and become depressed.

Histologic clue: "the naked epidermis", i.e. the superficial cell layers of the stratum corneum are gone (due to superficial acantholysis) and the epidermis appears "nude". Confirmation of the diagnosis by direct immunofluorescence (cf. figure). Differential diagnosis: superficial impetigo contagiosa, which may look very similar histologically but contains an abundance of bacteria, immuno-stainable with anti-mycobacterium bovis BCG, which cross-reacts with many other organisms incl. bacteria. What is pemphigus vulgaris?
Pemphigus vulgaris is a rare autoimmune disease that is characterised by blisters and erosions on the skin and mucous membranes, most commonly inside the mouth. It is the most common subtype of pemphigus, accounting for 70% of all pemphigus cases worldwide although it is extremely rare in New Zealand (about one case per million). The other two main subtypes of pemphigus are pemphigus foliaceus and paraneoplastic pemphigus
What causes pemphigus vulgaris?
Pemphigus vulgaris is an autoimmune blistering disease, which basically means that an individual's immune system starts reacting against his or her own tissue. The building block cells of the epidermis are called keratinocytes. These cells are cemented together at special sticky spots called desmosomes. In pemphigus vulgaris immunoglobulin type G (IgG) autoantibodies bind to a protein called desmoglein 3, which is found in desmosomes in the keratinocytes near the bottom of the epidermis. The result is the keratinocytes separate from each other, and are replaced by fluid, the blister.

Hailey-Hailey disease
Hailey-Hailey disease, sometimes called familial benign chronic pemphigus is a rare hereditary blistering skin disease. The Hailey brothers first described it in 1939. Hailey-Hailey disease usually appears in the third or fourth decade, although it can occur at any age. It typically begins as a painful erosive skin rash in the skin folds. Common sites include the armpits, groins, and neck, under the breasts and between the buttocks. The lesions tend to come and go and leave no scars. As the lesions get bigger the centre clears leaving a typical ring shape. If the lesions are present for some time they may become thickened. The skin then tends to macerate leaving quite painful cracks. Secondary bacterial infection, which is not uncommon, can give rise to an unpleasant smell. White bands on the fingernails and pits in the palms can also occur.

Peer Review Status: Internally Peer Reviewed Two of the more important categories of large blister diseases are pemphigus and bullous pemphigoid. Pemphigus involves blistering because of separation between cells within the epidermis, while bullous pemphigoid involves separation of the full thickness of epidermis from the basement membrane. Because of the more superficial location of the epidermal separation in pemphigus, the blisters or bullae in pemphigus tend to be much more fragile than those seen in bullous pemphigoid. PATIENT: 38-year-old man DURATION: Thirty-one years DISTRIBUTION: Lateral aspects of the neck and right axilla HISTORY: The lesions have recurred and remitted since childhood. He notes pruritus with a flare of his condition. Friction seems to exacerbate the eruption. There is a history of recurrent infections. Family history includes two siblings and a paternal uncle with the same condition.

There were no intact vesicles or bullae. LABORATORY DATA: . None HISTOPATHOLOGY: There is epidermal hyperplasia with areas of nearly full-thickness suprabasal acantholysis. DIAGNOSIS: Familial benign pemphigus COMMENT: Familial benign pemphigus, which is known as Hailey-Hailey disease, is an autosomal dominant condition that arises from a disorder in keratinocyte adhesion.[ ] Skin fragility is probably due to abnormalities in desmosomal components and keratin filament attachment to desmosomes.[
