Overview The pancreas is a long, flat gland that lies horizontally behind your stomach. The head of the pancreas rests against the upper part of the small intestine (duodenum), and its tail reaches toward your spleen. The pancreas has two main functions:
  • Its cells produce digestive juices and enzymes that help break down fats, carbohydrates and proteins (pancreatic exocrine function). Once produced, these juices and enzymes are then transported through a small duct that opens into your duodenum. It secretes the hormones insulin and glucagon into your bloodstream, along with somatostatin, another hormone that helps regulate their function. The primary role of insulin and glucagon is to regulate the metabolism of carbohydrates and to control the level of sugar in your blood.

Acute pancreatitis is a rare finding in childhood but probably more common than is generally realized. This condition should be considered in the evaluation of children with vomiting and abdominal pain, because it can cause significant morbidity and mortality. Clinical suspicion is required to make the diagnosis, especially when the serum amylase concentration is normal. Recurrent pancreatitis may be familial as a result of inherited biochemical or anatomic abnormalities. Patients with hereditary pancreatitis are at high risk for pancreatic cancer. P ancreatitis is a disease process with multiple triggers that may cause activation of proteases within the pancreas. It is rare in children, and the causes are more varied in children than in adults (70 to 80 percent of adult cases are related to either alcohol intake or gallstones). In about 25 percent of childhood cases, the etiology is unknown, but trauma, multisystem disease and drugs account for most identified causes. Illustrative Case

Pancreatitis
Pancreatic disorders occur frequently in the dog and cat. The pancreas is a gland in the abdomen that produces enzymes which aid in the digestion of food. It also produces some hormones such as insulin which are secreted into the blood. If, because of injury or disease, these digestive enzymes become activated within the pancreas before they are released, they will begin digesting the pancreas itself. This self-digestion causes severe inflammation called Pancreatitis which is associated with pain and tenderness of the abdomen. Pancreatitis is usually classified as either acute (occurs suddenly) or chronic (an ongoing condition).
Signs of Pancreatitis
The signs of pancreatitis usually occur suddenly in dogs and cats. You ll need to be aware of the signs so you can get treatment for your pet as soon as possible. A pet with pancreatitis will exhibit some or all of the following signs: lack of appetite, frequent vomiting and diarrhea which may contain blood. A pet may also drink more water than usual and vomit it soon after consumption. Weakness, inability to walk and abdominal tenderness or pain may be present. Body temperature will vary in pets with pancreatitis, but usually the temperature will be higher than normal at the onset of the disease and then fall to below normal as the condition continues. The eyes may become sunken, and the mouth and eyes may become very dry, indicating dehydration. These signs are not unique to pancreatitis; therefore your veterinarian may recommend tests to differentiate pancreatitis from other diseases.

Pancreatitis is an inflammation of the pancreas, an organ that is important in digestion. Pancreatitis can be acute (beginning suddenly, usually with the patient recovering fully) or chronic (progressing slowly with continued, permanent injury to the pancreas). Description Acute pancreatitis occurs when the pancreas suddenly becomes inflamed but improves. Patients recover fully from the disease, and in almost 90% of cases the symptoms disappear within about a week after treatment. The pancreas returns to its normal architecture and functioning after healing from the illness. After an attack of acute pancreatitis, tissue and cells of the pancreas return to normal. With chronic pancreatitis, damage to the pancreas occurs slowly over time. Symptoms may be persistent or sporadic, but the condition does not disappear and the pancreas is permanently impaired. The enzymes which the pancreas normally produces to help digestion in the intestine can attack the pancreas itself. The two common types of pancreatitis are acute and chronic Acute pancreatitis Acute pancreatitis is usually a sudden and severe illness when the pancreas rapidly becomes inflamed. Pancreas enzymes and various poisons (toxins) may enter the blood stream in an acute attack, and injure other organs such as the heart, lungs and kidneys. However, the pancreas can return to virtual normality if the cause of the attack is found and treated. Causes of acute pancreatitis The two commonest causes are gallstones , and too much alcohol. Gallstones commonly form in the gallbladder . However, if a stone moves into the bile duct (through the cystic duct), it can become impacted (stuck) at the exit into the duodenum ( papilla of Vater ). This exit hole is normally shared with the pancreatic duct, so that an impacted stone can block the pancreasand cause pancreatitis. Alcohol causes acute pancreatitis by direct poisoning of the gland. Patients (and pancreases) vary in their sensitivity to alcohol. There is no completely safe level of consumption, and yet many people drink heavily for years without ever developing pancreatitis (or other alcohol-related diseases such as liver and heart disease). We do know that women are more sensitive to alcohol than men, and most authorities recommend 3 or less drinks per day for men, and 2 for women. Once pancreatitis has occurred, alcohol should be avoided completely. Alcohol can aggravate pancreatitis even if it has originally been caused by something else.

What is pancreatitis?
The pancreas is a vital organ which lies on the right side of the abdomen. It has two functions:
1)to produce enzymes which help in digestion of food and,
2)to produce hormones, such as insulin.
When the pancreas becomes inflamed, the disorder is called pancreatitis. It is a disease process that is seen commonly in the dog. There is no age, sex, or breed predisposition.
There are two main forms of acute (sudden onset) pancreatitis: 1) the mild, edematous form and, 2) the more severe, hemorrhagic form. A few dogs that recover from an acute episode of pancreatitis may continue to have recurrent bouts of the acute disease, known as chronic, relapsing pancreatitis. The associated inflammation allows digestive enzymes to spill into the abdominal cavity; this may result in secondary damage to surrounding organs, such as the liver, bile ducts, gall bladder, and intestines.
What causes it?
The cause of pancreatitis is not known; however, there may be several contributory factors. It is often associated with a rich, fatty meal. In some cases, it may be associated with the administration of cortisone; however, some dogs with pancreatitis do not have exposure to either.
Under normal conditions, digestive enzymes produced by the pancreas are activated when they reach the small intestines. In pancreatitis, these enzymes are activated prematurely in the pancreas instead of in the small intestines. This results in digestion of the pancreas itself. The clinical signs of pancreatitis are often variable, and the intensity of the disease will depend on the quantity of enzymes that are prematurely activated.

The pancreas, positioned in the abdominal cavity behind the stomach and between the spleen and duodenum, is both an endocrine and exocrine gland. Alpha, beta, and delta cells, located in the islets of Langerhans on the surface of the pancreas, perform the endocrine function of this gland by secreting hormones directly into the bloodstream. These hormones include: glucagon (from alpha cells), which elevates serum glucose; insulin (from beta cells), which controls metabolism of carbohydrates, proteins, and fats; and somatostatin (from delta cells), which inhibits both glucagon and insulin secretions. The pancreas connects to the digestive system at the level of the duodenum via the ampulla of Vater, where the common bile duct meets the main pancreatic duct. This organ plays a major exocrine role in the digestion of food by secreting the enzymes necessary to break down carbohydrates, fats, and proteins. These enzymes combine with sodium bicarbonate to form pancreatic juice, which is secreted when chyme (partially digested food) is present in the duodenum.
