(13-Mar-04) Hi,
I am a 27 year old male and have a cosmetic case of pectus excavatum. It is mild, but I notice it when I turn sideways and look in the mirror.
I play sports and work out on a regular basis, so I don't think it affects my lung capacity. When doctors have seen me with my shirt off, they haven't mentioned anything about it.
I do get self-conscious about it sometimes in dating, etc. Most women don't see it because I wear a shirt most of the time, but when I go to the beach that's when it becomes noticeable.
Since it's cosmetic, should I still get it fixed? Just the thought of repositioning my sternum at my age is scary because I know my bones are in their fully grown state. Thanks for your help.
A Forum-M.D.-KP
(15-Mar-04) In pectus excavatum, it is believed that the heart is displaced to the left side of the chest, and there is a restriction of movement of the heart and lungs. Patients complain of a decrease in stamina and endurance during strenuous exercise (67%), frequent respiratory infections (32%), chest pain (8%) and asthma (7%).
If the pectus deformity is purely cosmetic, you can consider surgical treatment with a thoracic surgery evaluation. However, if the deformity causes heart and lung dysfunction, the surgery should be more strongly considered.

Pectus Excavatum By Daniel A. Degner, DVM, DACVS What is it?
This is a condition of young puppies and kittens The breast bone, also called the sternum is deformed such that the chest cavity size is compromised (see photo below with pen placed in the deformity) Diagnostics Generally only radiographs of the chest are taken If a heart murmur is detected, ultrasound should be considered Treatment If the pectus is mild it is not treated This condition is treated if the patient has symptoms or the chest is compromised, based on radiographs The age of the patient at the time of treatment should be 8 weeks or older; the youngest patient that received treatment was 7 weeks old There are two ways this condition is repaired Surgical removal of the affected portion of the sternum and replacement with a graft Use of a cast with sutures around the sternum (see below) First a fiberglass cast is molded to a normal shape of the bottom of the chest Then sutures are placed around the affected sternabrae (start caudally and the sutures are placed; pull up on the preceding suture so that it will pull the sternum away from the lungs and heart, as the next suture is placed); a piece of plastic (plastic bag) that has the same dimensions as the cast is placed exactly in position on the chest as the cast will normally be seated (the two black lines on the chest denote the hind extend of the cast); markings are made on the plastic template indicate where the sutures should run through the cast

Pectus excavatum
Definition: The descriptive term for an abnormal formation of the rib cage where the distance from the breastbone (sternum) to the backbone (vertebrae) is decreased, resulting in a caved-in or sunken chest appearance.
Alternative Names: Funnel chest
Considerations: Pectus excavatum is a congenital abnormality that can be mild or fairly marked. The child typically has a depression in the center of the chest over the sternum, and this may appear quite deep. It is caused by excessive growth of the costosternal cartilage (the connective tissue joining the ribs to the breastbone), with resulting inward deformity of the sternum.
If pectus excavatum is severe, it may affect the underlying organs (heart and lungs) leading to exercise intolerance or other problems. In addition, the cosmetic appearance of a pectus excavatum may pose a psychological difficulty.
Pectus excavatum may occur as the only abnormality, or in association with other syndromes.
Common Causes:
  • Isolated defect (i.e., not associated with other problems)

    http://www.cardiothoracicsurgery.wustl.edu/healthinfo/Pectus excavatum web.htm
PECTUS EXCAVATUM What is Pectus Excavatum? This term refers to a chest wall deformity resulting in the breast bone (sternum) being sunken in. It is sometimes referred to as “funnel chest”. It usually involves the lower half of the sternum. Although it is most common in the middle of the chest, it may move over to one side, usually the right. What causes Pectus Excavatum? The cause is unknown, although most children with this have had it since birth or early infancy. I suspect it occurs as a result of discoordinated growth between the ribs and the chest in general. If the ribs grow faster than the expansion of the heart and lungs (which push the sternum outward) then the sternum will be pushed inwardly. Once this has occurred, the deformity either stays the same or gets worse. What effect does Pectus Excavatum have on the heart and lungs? For normal day-to-day activities there is no impact on heart or lung function. Most children will have lung capacity that is slightly below average, but still within what is considered a normal range. However, the ability of the heart to pump effectively during strenuous exercise may not be normal.

Subject: Pectus Excavatum and Poland's Syndrome: Surgical Correction

    Important Note
This Clinical Policy Bulletin expresses Aetna's determination of whether certain services or supplies are medically necessary. Aetna has reached these conclusions based upon a review of currently available clinical information (including clinical outcome studies in the peer-reviewed published medical literature, regulatory status of the technology, evidence-based guidelines of public health and health research agencies, evidence-based guidelines and positions of leading national health professional organizations, views of physicians practicing in relevant clinical areas, and other relevant factors). Aetna expressly reserves the right to revise these conclusions as clinical information changes, and welcomes further relevant information. Each benefit plan defines which services are covered, which are excluded, and which are subject to dollar caps or other limits. Members and their providers will need to consult the member's benefit plan to determine if there are any exclusions or other benefit limitations applicable to this service or supply. The conclusion that a particular service or supply is medically necessary does not constitute a representation or warranty that this service or supply is covered (i.e., will be paid for by Aetna) for a particular member. The member's benefit plan determines coverage. Some plans exclude coverage for services or supplies that Aetna considers medically necessary. If there is a discrepancy between this policy and a member's plan of benefits, the benefits plan will govern. In addition, coverage may be mandated by applicable legal requirements of a State, the Federal government or CMS for Medicare and Medicaid members. PECTUS EXCAVATUM M.V .

    Food for Thought...

osteomalacia "weak bones". (1) When osteomalacia occurs in babies or children it's also called "rickets". Yes, rickets. I'll bet you thought that was a disease of the past! Well it's still around, even in Western countries, appearing whenever a marginally nourished body experiences huge demands for body-building materials during growth spurts, during pregnancy and lactation, during the stress of illness, during old age. especially calcium, phosphorous, vitamin D, or the special chemicals made by the body to use these ( especially the enzyme alkaline phosphatase and the calcium-regulating parathyroid hormone, PTH ). How can this happen in our "well-nourished" society? The body may be working fine but the diet is low in the nutrients needed. This especially applies to calcium and vitamin D. Several other nutrients are crucial in connective tissue formation including ascorbic acid (vitamin C) and copper, and deficiencies in both are common. And in folks who for whatever reason get little skin exposure to direct sunlight, one of the two major sources of "raw" (inactive) vitamin D is lost. The diet may provide the nutrients needed but damage to the digestive system blocks their absorption (or the absorption of other nutrients needed to use them). Malabsorption can happen if stomach acid is too low; if digestive enzymes and bile aren't produced in sufficient amounts; or if the lining of the intestine is damaged by chronic inflammation. This happens, for example, in celiac disease, cystic fibrosis, liver disease, Crohn's disease, inflammation of the pancreas, and lactose intolerance. Less dramatic gut wall damage is rampant in our society (most of us "just live with it" without diagnosis) but is enough to reduce our ability to absorb all we might from what we eat. If gut lining damage is significant, the second source for vitamin D is lost.
