It was on a farm somewhere in southern China where the mystery pneumonia we know as SARS probably began. Once in the cities, the virus spread and - thanks to air travel - is now a flaring epidemic in more than a dozen countries. What is SARS? How did it originate? What are the symptoms? And is it treatable? SARS, which stands for Severe Acute Respiratory Syndrome, is the name of a potentially fatal new respiratory disease only recently recognised by scientists. It is not the name of the micro-organism that causes the disease - this is suspected to be a virus, although exactly which one is still uncertain. Where did SARS come from? The disease was first identified by Dr Carlo Urbani, an infectious diseases specialist for the World Health Organisation who directed public health programs in Cambodia, Laos and Vietnam. Based in Hanoi, the 46-year-old Italian epidemiologist was called in after a visiting Chinese-American businessman, Johnny Cheng, became gravely ill and Vietnamese doctors were mystified by the disease. Urbani visited the man at the French Hospital in Hanoi on 26 February 2003 and, over the next week, took many samples. The disease looked like pneumonia - an acute disease marked by inflammation of the lungs and usually caused by bacteria, viruses or occasionally by chemical irritants. But the 48-year-old patient also had a high fever, cough, shortness of breath and other symptoms that suggested something else. Despite intensive care, Cheng eventually died - but not before he had infected 33 hospital staff and Urbani himself, who finally succumbed to the respiratory disease a month later. All of Vietnam's 62 cases can be traced to the businessman, who had arrived from Hong Kong only three days before being admitted to the hospital.

But convincing customers who've been scared silly by doomsday reports is a whole different matter. Many companies are reacting to such concerns. Others, from banks to utilities, have launched offensives to see just how alarmed their customers are and to reassure them that it will be all systems go next New Year's Day. Canadian Imperial Bank of Commerce, for instance, is surveying customers to determine if they're "ill-informed" or "panicked" about year 2000, said John Burns, a vice president at the Toronto bank. The bank wants to figure out if depositors plan to withdraw significant portions of their savings toward year's end so it can decide how much cash to stock in its automated teller machines during the latter part of December.

PANIC DISORDER
Jane Doe is a healthy woman who has never suffered from a mental illness. One day, she suddenly experiences episodes of shortness of breath, sweating, chest pain, dizziness, nausea, and stomach pain. Fearful that she's having a heart attack, Jane goes to the emergency room. The physician evaluates Jane, reassures her that she's healthy and that "nothing is wrong," and sends her home. Nevertheless, episodes of these symptoms continue and become more and more frequent. Jane even becomes afraid to leave her house out of a fear of having another attack. Finally, Jane sees a mental health professional and receives psychotherapy and medications that resolve these symptoms. Jane suffers from panic disorder, a condition of recurring anxiety attacks that affect one out of ten people. An anxiety attack, also known as a panic attack, involves four of the following symptoms simultaneously: sweating; shaking; shortness of breath; feelings of choking; chest pain; nausea or stomach upset; dizziness or fainting; feeling that oneself or reality is not real (derealization or depersonalization); a fear of dying; a fear of losing control; numbness or tingling in the arms or legs; chills or hot flushes (DSM IV, American Psychiatric Association Press, Washington, DC, 1994). Non-psychiatric conditions, such as alcohol consumptions, thyroid disorders, and diabetes, can cause panic attacks. Heavy exercise, caffeine, tobacco, and hyperventilation can also bring about panic attacks. Absent physical factors that cause panic attacks and without another psychiatric condition such as depression, the diagnosis of panic disorder can be made.
