Abnormalities of blood flow to the vestibular system are very common causes of vestibular symptoms and are often difficult to distinguish from well-known end organ disorders.

Migraine headache results from dilatation of extracranial and dural arteries, causing stretching of sensitive pain fibers in the walls of the arteries, the symptoms of the aura reflect ischemia secondary to intracerebral. Serotonin is implicated in the pathogenesis of these phenomena.[2]
In order to elicit the appropriate history for a migraine the following must be asked:
1. scotomata, or blind spots
2. teichopsia, or fortification spectra, a zigzag pattern in the visual field 3. flashing (photopsia) or colored lights 4. paresthesia.

Ocular Motility Disorders Nystagmus, Pathologic Nystagmus, Pathologic Nystagmus, Congenital MeSH description Involuntary movements of the eye that are divided into two types, jerk and pendular. Jerk nystagmus has a slow phase in one direction followed by a corrective fast phase in the opposite direction, and is usually caused by central or peripheral vestibular dysfunction. Pendular nystagmus features oscillations that are of equal velocity in both directions and this condition is often associated with visual loss early in life. (Adams et al., Principles of Neurology, 6th ed, p272)

BACKGROUND: Exposure to high-intensity low frequency underwater sound at 160 dB (re 1 P) for 15 continuous minutes resulted in post exposure disequilibrium and visual frame shift (oscillopsia) in a diver, necessitating assessment of vestibular function in subsequent studies.
METHODOLOGY: A neuro-otologic assessment battery was used to study vestibular function in divers exposed to U/W sound. Dynamic platform posturography was used to assess vestibulospinal function (equilibrium under different sensory conditions) and quantitative oculography was used to assess oculomotor performance (Vestibular Ocular Reflex, visual enhanced VOR, VOR suppression, pursuit, and saccades) at the beginning and end of the study. The Dynamic Visual Acuity Test (visual acuity during active horizontal head movements) and the modified posturography test (eyes closed on unstable platform making head movements) were used to rapidly assess diver vestibular function before and after each U/W sound exposure. An U/W video-oculography camera monitored divers' eye movements for pathologic nystagmus (spontaneous primary position nystagmus, particularly vertical or torsional nystagmus) during U/W sound exposure.
